How Personalized Cancer Vaccines Are Made From a Tumor Sample

Personalized cancer vaccines begin with tumor tissue, but sequencing, immune-target selection and specialized manufacturing stand between a sample and a study-specific treatment.

A personalized cancer vaccine is designed around features found in one patient’s tumor. In a representative National Cancer Institute (NCI) mRNA protocol, clinicians obtain tumor tissue, sequence it to find expressed mutations, assess which mutation-derived targets may be recognized by the patient’s immune system, and send the selected targets into specialized manufacturing. The details vary by clinical study: this is an investigational treatment process, not a single standard recipe or a vaccine patients can routinely order.

What “personalized” means in this process

A tumor sample does not go straight into a vaccine. It is the starting material for a chain of clinical and laboratory decisions. The goal is to identify differences between the patient’s cancer cells and normal cells, then design a treatment intended to direct the immune system toward some of those differences.

Many personalized cancer-vaccine approaches focus on neoantigens: targets that can arise from mutations in cancer cells. The mutations are individual to a tumor, and not every mutation will make a useful target. A study therefore has to analyze the sample, estimate which candidates could be recognized by the patient’s immune system, and choose which ones to encode or otherwise use in its vaccine format.

“Personalized cancer vaccine” describes a family of investigational approaches, not one universal product. The NCI examples include a patient-specific mRNA vaccine and a separate strategy using tumor-RNA-loaded dendritic cells together with tumor-specific lymphocytes. Their workflows and treatment plans differ.

As Julie Bauman, M.D., deputy director of the University of Arizona Cancer Center, put it: “Personalized cancer vaccines may teach the immune system how cancer cells are different from the rest of the body.” The idea is to focus immune recognition on tumor-associated differences; it does not mean that the vaccine is made by simply injecting tumor tissue back into the patient.

How the tumor sample becomes a set of candidate targets

1. Obtain tumor tissue and, when required, a reference sample

Tissue may come from surgery or a biopsy performed as part of a clinical protocol. The representative NCI mRNA protocol uses resected tumor tissue. Another NCI-listed study permits clinically obtained tissue or suitable stored tumor tissue. Whether previously collected material is usable depends on the particular protocol; the existence of one study allowing stored tissue does not establish that every sample or trial can use it.

Some workflows also use a blood sample as a reference. Comparing tumor information with a patient reference can help distinguish changes associated with the tumor from inherited variation. The sample requirements are set by the study, so having a tumor specimen alone does not guarantee that a person meets a protocol’s laboratory or clinical criteria.

The tumor sample is a source of information for the design process. In an mRNA approach, the selected targets are encoded in a manufactured product; the sample itself is not the finished vaccine. The biopsy or surgery is therefore only one part of the pathway, and the patient’s care team must determine whether the available tissue is suitable for the specific study.

2. Sequence tumor material

In the NCI mRNA example, the laboratory uses both exome sequencing and RNA sequencing. Exome sequencing surveys the protein-coding portions of genetic material for mutations; RNA sequencing helps assess which altered genes are being expressed. That distinction matters because finding a mutation is not the same as establishing that a tumor is making the altered material that a vaccine might target.

The sequencing results are analyzed to identify mutations that could give rise to neoantigens. The outcome at this point is a candidate list, not a finished vaccine formula. Some candidates will be less promising than others, and the study’s method determines how far each candidate is tested before a final selection is made.

In broad terms, the team is trying to move from “a change is present in the tumor” to “this change may produce a target the immune system can recognize.” Sequencing provides evidence for that evaluation, but target selection also depends on the study’s prediction and testing methods. The workflow is not simply a count of mutations followed by a choice of the most common ones.

3. Predict and assess possible neoantigens

Computational tools estimate whether a mutation-derived target could bind to the patient’s HLA molecules and potentially stimulate an immune response. HLA molecules are part of the immune system’s way of presenting material for recognition. Since the patient’s HLA profile is relevant to that estimate, target selection is not simply a search for the most numerous tumor mutations.

In the NCI protocol, computational prediction is supplemented by high-throughput immune screening of mutated epitopes. The amount of laboratory confirmation is protocol-dependent: the cited examples do not establish one mandatory test sequence for every personalized vaccine program. Prediction narrows the field, but it cannot guarantee that a selected target will produce a useful immune response in a patient.

The distinction between prediction and confirmation is important for patients interpreting descriptions of a vaccine. A candidate may look promising based on its sequence and predicted interaction with HLA, yet that does not prove it will prompt a strong response in the body or help control the cancer. As Mathias Vormehr, Ph.D., co-director of Cancer Vaccines at BioNTech, described the logistical challenge: “A highly individualized vaccine combination must be designed and produced within weeks of taking a tumor biopsy.”

