How Nerve-Stimulation Implants Are Used to Treat Chronic Pain

Spinal cord stimulation and peripheral nerve stimulation target different nerves and are reserved for selected chronic pain cases. Here is how specialists assess candidates, run a trial, implant and program devices, and weigh benefits, limitations, MRI conditions, and risks.

Nerve-stimulation implants can reduce pain for some people whose chronic pain remains disabling despite other treatment. They are not one device or a universal cure. The two main approaches are spinal cord stimulation (SCS), which places leads near the spinal cord, and peripheral nerve stimulation (PNS), which places a lead near a selected nerve outside the brain and spinal cord. A pain specialist matches the target, stimulation system, and treatment plan to a specific diagnosis, pain distribution, previous treatment response, health factors, and personal goals.

Most SCS pathways include a temporary trial with an external pulse generator before any permanent implant. A successful trial can support proceeding to implantation, but it cannot guarantee lasting relief. Device labeling, MRI conditions, approved indications, and availability differ by system and country, so the clinical team must confirm what applies to you.

What “nerve-stimulation implant” means

The phrase covers several clinician-managed systems rather than a single standard treatment. Each system has electrodes or leads, a pulse generator, programming controls, and specific safety instructions. The target is the key distinction.

Approach Where stimulation is delivered How it is used
Spinal cord stimulation (SCS) Leads lie in the epidural space near the spinal cord; a pulse generator is usually implanted near the hip or another site selected by the team. Electrical pulses are programmed to modulate pain signaling for selected chronic pain conditions.
Peripheral nerve stimulation (PNS) A lead is positioned near a named peripheral nerve outside the brain and spinal cord. Stimulation is directed at a defined nerve target when the pain pattern and diagnosis support that approach.

The U.S. Food and Drug Administration (FDA) describes an Abbott SCS system as delivering mild electrical stimulation to the spinal cord through implanted leads and a pulse generator, with external controls for the patient and clinician. The exact biology is not identical across systems: clinicians and manufacturers describe effects such as modulation of pain signaling, but responses vary and no single mechanism explains every person’s result.

PNS is more anatomically selective. The 2024 American Society of Interventional Pain Physicians (ASIPP) guideline describes electrodes placed near targeted peripheral nerves, while 2022 guidance from the American Society of Pain and Neuroscience (ASPN) emphasizes that a well-defined nerve target can make PNS appropriate in selected cases. Evidence differs by nerve, pain condition, lead design, and stimulation protocol.

When specialists consider an implant

These therapies are generally considered for chronic, difficult-to-manage pain after reasonable non-implant treatments have not provided enough relief, produced unacceptable side effects, or failed to restore useful function. An implant is not automatically a next step for every backache, neuropathy, or postsurgical pain syndrome, and it is rarely a first-line treatment.

FDA device summaries illustrate how narrow labeling can be. Depending on the system and stimulation mode, U.S. examples include selected cases of failed back surgery syndrome, intractable low-back or leg pain, chronic pain of the torso or limbs, and diabetic peripheral neuropathy. Those examples belong to the named device’s labeling; they are not a universal list of indications for every SCS or PNS system.

What the assessment covers

Your team will usually map the pain precisely and establish what “success” would mean before recommending a trial. Assessment commonly includes:

  • Pain pattern: location, spread, quality (burning, electric, aching, or mixed), triggers, and whether the pattern is stable enough to target.
  • Diagnosis and anatomy: imaging, neurological findings, prior operations, nerve injury, and other causes that might require a different treatment.
  • Previous care: physical rehabilitation, medicines, injections, surgery, behavioral care, and the amount of relief or functional improvement each produced.
  • Functional goals: walking farther, sleeping, returning to work, reducing rescue medicine, or tolerating a specific daily activity.
  • Medical safety: infection risk, bleeding risk, anticoagulant use, other implants, allergies, and the ability to attend programming and follow-up visits.
  • Psychosocial factors: mood, coping, expectations, substance use, social support, and barriers to postoperative care.

