NIH research networks are built primarily to conduct or enable studies and generate evidence; international disease-surveillance collaborations are built primarily to detect and characterize health threats across borders, share information, and support public-health action. They can overlap, but their main questions, operating rhythms, members, and outputs are different. NIH is a research agency and funder, not one network, while international surveillance includes multiple systems with distinct mandates and governance.

What “network” means in each context

A network is a way for people and institutions to coordinate work; the word alone does not tell you what that work is. The most reliable way to distinguish these collaborations is to ask what they are organized to accomplish and what they produce.

The National Institutes of Health (NIH) has a broad biomedical research mission. It states that mission as: “To seek fundamental knowledge about the nature and behavior of living systems and to apply that knowledge to enhance health, lengthen life, and reduce illness and disability.” NIH supports many programs, each with its own scientific focus and structure. There is no single membership model or operating procedure that describes every NIH research network.

International disease-surveillance collaborations, by contrast, coordinate information about disease activity and health threats across countries. Some maintain routine monitoring; others assemble technical expertise and resources when an outbreak or public-health emergency requires a response. Not every international collaboration is a WHO system, and surveillance work is not necessarily separate from research.

The core differences at a glance

Question NIH-supported research networks International disease-surveillance collaborations
Primary purpose Conduct or enable biomedical and clinical research, including studies carried out at multiple sites. Detect and characterize disease activity or health threats across borders, inform preparedness, and support alerts or response.
Typical trigger A research question, protocol, funding program, or need to coordinate investigators and participating sites. Ongoing surveillance cycles, a signal requiring assessment, or an outbreak or emergency that calls for coordination.
Common activities Recruitment and enrollment; study protocols and trials; shared data, harmonization, analysis, and research capacity building. Reporting and information exchange; laboratory testing and characterization; epidemiologic monitoring; risk assessment; alerts and response coordination.
Participants and geography May be U.S.-focused or international. Depending on the program, participants can include investigators, clinical sites, patients, and advocacy groups. Typically cross-country, with national public-health bodies, laboratories, WHO structures, and technical partners among the participants.
Typical outputs Research findings, trial evidence, natural-history data, shared research infrastructure, or greater research capacity. Surveillance information, monitoring and risk assessments, alerts, and coordinated response; some outputs can also inform policy or research.
Coordination and governance NIH institute, center, or program funding and scientific oversight, alongside network-specific coordinating structures. International or WHO coordination and partner governance, with rules that depend on the system and its reporting, data-sharing, specimen-sharing, or response arrangements.

These are tendencies, not hard boundaries. An NIH-supported study can involve sites in more than one country, and surveillance data can inform research. The useful distinction is the primary mandate and the work a collaboration is set up to deliver.

How a research network turns a question into evidence

It starts with a planned study or research need

A research network helps investigators answer a question that one team or site may not be able to address alone. A shared protocol can align work across sites; participating sites can recruit or enroll participants, collect information, and contribute data for analysis. Coordinating those activities can make a multisite study possible and help reduce duplicated effort.

NIH’s Common Fund described these benefits in its archived Networks and Collaborations (NECTAR) initiative: networks and shared information tools can support large studies, collaboration, and less duplicated work. That page is explicitly archived and was last reviewed on June 4, 2024, so it is useful for explaining the model, not for identifying a current, complete inventory of NIH networks.

Programs define their own scope and participants

The Rare Diseases Clinical Research Network (RDCRN), supported through NIH’s National Center for Advancing Translational Sciences (NCATS), is a concrete current example. NCATS describes RDCRN as supporting clinical studies, collaboration, participant enrollment, and data management. Its program page reports scientists working across hundreds of clinical sites with patients and advocacy groups to study more than 280 rare diseases. That figure describes RDCRN’s stated scope, not the number of diseases covered by NIH overall or the number of NIH networks.

NCATS also describes work that includes natural-history studies, trial readiness, biomarkers, outcome measures, and early-phase trials. These activities illustrate how a research network can build knowledge and practical readiness for future studies, but they should not be assumed to be standard features of every NIH-supported network.

Shared data can be a research infrastructure

One model discussed in a 2019 peer-reviewed paper is the distributed research network. The paper describes networks, including the NIH Health Care Systems Research Collaboratory and PCORnet, that reuse electronic health record and claims data for research. In a distributed model, participating organizations can contribute to research using data held across systems rather than relying on one common, centralized dataset. The paper also identifies challenges: data may be collected inconsistently, care can be fragmented across providers, and research participants’ protections must be maintained.

This is one way to coordinate research, not a definition of all NIH networks. A clinical trial network, a rare-disease program, and a distributed data network can all be collaborative research structures while differing in participants, data, procedures, and scientific aims.

