How Referrals and Prior Authorization Affect Access to a Specialist

A referral may be required for a specialist visit, while prior authorization applies to specified services or treatment. Learn what to verify with your plan.

In U.S. health insurance, a referral and prior authorization are different requirements: a referral directs you to a specialist and may be required for the plan to pay for the visit; prior authorization is the plan’s advance review of a particular service or treatment. Your plan may require one, both, or neither. Check the rules for your exact plan, specialist, and proposed care before you schedule when circumstances allow.

Referral and prior authorization are not the same thing

A referral is generally a direction from a primary care clinician or another authorized clinician to see a specialist. Depending on the plan, getting that referral may be a condition for the plan to pay for the specialist visit. The Centers for Medicare & Medicaid Services (CMS) explains that a patient may need a referral or specific instruction from a primary care provider before seeing a specialist for the health plan to pay for the visit (CMS Coverage to Care Roadmap).

Prior authorization—also called preauthorization, prior approval, or precertification—is a health plan’s advance decision about whether a specified service, treatment, prescription drug, or durable medical equipment meets the plan’s rules, including medical-necessity criteria. It is not simply another name for a referral (CMS overview of referrals and authorization).

For example, your plan could require a referral for a specialist appointment, while a procedure, scan, or medication the specialist recommends may separately require prior authorization. Confirm each requirement independently.

How each requirement can affect specialist access

Referral: permission or direction for the visit

If your plan requires a referral, you may need an eligible clinician to submit or provide it before the specialist visit for the plan to pay. Whether a referral is required depends on the plan and benefit. CMS describes HMOs as plans that may require a primary-care referral for specialist access; point-of-service (POS) plans may also require referrals. These are general descriptions, not a decision about your particular coverage.

Prior authorization: review of a specific service

Prior authorization applies to the service or treatment identified in the request, not automatically to every appointment with a specialist. It can affect when that service proceeds and what coverage the patient can expect. Requirements, decision outcomes, and the consequences of proceeding without approval vary by plan, benefit, and circumstance; there is no single outcome that applies to every case.

Network status: a separate coverage question

Being referred to a specialist or receiving an authorization does not by itself establish that the specialist is in network. CMS says an HMO generally limits coverage to contracted providers and generally does not cover out-of-network care except in an emergency, when prior authorization for out-of-network care has been approved, or when otherwise required by law (CMS Health Insurance Basics). Check the provider’s network status for your exact plan and location, and ask the plan about any applicable exceptions.

  1. Check the network. Ask the plan whether the specialist is currently in network for your exact plan and location. Verify with the specialist’s office as well.
  2. Check the referral rule. Ask whether your plan requires a referral for this specialty or visit, who is authorized to provide it, and who must submit it.
  3. Check for prior authorization. Ask whether authorization is required for the consultation itself or for a proposed procedure, imaging, medication, or other service. Confirm which clinician or facility is responsible for submitting the request.
  4. Confirm the request is complete. Ask what records or clinical information the plan needs and how you can verify that it received the request.
  5. Understand any decision. If authorization is approved, ask which service it covers and whether the approval has an expiration or other limits. Confirm separately whether network, benefit, deductible, or cost-sharing rules affect what you may owe.
  6. Ask about exceptions or review. If care is urgent or the specialist is out of network, ask the plan what exceptions or review process apply to your situation.

An approval is not, by itself, confirmation that every part of the care will be paid for. These checks help clarify the plan’s process but do not guarantee approval or payment. CMS’s consumer guidance explains general concepts; only your plan’s current documents and representatives can address your individual benefits.

What federal prior-authorization changes mean for patients

CMS’s 2024 Interoperability and Prior Authorization final rule, CMS-0057-F, establishes API and process requirements for specified payer groups. They include Medicare Advantage organizations; state Medicaid and CHIP fee-for-service programs; Medicaid managed care plans; CHIP managed care entities; and Qualified Health Plan issuers on the Federally Facilitated Exchanges. CMS says the requirements are intended to improve data exchange and streamline prior authorization (CMS final-rule fact sheet).

The rule does not apply to every insurer, plan, or type of authorization, and it does not abolish prior authorization. CMS states that, by January 1, 2027, impacted payers must make information about non-drug prior-authorization requests and decisions available to patients through the Patient Access API. That is a future implementation deadline, and the requirement covers specified information for the affected payers—not every authorization or every plan (CMS API standards page).

How to compare plans for specialist access

When choosing or reviewing coverage, compare features that shape both access and cost:

  • Specialist network: Which specialists and facilities are in network in your area?
  • Referral rules: Does the plan require a referral for specialist visits, and which clinician can provide it?
  • Authorization rules: Which services or treatments require advance review, and how does the plan communicate its decisions?
  • Exceptions and review: What process does the plan describe for urgent needs or out-of-network care?
  • Cost-sharing: What costs apply to in-network and out-of-network care, including deductibles and other member cost-sharing?

These features vary by plan. Review the current benefit documents and confirm details with the plan rather than assuming that one plan’s referral or authorization rules apply to another.
