A first-person case study describes three bugs that reportedly survived linting, type checking, the full test suite, and a successful build; an automated code reviewer also found no actionable defects on two pull requests. The author says reading the code against the written specification exposed them. The available account details only the first bug, so the other two cannot be explained reliably.
What the reported bug shows—and what it doesn’t
In the detailed example, a game board was supposed to display five stocks each round. A filter intended for one board mode also removed industries the player already owned in another mode. As a result, the board shrank over successive rounds.

The author reports that a test had treated the shrinking board as correct because it reflected the buggy implementation rather than the written specification. The reported fix selected from every industry with an available stock and visually disabled stocks the player already owned instead of removing them. The author also reports checking 5,000 generated game states across five rule sets with zero violations. That is the author’s account, not an independently verified result.
The post’s surfaced text does not give the mechanisms behind bugs two and three, so their causes should not be guessed. The case supports a focused lesson: passing checks can still leave a mismatch between implementation and requirement when the checks encode the same mistaken assumption.

Why automated checks can all pass
Different checks inspect different evidence. Tests execute selected cases and compare results with expected behavior; static analysis inspects source code for patterns a tool knows how to identify; fuzzing explores varied inputs; dynamic analysis observes behavior while code runs. None automatically proves that every requirement, mode, boundary, or dependency has been covered.
- Tests: Unit and integration tests can validate specified examples and user journeys. But an assertion only checks what it encodes. If expected results were derived from faulty behavior instead of the requirement, a green test can preserve the defect.
- Fuzzing: Varied or generated inputs can expose failures that hand-picked examples miss. Its reach depends on which parts of the system are exercised and whether failures are observable.
- Static analysis: Tools can flag certain source-code patterns without running the program, but capabilities differ. NIST’s 2023 SATE VI summary found performance varied with bug class, test case, and complexity; simpler bugs were easier for tools to find. NIST recommends assessing a tool on the intended codebase rather than assuming it will fit. The evaluation focused on security analysis, so its findings should not be generalized to every application defect. NIST’s SATE VI summary.
- Dynamic analysis: Running code can reveal runtime problems such as memory violations or race conditions, but only along behavior the analysis actually reaches. Google Cloud documents a workflow using several complementary methods rather than relying on one check. Google Cloud’s testing and automation guidance.
- Human review: A reviewer can compare a change with its purpose, specification, tests, and surrounding system behavior. Review can miss defects too; it is another source of evidence, not a guarantee.
How to review code against the requirement
For a change with multiple modes or states, read for the behavior the requirement calls for—not just whether the code looks familiar or the tests pass. These questions are practical prompts drawn from the reported case and documented quality workflows, not a validated checklist from a controlled study:

- What does the written requirement say should happen, including the expected number or type of results?
- Does the change implement that behavior in every relevant mode, state, and round?
- Were test expectations derived independently from the requirement, or copied from existing implementation behavior?
- What boundaries, input combinations, dependencies, or data assumptions are absent from the tests?
- Does the implementation remove an item, or should it remain visible but unavailable? Could that distinction change behavior elsewhere?
The last question echoes the case study’s reported fix: owned stocks remained on the board but were disabled. That is not a universal design rule; it illustrates why the intended behavior matters when choosing how to handle an item.

The Software Engineering Institute argues that inspection and review of engineering decisions should augment testing and code analyzers. In its discussion of Apple’s 2014 “goto fail” flaw, a duplicate line bypassed credential validation; the flaw escaped development safeguards, and the source says review should have identified it. That is not an example of review catching the problem before release. The same distinction applies to Heartbleed: an unchecked payload length enabled an over-read of server memory. SEI’s discussion is historical, not a claim that review prevented either vulnerability. SEI’s account of Heartbleed and “goto fail”.

Evidence on reviews also argues against treating them as a safety net. Mozilla’s 2018 overview of reviewed Firefox crash-prone code says defects could elude reviewers and reach users; in the examined patches, complexity and many class dependencies were associated with crash-prone code, while memory and semantic errors were major root causes. Microsoft Research’s 2015 paper record summarizes a related limitation: code reviews often miss functionality issues that should block submission, and reviewer skills and social context matter. A separate 2026 arXiv preprint by Domenico Cotroneo, Giuseppe De Rosa, Cristina Improta, and Benedetta Gaia Varriale says the authors mined more than 14,000 defects across C/C++ and Java systems. They report post-release defects concentrated in older, frequently modified, high-churn components. This preliminary finding does not establish that churn causes defects or that every older component is riskier. It suggests a reason to consider change history when deciding where scrutiny may be useful—not a substitute for examining the actual requirements and code. The authors’ 2026 preprint.

What a passing build can—and cannot—tell you
A green build establishes that the configured checks passed for the code and conditions they ran. It does not, by itself, establish that the checks represent every requirement or that the implementation behaves correctly in every relevant state. The reported game-board bug makes that gap concrete: the test suite reportedly accepted a result that contradicted the specification.
