How Fluorescence Microscopy Reveals Cellular Metabolism in Real Time

Fluorescence microscopy can track changes in cellular metabolic state through endogenous NAD(P)H and FAD signals, but those signals are indicators—not direct measures of pathway flux.

Fluorescence microscopy can follow changes in cellular metabolic state by measuring light emitted naturally by molecules such as NADH and FAD. Researchers compare signal brightness, spatial patterns, or fluorescence lifetime—and may combine NAD(P)H and FAD measurements into an optical redox ratio. These label-free readouts can show metabolic changes over time, but they are indicators of cellular state, not direct measurements of a particular pathway’s activity or rate.

What fluorescence microscopy observes

Some molecules inside cells are fluorescent: after absorbing excitation light, they emit light that can be detected through a microscope. In optical metabolic imaging, researchers can use fluorescence from endogenous cofactors—molecules already present in cells—rather than adding a fluorescent metabolic label. The principal signals discussed in this context come from reduced nicotinamide adenine dinucleotide (NADH), often measured together with NADPH as NAD(P)H, and oxidized flavin adenine dinucleotide (FAD).

That makes the approach label-free in the sense that the metabolic readout can come from the cell’s own fluorescent molecules. It also allows measurements to be spatially resolved: a researcher can examine signals across a cell or tissue and compare how they change over time. Multiphoton excitation is one method used for label-free optical metabolic imaging in cells and tissues; it is not a feature of every fluorescence microscope, nor does every fluorescence microscope measure fluorescence lifetime. The scope and capabilities depend on the instrument and experimental design. Georgakoudi and Quinn’s 2023 review describes the approach and its use for monitoring cell and tissue status and dynamic metabolic changes.

What NADH, NADPH and FAD signals mean

NADH and NAD(P)H

NADH is the reduced form of a central electron-carrying cofactor. Its fluorescence can change with its biochemical environment, including whether it is bound to proteins. A change in signal can therefore reflect a change in the cofactor’s environment or state as well as a change in how much fluorescent signal is detected.

Many measurements are described as NAD(P)H because the detected signal can include contributions from both NADH and NADPH. The two molecules have distinct biochemical roles, but a combined fluorescence measurement does not automatically separate them. Interpret the signal in light of the cell type, experimental conditions and biological question rather than treating every NAD(P)H change as a uniquely identified process. The distinction and interpretive caveats are discussed in the 2025 consensus guidelines and the 2019 review, “Evaluating Cell Metabolism Through Autofluorescence Imaging of NAD(P)H and FAD.”

FAD

Oxidized FAD is fluorescent; reduced flavin states have different fluorescence behavior. Measuring FAD alongside NAD(P)H supplies a complementary view of cellular redox balance. The paired signals can be used to calculate an optical redox ratio, but that ratio is still an indicator derived from fluorescence. It is not a universal, direct readout of glycolytic rate, oxidative phosphorylation rate, or flux through another specific pathway.

In practical terms, a fluorescence change can tell a researcher that the measured cofactor signals or their distribution have changed under the conditions being studied. The observation alone does not establish which biochemical reaction caused the change. This distinction is central to interpreting autofluorescence as a sensitive view of metabolic balance rather than a pathway-specific assay. The 2023 review emphasizes both the promise of dynamic optical metabolic imaging and the remaining work needed to interpret it quantitatively.

Intensity, redox ratios and fluorescence lifetime answer different questions

There are several ways to extract information from endogenous fluorescence. Brightness-based measurements report signal intensity; paired NAD(P)H and FAD measurements can be expressed as an optical redox ratio; and fluorescence lifetime imaging microscopy (FLIM) measures how long fluorescence persists after excitation. These approaches are related, but they are not interchangeable.

Readout What it measures What it can help reveal Important constraint
Fluorescence intensity How bright the detected signal is, including its spatial distribution. Changes in the measured endogenous fluorescence across cells, subcellular regions or time. Brightness can be influenced by signal collection and background as well as by the cofactor’s biochemical state. NAD(P)H can include contributions from both NADH and NADPH.
Optical redox ratio A ratio calculated from NAD(P)H and FAD fluorescence measurements. A comparative indicator of cellular redox balance using two complementary cofactor signals. It is not a direct, universal measurement of a pathway’s rate or flux. Interpretation depends on the signals and experimental context.
NADH fluorescence lifetime (FLIM) The time fluorescence persists after excitation, rather than only the signal’s brightness. Changes in the fluorescence environment or binding state that intensity alone may not resolve. It requires appropriate lifetime-capable instrumentation and analysis; the result remains an indicator to interpret biologically.