How a study chooses targets and designs the vaccine

Once candidate targets have been evaluated, the team selects targets according to the study’s criteria and designs a patient-specific treatment. In the NCI mRNA protocol, selected neoantigens are encoded in an mRNA product. A protocol listing allows selection of up to 15 predicted neoantigens. NCI’s broader explainer describes a different approach that could encode up to 34. Those figures belong to separate examples; neither is a standard target count for personalized cancer vaccines generally.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The difference between those counts reflects distinct examples, not a target-count rule that applies across cancer types or platforms. The number chosen is a protocol-specific design decision. A vaccine with more encoded candidates is not automatically better, and the cited figures do not establish a direct comparison of effectiveness.

The design has to fit the vaccine platform being studied. An mRNA vaccine encodes selected targets in mRNA. Other approaches can use different materials or cells; the cited NCI studies include a tumor-RNA-loaded dendritic-cell approach combined with separately prepared lymphocytes. The sample analysis may inform treatment design in both cases, but the actual product and preparation steps are not interchangeable.

Approaches can be compared along several practical lines: the platform used, how targets are identified, whether the protocol needs tumor tissue alone or a reference blood sample as well, the reported sample-to-dose timeline, and the clinical-trial setting. The NCI mRNA example uses sequencing and computational prediction supplemented by immune screening; the dendritic-cell study describes a different cell-based preparation. The sources do not establish a common timeline or a fair efficacy ranking across these unrelated studies.

What happens during manufacturing and treatment

Patient-specific mRNA product

After the targets are selected and the product is designed, a specialized facility manufactures the individual vaccine. The NCI protocol describes the product as made under current good manufacturing practice (cGMP) conditions. It is then administered to the patient whose tumor sample was used to develop it, according to that study’s treatment schedule.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Because the product is tailored to an individual, this is not simply a standard vaccine taken from stock and matched to a diagnosis. Tissue analysis, target selection, product design, manufacturing, and the clinical plan all have to fit together. The exact manufacturing and dosing details depend on the trial.

The sequence also explains why a vaccine cannot be made merely by deciding which cancer type a patient has. The selected targets are based on analysis of that individual’s tumor and, in the NCI example, the patient-specific immune context. A specialized facility then produces the product under the protocol’s manufacturing conditions; the vaccine is intended for the patient whose material informed its design.

Dendritic-cell and lymphocyte approach

A distinct NCI-listed study describes tumor-RNA-loaded dendritic cells and separately prepared tumor-specific lymphocytes. This illustrates why “personalized vaccine” should not be used as shorthand for “mRNA shot.” The platform can change what is prepared from the sample and how the treatment is delivered. The cited description does not establish that this approach follows the same production steps or schedule as the mRNA protocol.

In general, platform is one of the key differences a patient should ask about when reviewing a study. An mRNA product and a cell-based approach are not interchangeable preparations, even if both are described as personalized cancer vaccines. Study descriptions also matter for understanding which samples are collected and what treatment is administered alongside or as part of the vaccine strategy.

How long the process can take

Published timelines differ because they refer to particular programs and intervals, not a guaranteed turnaround for every patient. In 2022, NCI described production of personalized mRNA cancer vaccines in its examples as taking one to two months after tissue collection. In the head-and-neck cancer trial discussed by NCI, Moderna made each vaccine in about six weeks. A 2023 NIH summary of a pancreatic cancer vaccine study reported an average of about nine weeks from surgery to the first dose. The nine-week figure covers surgery-to-dose, while the NCI estimates describe production; they are not directly interchangeable measures.

Reported interval What it measures Attribution and context
One to two months Production after tissue collection NCI, 2022; personalized mRNA vaccine examples
About six weeks Time to make each vaccine NCI, 2022; head-and-neck cancer trial discussed by NCI
About nine weeks on average Surgery to first dose NIH, 2023; pancreatic cancer vaccine study

Actual timing is tied to the study’s workflow and the patient’s circumstances. Tissue availability, sequencing, immune testing, product design, manufacturing, and the treatment protocol can all affect when a dose is ready or given. A time quoted for one trial should not be used as a promise for another platform or cancer type.

The figures describe different intervals and settings. “Production after tissue collection” does not necessarily include the same steps as “surgery to first dose,” and an average from one pancreatic cancer study is not a universal estimate. Vormehr’s comment about producing a highly individualized combination within weeks captures the aim of rapid development, not a guarantee that every clinical program will finish in the same period.

