You can add review structured data to a healthcare website only when it accurately describes reviews visitors can see and the reviewed entity is eligible under Google’s rules. The key limitation for a medical practice is the self-serving review policy: a practice cannot earn Google review stars by marking up reviews about itself on pages using LocalBusiness or another Organization type when it controls those reviews. Putting the same reviews in a third-party widget does not change that.
For an eligible page, the process is to identify the reviewed entity, choose the appropriate review properties, make the marked-up content visible, validate it with Google’s Rich Results Test, and check the deployed page with URL Inspection. Correct markup can make a page eligible for a rich result; it does not guarantee that Google will show stars.

First decide whether Google can show review stars for this page
Structured data describes page content in a format search engines can process. It can help Google understand reviews and ratings, but the existence of a schema.org type or a syntactically correct JSON-LD block does not establish eligibility for a Google review snippet. Google documents which types may qualify and applies additional content and policy requirements.
For healthcare, start with two questions: What entity is being reviewed? and Who controls the reviews? If the page belongs to a clinic, physician’s practice, or healthcare organization and presents reviews about that same entity, the self-serving rule is usually decisive. Google says that when the reviewed entity controls its own reviews, pages using LocalBusiness or another Organization type are ineligible for the review star feature. The rule applies whether the reviews are marked up directly or displayed through a third-party widget. See Google’s Review Snippet documentation.

- [Deep Bass with Clear Vocals] High-performance drivers produce punchy bass while keeping vocals clean and detailed for everyday listening.
- A practice’s own website featuring patient reviews of that practice: do not expect Google review stars from review markup. The practice controls the subject of the reviews, so the self-serving restriction applies.
- A third-party publisher reviewing or collecting ratings about other healthcare businesses: eligibility may be possible if the reviewed entity fits a type Google supports, the ratings are direct and genuine user submissions, and all other requirements are met.
- A physician, clinic, or medical directory page using a different schema type to seek stars: do not assume that a schema.org type such as
PhysicianorMedicalClinicqualifies. Use only a type Google currently documents for review snippets; changing the label does not bypass policy.
Google’s Local Business structured data documentation likewise says review and aggregateRating are recommended for sites that capture reviews about other local businesses. A practice adding patient testimonials about itself is not equivalent to an independent site collecting reviews about other businesses.
Distinguish a review, an aggregate rating, and eligibility
Use Review to describe an individual review. Use AggregateRating to describe a combined rating drawn from multiple reviews. These properties describe different content; neither one overrides Google’s eligibility rules. A practice may have many authentic testimonials and a correctly calculated average yet still be ineligible for Google’s review star feature on its own LocalBusiness or Organization page.
- Individual review: the page shows the specific reviewer-submitted review and its rating, and the markup describes that same review.
- Aggregate rating: the page shows the combined rating and an applicable count, using
ratingCountorreviewCountas appropriate. The value and count must match the underlying ratings represented on the page. - Reviewed item: when review data is nested, include the reviewed item’s name and identify the entity accurately. Do not mark up a physician as another kind of entity merely because that type seems more likely to qualify.
Google’s review snippet documentation lists eligible types, including Product, Recipe, Event, and SoftwareApplication. It also documents LocalBusiness for sites that capture reviews about other local businesses and Organization for sites that capture reviews about other organizations. A vocabulary’s availability in schema.org is not proof that Google supports it for review stars.

Check the content and source of every rating
Google’s content guidance requires the marked-up review or rating to be available to users on the page. Marking up a rating in JSON-LD that visitors cannot see does not satisfy that requirement. The structured data should describe the visible review content, not a separate score stored only for search engines.
Ratings must come directly from users. Do not mark up fabricated reviews, undisclosed incentivized reviews, or an aggregate compiled from reviews copied from other websites. For LocalBusiness and Organization review features, Google also says publishers must not rely on human editors to create, curate, or compile ratings for local businesses. A widget does not make a review eligible if its source, control, or visibility fails the rules.

- Show the review text and its rating when marking up an individual review.
- Show the aggregate score and the count represented by it when marking up an aggregate.
- Keep the visible content and structured values synchronized as reviews are added, removed, or corrected.
- Do not imply that a rating covers all patients, all providers, or a broader period than the reviews actually represent.
- Do not mark up testimonials as user ratings if they do not include a rating or otherwise correspond to the review data being described.
Build the markup only after the eligibility check
Use JSON-LD or another supported structured-data format only after confirming the page qualifies. JSON-LD is a way to express the data; it is not an eligibility workaround. A hypothetical independent directory page that is eligible to collect reviews about other local businesses could represent an aggregate rating with properties such as these:
{
"@context": "https://schema.org",
"@type": "LocalBusiness",
"name": "Example Clinic",
"aggregateRating": {
"@type": "AggregateRating",
"ratingValue": "4.6",
"reviewCount": "28"
}
}
This is an illustrative structure, not a claim about any real clinic or its ratings. Replace the example only with accurate values visible on the page, and use it only where the publisher is eligible to review the business. It is not suitable for a clinic to paste onto its own site to mark up patient ratings of that clinic. If a page describes one individual review rather than an aggregate, its content and markup should describe that individual review instead.

