Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Build human approval into an AI workflow by defining what the AI is allowed to decide, which cases require review, what evidence reviewers need, and what the system does when review cannot happen. The right design depends on the decision’s consequences, the AI’s autonomy, and the applicable law or policy; not every AI workflow needs a human approval gate.
Start by defining the decision and who has authority
Write down the task, the AI system’s intended role, who owns the final decision, and who is responsible for review, escalation, and suspension. Distinguish among three arrangements: AI makes a decision; AI recommends an outcome for an expert to decide; or AI provides an additional opinion to a human decision-maker. These arrangements give people different levels of control, so “human in the loop” is not a sufficiently precise design description.

NIST’s voluntary AI Risk Management Framework says human roles and responsibilities in decision-making and oversight should be clearly defined and differentiated. See NIST AI RMF Appendix C.

Decide which outputs need review
Set review triggers by considering the people affected, the potential harm of an incorrect outcome, whether the outcome can be reversed, the quality of available evidence, and how independently the AI acts. Increase scrutiny when consequences are serious or hard to undo, evidence is weak, or the system has greater autonomy. Treat this as a risk-based design method, not a universal legal requirement.

For high-risk AI systems covered by the EU AI Act, Article 14 requires human oversight measures proportionate to the risks, level of autonomy, and context of use. That requirement does not automatically apply to every AI system. The consolidated EU text linked here is dated July 27, 2026; confirm applicability, transitional dates, amendments, and relevant national law before relying on a legal interpretation: EU AI Act, consolidated text.
Use these questions to shape the gate:
- Could a mistaken outcome cause significant harm or materially affect someone’s rights or opportunities?
- Can the decision be reversed or corrected in time to prevent harm?
- Are inputs incomplete, conflicting, stale, or outside the conditions in which the system is meant to operate?
- Does the output concern a case where a qualified person must weigh context the model cannot reliably assess?
- Can the review team handle the expected volume without creating unsafe delays or rubber-stamping?
Do not invent a universal confidence score or queue target. Establish thresholds using the actual workflow, validate them, and account for the cost of both missed escalations and unnecessary reviews. The cited guidance does not prescribe a universal numeric threshold or service-level target.
Specify each approval gate
For every gate, define its trigger, required information, reviewer role, authority, response options, and deadline. A useful gate lets a reviewer approve, reject, request more evidence, override the recommendation, or escalate it. Make clear that the AI’s recommendation is not the final decision when a person owns that decision.

For EU AI Act-covered high-risk systems, Article 14 describes oversight capabilities that include understanding system limitations, interpreting outputs, disregarding or reversing them, and intervening or stopping the system safely. Article 26 also requires deployers to assign oversight to people with the necessary competence, training, authority, and support. See the European Commission AI Act Service Desk, Article 26.

Give reviewers relevant input data or references, the output and its rationale where available, known limitations, uncertainty, and information about missing or conflicting evidence. They need enough context to judge the recommendation rather than merely confirm that a model produced it.
Make review substantive, not a confirmation click
Design the interface and process so a reviewer can exercise independent judgment. Make approval and rejection equally clear, require a concise rationale for overrides, and avoid making acceptance the effortless default. Provide training on system limits and on when to seek more information or escalate. Ensure reviewers can actually change the outcome.
In UK guidance on relevant automated decisions, the Information Commissioner’s Office says human intervention must be more than a token gesture and should be carried out by someone with authority and capability to change the decision. For decision-support use, reviewers should actively check, weigh, and interpret the recommendation and be able to go against it. These statements concern the ICO’s UK-specific guidance, not a global rule. Read the ICO’s guidance on individual rights in AI systems.
A nominal human gate can still fail. NIST discusses automation bias and variation in human-AI interaction; the ICO also flags risks involving automation bias and interpretability. Avoid presenting a model output as unquestionable, and monitor whether reviewers routinely accept recommendations without meaningful examination. NIST’s AI RMF is voluntary, not a statute or a replacement for sector-specific requirements; see its AI RMF Playbook.

Define fallback rules before launch
For each failure condition, name the workflow action and the person or team responsible. Do not let an exception silently proceed as an ordinary approval. Common triggers include:
- Missing, invalid, or conflicting input data.
- Low confidence, an out-of-distribution case, or an output that conflicts with other evidence.
- An anomaly, unexpected behavior, or a system outage.
- No qualified reviewer available within the decision window.
- A reviewer who lacks the authority or expertise to handle the case.
Choose a safe response appropriate to the decision: correct or retry the input, request more information, route to a qualified second reviewer, pause for manual handling, defer the decision, or stop the AI pathway. These are practical design patterns based on oversight and intervention guidance; no single fallback is mandated for every AI workflow.
For a high-risk EU AI system, Article 14 requires the oversight design to allow an operator to intervene or interrupt operation through a stop button or similar procedure that brings the system to a safe state. Article 26 also describes a suspension duty in a specific risk circumstance; check the applicable consolidated text for the precise trigger. In the UK context covered by the ICO guidance, grave or frequent mistakes warrant immediate investigation and, if necessary, suspension of the automated system.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesWhen no reviewer is available
Decide in advance whether the case waits, moves to a trained backup, or enters a manual or deferred path. Do not treat reviewer unavailability as implicit approval. Set an escalation owner and a safe state for the workflow; the appropriate choice depends on the consequences of delay and the harm a wrong decision could cause.

Keep a review record and learn from exceptions
Record enough to reconstruct what happened: the model and workflow version, references to material inputs, the output, review assignment, reviewer action and rationale, escalations, final decision, and event times. Apply data-minimization, access, and retention rules that fit the jurisdiction and use. The ICO recommends recording decisions and whether a person requested intervention, expressed a view, contested the decision, and whether it changed.
For EU AI Act deployers, Article 26 requires keeping AI-generated logs under their control for an appropriate period of at least six months, unless applicable Union or national law provides otherwise. That is a scoped legal retention minimum, not a universal recommendation for every organization or workflow. See the Article 26 guidance.
As operational indicators, track review and override rates, complaints, appeal reversals, fallback frequency, and incidents. Investigate recurring overrides or serious errors across the model, input process, thresholds, and interface. In the UK ICO guidance, grave or frequent mistakes call for immediate action and may require suspending the system. Consider whether corrections should inform system improvement, while separately evaluating privacy, bias, and safety effects.

Check the design against the applicable rules
Legal duties depend on jurisdiction, system category, and use. The EU AI Act’s cited human-oversight and deployer provisions concern covered systems, including high-risk systems. The ICO source is UK guidance on individual rights and automated decisions under UK data-protection law. NIST AI RMF is voluntary risk-management guidance. None of these sources establishes that every AI workflow must have human approval or a particular fallback. Identify the rules that apply to the specific decision before deployment, and document the reasoning behind your control choices.