What early study results do—and do not—show

The timelines and early response observations are evidence from small, specific studies, not proof that personalized vaccines work for every patient. NCI’s 2022 discussion of a head-and-neck cancer group reported two complete responses and five tumors that shrank among the first 10 participants. NIH’s 2023 summary of a small pancreatic cancer study described a strong anti-tumor immune response in half of the participants. These findings come from different studies and should not be combined into a general response rate or used to compare the platforms’ effectiveness.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

An immune response is not automatically the same as durable control of cancer, and an early observation in a small group cannot show how a treatment will perform across a larger or different population. The cited NCI material characterizes these vaccines as investigational and discusses early trial observations. A patient’s care team can explain what a particular study measures, what is known about its stage and design, and how trial treatment fits with other care.

For example, the head-and-neck report’s two complete responses and five shrinking tumors refer to the first 10 participants discussed by NCI, not to every participant in all personalized-vaccine trials. Likewise, the pancreatic-study finding that half the participants had a strong immune response is an immune finding in that small study, not a general prediction about a patient’s chance of benefit. Study size, tumor type, platform, and measured outcome all shape what those observations can mean.

Why a tumor sample does not guarantee a vaccine

  • Not every mutation is a target. A mutation must be assessed for expression and potential immune recognition; the presence of a tumor mutation alone does not establish that it should be included.
  • Predictions are not guarantees. Computational estimates and any laboratory screening help select candidates, but do not ensure that a vaccine will trigger a clinically useful response.
  • Eligibility and material requirements are protocol-specific. One study’s acceptance of stored tissue or use of a particular reference sample does not establish that another study will accept the same material.
  • Platforms differ. An mRNA product and a cell-based approach can involve different manufacturing and treatment workflows.
  • Availability is tied to clinical development. The cited examples describe clinical protocols and investigational development, not a routine consumer service or an over-the-counter vaccine.

Personalized vaccine development also depends on a coordinated clinical and laboratory process: obtaining usable material, analyzing it, choosing targets, manufacturing the study product, and administering it through the clinical plan. A tumor sample by itself does not mean a vaccine is automatically available, that a suitable candidate target will be found, or that the treatment will be appropriate for a particular patient.

For someone considering participation, the practical next question is not whether a personalized vaccine exists in the abstract, but whether a relevant clinical study is open and whether its eligibility, tissue, testing, and treatment requirements fit that person’s situation. The treating oncology team or the trial team can clarify those study-specific details.

Questions readers often ask

Is a personalized cancer vaccine made directly from the tumor?

The tumor provides material for analysis, but the sample is not simply turned into an injection. In the NCI mRNA example, sequencing and target assessment guide the design of a patient-specific mRNA product. Other study platforms, such as tumor-RNA-loaded dendritic cells with separately prepared lymphocytes, use different preparations.

How long does a personalized cancer vaccine take to make?

There is no single turnaround time for every program. NCI reported one to two months for production in personalized mRNA examples in 2022, and about six weeks for each vaccine in the head-and-neck trial it discussed. NIH reported an average of about nine weeks from surgery to first dose in a pancreatic cancer study in 2023. These figures describe different programs and intervals.

Can a patient get one outside a clinical trial?

The cited NCI examples describe investigational clinical protocols, not a routine consumer service or an over-the-counter vaccine. Whether a study is available and whether a person qualifies depend on that study’s requirements. An oncology team or trial team can explain current options and eligibility.

Does finding a tumor mutation mean it will be included?

No. A mutation is only a candidate. Teams assess whether it is expressed and whether the mutation-derived target may be recognized in the patient’s immune context. Computational prediction and, in some protocols, immune screening help narrow the list, but neither guarantees clinical benefit.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do all personalized cancer vaccines use mRNA?

No. The cited NCI examples include a patient-specific mRNA product and a separate approach involving tumor-RNA-loaded dendritic cells and tumor-specific lymphocytes. Platform, sample handling, manufacturing, and treatment plans can differ from one study to another.

The Bottom Line

A personalized cancer vaccine is designed through a study-specific sequence of tumor sampling, mutation analysis, neoantigen assessment, target selection, and specialized manufacturing. NCI examples report production periods of roughly six weeks to one or two months for certain mRNA programs, while a separate NIH pancreatic cancer study reported about nine weeks on average from surgery to first dose. These are not universal timelines or proof of broad efficacy: personalized cancer vaccines remain investigational, and platform, eligibility, and treatment details depend on the clinical study.