The European expert consensus on SCS referral and selection identifies pain type, location and spread, anatomy, response to previous therapy, and psychosocial factors as relevant to referral decisions. The 2023 American Society of Regional Anesthesia and Pain Medicine (ASRA) consensus guideline states: “All patients must be screened with an objective validated instrument for psychosocial factors, and this must include depression.” This is a clinical screening requirement, not a judgment that pain is psychological and not a checklist for self-diagnosis.

How spinal cord stimulation works

In SCS, one or more leads are advanced through a needle or placed surgically so their contacts sit near the spinal cord at a level selected for the painful body region. The leads connect to an implanted pulse generator. The generator sends programmed electrical pulses; the patient may use an external controller to select permitted programs or adjust intensity within clinician-set limits.

Older stimulation patterns may create a tingling sensation called paresthesia. Newer systems can use stimulation modes designed to reduce or avoid noticeable tingling. Programming is adjusted to coverage, comfort, movement, and functional goals. The lead position, waveform, frequency, pulse width, and available control features differ among systems, so an explanation of one model should not be treated as a description of every SCS implant.

FDA’s 2024 Abbott SCS overview says, “The Abbott SCS System may help to treat chronic, difficult to manage pain of the torso, arms, and legs.” The wording “may help” is important: stimulation is intended to reduce pain or improve function, not to repair every underlying disease or guarantee elimination of pain.

How peripheral nerve stimulation works

PNS places a lead beside a selected peripheral nerve, such as a nerve supplying the painful area. The pulse generator can be implanted or, for some systems and treatment plans, remain external for a defined period. The clinician programs pulses to influence signaling along that nerve and the related pain network.

A clearly identifiable nerve target is central to the decision. Diffuse pain without a plausible target may be less suitable for PNS, while pain linked to a particular injured or irritated nerve may offer a clearer rationale. ASIPP’s 2024 guideline discusses U.S. systems including Freedom, StimRouter, SPRINT, Nalu, and ReActiv8; their availability, indications, implant duration, and labeling are not interchangeable. Evidence for one nerve or condition cannot be assumed to apply to all PNS uses.

What happens during an SCS trial

A trial tests whether stimulation can produce meaningful, usable improvement before a permanent generator is implanted. Protocols vary by clinician, diagnosis, and system, but a traditional trial usually lasts several days.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Planning and consent: The team confirms the target, reviews medicines that affect bleeding, discusses infection prevention, defines outcome goals, and explains the system’s warnings and MRI conditions.
  2. Lead placement: After sterile preparation and local anesthetic, temporary leads are inserted percutaneously and guided to the planned spinal level. Some situations use surgically placed paddle leads instead.
  3. Connection to an external generator: The temporary leads exit through the skin and connect to an external pulse generator. A patient handbook for the WaveWriter Alpha trial describes this arrangement, with the external stimulator powering leads positioned along the spinal cord.
  4. Programming: The clinician activates and adjusts programs to cover the painful region while checking comfort and unwanted sensations. You may receive instructions for permitted controller adjustments.
  5. At-home observation: For the trial period, record pain intensity, sleep, walking or other agreed activities, medication use, and adverse symptoms. Avoid actions that could pull or contaminate the temporary leads, following the team’s dressing and bathing instructions.
  6. Review and removal: The team removes the temporary leads and compares the results with the pretrial goals. A decision to implant permanently should consider both pain change and function, not a single good hour.

The ASRA consensus guideline recommends a trial before definitive implantation for the patient groups it addresses, while noting an exception for some chronic anginal pain cases. A trial response informs the decision; it is not a promise of durable benefit. Temporary lead movement, a short observation window, changing activity, and placebo or expectation effects can all make trial results imperfect.

Permanent implantation and follow-up

If the trial meets the agreed threshold and there is no safer or more suitable alternative, the team may implant a permanent pulse generator and secure the leads. The generator is commonly placed under the skin near the hip, abdomen, or another location that accommodates the device and your body habitus. The operation may be performed under local anesthesia with sedation or general anesthesia, depending on the lead type, anatomy, and clinical plan.

After healing, programming is iterative. Settings may be changed as pain shifts, activity increases, or stimulation becomes uncomfortable. Follow-up also checks the incision, battery or recharge routine, lead integrity, and whether the therapy is meeting functional goals. Keep the device identification and programming information available for emergency and imaging appointments.