Intensity ratios are comparatively straightforward to obtain, but they can be affected by background, signal collection and the mixture of cofactor contributions. FLIM adds decay-time information that can help distinguish environmental or binding-state changes, while also requiring suitable equipment and analysis methods. Neither readout is established as best for every experiment. The useful choice depends on what the researcher needs to resolve, the spatial and tissue context, the available signal quality and acquisition time, and how results can be calibrated and interpreted. These distinctions are covered in the 2023 review and the 2019 review.

How to plan an optical metabolic imaging experiment

The method is most informative when the readout is chosen to answer a defined biological question. The following is a planning sequence, not a universal instrument protocol: acquisition settings, calibration and analysis must be matched to the specific microscope, specimen and measurement.

  1. Define the question and comparison. Specify what cellular or tissue change you want to observe and which conditions or time points will be compared. The fluorescence readout can track a change in metabolic state; it does not independently identify the pathway responsible.
  2. Choose the measurement axis. Decide whether signal intensity, a paired NAD(P)H/FAD redox ratio, fluorescence lifetime, or a combination best addresses the question. If NAD(P)H is involved, account for possible NADH and NADPH contributions rather than assuming the signal isolates one molecule.
  3. Match the readout to the spatial and temporal need. Consider whether the question concerns cells, subcellular distribution or tissue, and how much acquisition time is available. Field of view, signal strength, instrumentation and analysis choices affect how quickly useful data can be collected.
  4. Plan controls, calibration and analysis before acquisition. Decide how background and signal quality will be handled and how the chosen measurement will be quantified. The 2025 consensus guidelines address acquisition, calibration, analysis and reporting for intensity-based redox ratios and fluorescence lifetime measurements.
  5. Interpret the signal with independent context. Evaluate fluorescence as a complementary optical readout. If a conclusion depends on the rate or activity of a particular pathway, pair the imaging result with appropriate controls or an independent metabolic assay capable of supporting that pathway-level claim.

The 2025 consensus guidelines identify reproducible acquisition, calibration, analysis and reporting as important for cellular label-free optical metabolic imaging. They address approaches including multiexponential fitting and phasor analysis for lifetime-related data. Because methods and instruments differ, those analysis labels should not be treated as a substitute for specifying how an individual dataset was acquired and processed.

What “in real time” can—and cannot—mean

Here, “real time” means that endogenous fluorescence can be used to monitor dynamic changes during an experiment, rather than relying only on a single endpoint. It does not imply one standard frame rate, a particular number of measurements per second, or a universal delay between a biological change and a reported result. The reviewed evidence does not establish a single temporal resolution or a universal definition of real time for these techniques.

Acquisition speed depends on the instrumentation, signal strength, field of view and analysis choices. A measurement intended to follow rapid changes may face different trade-offs from one designed to map a larger region or obtain a more detailed signal. State the relevant timing for the particular experiment if it is known; avoid assigning a speed to fluorescence microscopy as a whole. The 2023 review describes dynamic monitoring while also noting that quantitative interpretation of dynamic metabolism remains an area needing further development.

Limitations that affect interpretation

  • The readout is not pathway flux. NAD(P)H and FAD fluorescence can be sensitive to metabolic balance, but fluorescence alone does not directly quantify the rate of a particular pathway. A pathway-level conclusion needs additional evidence appropriate to that claim.
  • NAD(P)H combines distinct biochemical contributors. NADH and NADPH have different roles, but the measured signal may include both. Do not attribute a change to one of them unless the experiment distinguishes their contributions.
  • Brightness is not determined only by metabolism. Intensity-based results can be affected by signal collection and background. Changes in the signal should therefore be assessed against the acquisition and analysis context, not read as an unambiguous biochemical change.
  • Tissue autofluorescence can complicate signals. Sources such as collagen or lipofuscin can overlap with or complicate the fluorescence under study. Background and specimen context matter when evaluating tissue measurements.
  • Lifetime adds information but also analytical demands. FLIM can reveal environmental or binding-state differences not resolved by brightness alone, but extracting and comparing lifetime measures requires appropriate instrumentation and methods.
  • Results are not automatically comparable across experiments. Acquisition, calibration, analysis and reporting choices affect interpretation. Clear methods and suitable controls are important when comparing conditions or studies.

These limitations do not make autofluorescence imaging uninformative. They define the strength of conclusion it supports: an optical view of metabolic state and change, interpreted with biochemical context and, when needed, independent measurements. The 2019 review and the 2023 review describe the sensitivity and interpretive limits of NAD(P)H/FAD imaging; the 2025 consensus guidelines address the practices needed to improve accuracy and reproducibility.

What a fluorescence result supports

A well-designed experiment can use endogenous NAD(P)H and FAD fluorescence to observe where signals occur and how they change under defined conditions. Intensity and redox-ratio approaches provide brightness-based indicators; FLIM adds information about fluorescence decay and molecular environment. The strongest interpretation stays within those measurement boundaries, reports the acquisition and analysis context, and uses complementary evidence when the question is about a specific pathway’s rate.