How surveillance collaborations detect and respond to threats

Routine monitoring creates a continuing picture

Surveillance collaborations are organized around information about disease activity, not only around a particular planned study. National institutions and laboratories can collect, test, characterize, or report information that helps partners understand what is circulating and where. Shared information can support monitoring and assessment across borders.

The World Health Organization’s Global Influenza Surveillance and Response System (GISRS) has conducted global influenza surveillance since 1952. WHO describes its work as covering surveillance, preparedness, and response for seasonal, pandemic, and zoonotic influenza. The current WHO GISRS page reports 166 institutions in 136 Member States; these are system-specific counts reported on the page accessed October 9, 2026, and may change.

GISRS has more than one information stream: FluNet carries virologic surveillance data, while FluID carries epidemiologic surveillance data. The distinction matters because a surveillance system can combine different kinds of information rather than relying on a single measure. The figures for GISRS should not be treated as counts of all international surveillance organizations or all participating countries in every system.

Systems can expand as threats and needs change

WHO guidance dated December 4, 2024, describes expanding GISRS functions to include integrated surveillance for SARS-CoV-2 and respiratory syncytial virus (RSV), alongside influenza. The described activities include specimen collection, testing, sequencing, sharing, and analysis. This is an example of a surveillance system broadening its operational scope; it does not mean every influenza surveillance collaboration has identical responsibilities or that all sites perform every activity.

WHO also says its health-emergency surveillance systems detect about 7,000 public-health threat signals each month. A signal is an item that may warrant assessment, not a confirmed outbreak. The distinction between signals and verified events is essential: monitoring is designed to surface information for evaluation, not to declare every signal a real emergency.

Response networks can mobilize expertise when requested

The Global Outbreak Alert and Response Network (GOARN) has a different emphasis from a routine surveillance system. WHO describes it as a network that coordinates technical institutions and networks to deploy personnel and resources to countries affected by outbreaks and public-health emergencies when requested. WHO’s current page reports over 360 technical institutions and networks, with a 21-partner steering committee and an operational support team. These are time-sensitive, GOARN-specific figures reported on the page accessed October 9, 2026.

In short, a surveillance collaboration may keep gathering and exchanging information during ordinary operations, while an outbreak response network can help organize specialized support when circumstances call for it. These functions can complement each other, but they are not interchangeable: information about a possible threat and deployment of response capacity are different outputs.

Where research and surveillance overlap

The boundary is practical rather than absolute. Surveillance can identify patterns that lead to research questions, and research can improve the tools or evidence used in public-health decisions. A collaboration may therefore contribute to both preparedness and evidence generation while still having a primary mandate that helps explain its structure.

WHO’s 2024 integrated respiratory-virus surveillance guidance is an example of surveillance operations that include activities—such as testing, sequencing, and analysis—that can generate information useful to research as well as public-health monitoring. The guidance describes a surveillance framework; it does not turn all participating work into a clinical research study.

The Mexican Emerging Infectious Diseases Clinical Research Network (LaRed) illustrates the other direction. A 2023 peer-reviewed article describes the network as an international clinical research collaboration formed after the 2009 H1N1 pandemic, showing how clinical research partnerships can intersect with surveillance and preparedness. That article is a historical description, not a live status update about the network’s present activities.

When a project appears to fit both categories, ask what it is accountable for producing. A protocol, enrolled cohort, trial result, or natural-history dataset points toward a research mandate. Ongoing reporting, surveillance summaries, risk assessments, alerts, or requested deployment point toward surveillance or response. Some collaborations will produce outputs from both lists.

How to classify a collaboration without overgeneralizing

  1. Identify the main question. Is the collaboration designed to answer a planned biomedical or clinical research question, or to keep watch for disease activity and health threats?
  2. Look at what starts the work. A protocol, funding program, and enrollment plan commonly organize research. Routine reporting, a signal requiring verification, or an emergency requiring coordination commonly organizes surveillance and response.
  3. Check the work participants actually do. Study visits, clinical protocols, enrollment, and research data management indicate research operations. Specimen testing, disease reporting, cross-border information exchange, and risk assessment indicate surveillance operations. A single project may do both.
  4. Read the governance in the specific program’s terms. NIH institutes, centers, and programs provide research funding or oversight in arrangements that vary by network. International collaborations also differ: WHO systems and partner networks have their own coordinating structures and rules. Do not infer a universal membership or data-sharing policy from one example.
  5. Interpret outputs and counts narrowly. A trial finding is not the same product as a surveillance alert. Likewise, RDCRN’s more-than-280-disease scope, GISRS’s institution and Member State counts, and GOARN’s partner count each describe a named program, not an entire sector.

This method is more reliable than classifying a collaboration solely by its name, whether it is international, or whether it uses data. Research networks can be international, and surveillance systems can produce information that researchers use; the deciding factor is the purpose that primarily organizes the work.