Potential benefits and what the evidence does—and does not—show

Some patients obtain substantial pain reduction, improved sleep, greater activity, or less reliance on rescue medication. The relevant outcome is the change that matters to you, measured against the goals set before the trial.

Results are not assured and vary by diagnosis, target, stimulation mode, lead position, and patient. The 2023 ASRA consensus guideline reports that nearly 30%–40% of patients describe inadequate relief beyond 24–36 months. That is a guideline-reported population range, not an individual forecast and not a head-to-head comparison of all current devices.

A separate FDA 2024 summary of Boston Scientific’s WaveWriter system describes the SOLIS study, in which about 56 patients receiving SCS therapy experienced a 50% or greater reduction in average overall pain intensity at one year. The finding belongs to that device and study context; it should not be generalized to every system, diagnosis, or patient.

There is no established single “best” implant for all chronic pain. The most relevant evidence is the evidence for your diagnosis, anatomical target, stimulation mode, and health circumstances.

Risks, limitations, and practical burdens

Implantation is an invasive procedure. FDA clinician labeling lists possible risks including temporary implant-site pain, infection, cerebrospinal-fluid leakage, and rare epidural hemorrhage, seroma, hematoma, or paralysis. Hardware can move, break, or stop functioning. Stimulation may feel unwanted, fail to cover the painful area, or interfere with movement or sleep. Electromagnetic interference and MRI exposure can create hazards or require special precautions.

Risk is system-specific and depends on surgical technique and personal health factors. Ask for the current labeling for the exact generator and leads rather than relying on a generic internet list. Clarify warning signs that require urgent contact, how wound care works, when driving or lifting can resume, and what to do if stimulation suddenly changes.

MRI and other device restrictions

Some systems are approved for particular MRI conditions, while others may have restrictions based on body region, scanner strength, programming mode, lead configuration, or implant integrity. Never assume that an implant is “MRI safe” in every setting. Before an MRI, tell the imaging center about the device and bring the exact identification card. The imaging and pain teams must verify the system’s current conditions.

Battery and control requirements

Rechargeable generators require a regular charging routine; nonrechargeable generators eventually require replacement surgery when the battery is depleted. Controllers, chargers, software, and programming access are system-specific. Discuss how often you would charge, what happens if the controller is lost, and how follow-up is handled if you move.

How to compare systems with your clinical team

Rather than comparing brand names in isolation, compare the treatment plan and evidence for your particular indication. Use this checklist:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • What exact diagnosis and pain region does this system’s current labeling cover in my country?
  • Why is SCS or PNS a better fit than continued medication, rehabilitation, injection, surgery, or another non-implant option?
  • Where will the leads be placed, and what happens if my pain spreads or changes?
  • Which stimulation modes are available, and will I feel paresthesia?
  • What are the trial’s success criteria, duration, restrictions, and removal plan?
  • Is the generator rechargeable, how long might charging take, and what replacement procedure is expected?
  • What MRI scans are permitted, under exactly which conditions?
  • What complications are most relevant to my health history, and who should I call after hours?
  • How often will programming visits occur, and who pays for replacement, charging equipment, or follow-up under my insurance?
  • How much experience does the team have with this target and indication, and what alternatives remain if the implant does not help?

Preparing for a pain-specialist appointment

Bring a concise pain history, medication list, prior procedure and imaging reports, and a record of what activities the pain prevents. Write down one or two measurable goals, such as standing for 20 minutes or sleeping through the night. Ask the team to distinguish the expected effect on pain intensity from the expected effect on function. If you take anticoagulants, have diabetes, use nicotine, or have a history of infection or difficult wound healing, raise it before scheduling a trial.

Caregivers can help by tracking function and side effects without interpreting every fluctuation as success or failure. The decision should be shared with the person who will live with the implant, including the charging, controller, travel, MRI, and follow-up responsibilities.

What a realistic decision looks like

An implant is a targeted option for selected chronic pain, not a general-purpose painkiller. The strongest decision combines a plausible anatomical target, failure or intolerance of appropriate previous care, realistic functional goals, validated psychosocial screening, a carefully monitored trial when recommended, and a clear plan for complications and long-term follow-up. If those pieces are not in place, asking what should be addressed first is as important as asking which device to choose.