Do not add fields simply because a plugin offers them, and do not make up a rating scale, count, reviewer, or review text. If an entity model does not fit the actual page, revise the entity description or do not pursue review markup for a Google rich result.
Implement and validate in a controlled sequence
- Identify the page and subject. Write down which entity the review is about, which page displays it, and who controls the reviews. For a practice’s own reviews of itself, stop if the goal is Google review stars: the self-serving policy makes the page ineligible under the covered LocalBusiness and Organization types.
- Confirm a Google-supported type. Check the current review snippet documentation rather than relying on a schema plugin’s list of schema.org types. Confirm that the type applies to the entity and that the page’s review relationship fits Google’s requirements.
- Choose the data structure. Select
Reviewfor an individual review orAggregateRatingfor a combined rating. For an aggregate, use the applicable count property,ratingCountorreviewCount, and include the reviewed item’s name when the item is nested. - Match markup to the visible page. Ensure the review text and rating, or the aggregate rating and count, can be seen by visitors on that same page. Check that the marked-up values correspond to the reviews actually displayed.
- Verify authenticity and calculation. Confirm that ratings are genuine direct user submissions and that the displayed aggregate uses the represented reviews. Remove any unsupported, copied, fabricated, or undisclosed incentivized material from the markup.
- Run Google’s Rich Results Test. Test the page or code, resolve critical errors, and inspect warnings to decide whether they matter for the selected type. A passing test checks technical eligibility signals; it does not grant an exemption from content policies.
- Deploy to a small set of pages. Publish the markup where it is genuinely applicable before applying a template sitewide. Review generated output after deployment, particularly if a content-management system or plugin creates structured data automatically.
- Inspect the live URL. Use URL Inspection in Search Console to check how Google sees the deployed page. Ensure it is crawlable and not blocked by
robots.txt, anoindexdirective, or a login requirement. Google says recrawling and reindexing may take several days; a sitemap can help Google discover future changes.
What the validation tools can and cannot tell you
The Rich Results Test is for checking whether the markup has errors that affect rich-result eligibility. A clean result does not verify that the reviews are genuine, that the page is not self-serving, or that every value is visible to users. Those are content and policy questions that require reviewing the page and its review process.
URL Inspection checks the deployed URL as Google can access it. It helps find crawl and indexing obstacles, but it cannot promise that stars will appear. Google says, “When Google finds valid reviews or ratings markup, we may show a rich snippet that includes stars and other summary info from reviews or ratings.” The word “may” is important: markup can be valid and eligible while the search result still has no stars. Google also notes that the actual appearance can differ from a test preview.

Common healthcare review markup problems
Adding patient stars to the practice’s own page
Why it fails: the practice controls reviews about itself, so Google’s self-serving review restriction applies to LocalBusiness and other Organization structured data. Authenticity and valid syntax do not remove that restriction.
What to do: keep patient feedback visible if it serves patients, but do not represent it as eligible for Google review stars on the practice’s own page. Do not change the structured-data type solely to pursue stars.
Embedding an outside review widget and expecting an exception
Why it fails: displaying a third-party widget does not change who controls the reviews about the entity. Google explicitly includes embedded third-party widgets among examples covered by the self-serving policy.
What to do: assess who controls the reviews and what entity is being reviewed, not just where the widget is hosted or which platform supplied it.
Marking up ratings visitors cannot see
Why it fails: a hidden JSON-LD rating does not represent content readily visible to users.

What to do: display the corresponding review or aggregate rating on the same page and keep the visible values consistent with the structured data.
Using a schema type because it exists
Why it fails: schema.org defines types beyond those Google supports for review snippets. A vocabulary type associated with medicine does not automatically qualify for stars.
What to do: choose only from Google’s documented supported review snippet types and only when the actual entity and publisher meet the stated conditions.
Expecting a successful test to guarantee a rich result
Why it fails: a test can detect markup issues, but Google’s display is discretionary and subject to eligibility, content, crawl, and processing conditions.

Choose an implementation by policy fit, not by promises of stars
When evaluating a plugin, developer implementation, or structured-data service, use the same five checks. The tool does not change the rules, so claims that it can “get stars” are not a substitute for verifying the page itself.
- Eligibility: Does the reviewed entity match a type Google documents, and does the review relationship avoid the self-serving restriction?
- Source: Are ratings direct, authentic user submissions rather than imported or editorially compiled ratings?
- Visibility: Can visitors see the exact review or rating represented by the markup on that page?
- Accuracy: Are the entity name, score, and applicable count complete and consistent with the visible content?
- Crawlability: Can Google access and process the deployed page without a robots block, noindex directive, or login barrier?
A plugin may generate syntactically correct structured data while applying it to the wrong entity, hidden content, or an ineligible self-review page. Review the rendered page and the generated markup rather than assuming that installation alone makes the result compliant.
Keep markup accurate as the site changes
Structured data can become inaccurate after a redesign or a change to the review feed. Recheck the relevant pages when the site changes its visible reviews, aggregate calculation, entity naming, template, or access controls. If the page no longer displays the underlying rating, remove or update markup that would otherwise describe content users cannot see.
