A rapid test may identify a particular bacterium or resistance marker, while conventional susceptibility testing measures how a recovered isolate responds to specific antibiotics. Neither a positive marker nor a lab report, by itself, determines whether someone has an active infection or which treatment is right.
How the testing and treatment process works
The exact tests depend on the suspected infection, the specimen, and what the laboratory can perform. For bloodstream infections, for example, blood cultures are used to detect and recover bacteria so they can be identified and tested. A swab, blood draw, or other specimen is not a generic resistance test: the result depends on what is tested and how.

- Assess the suspected infection. A clinician evaluates symptoms and the likely infection site, then orders a specimen appropriate to that site.
- Detect and identify bacteria. The clinical microbiology laboratory checks for growth and identifies the organism. Some rapid methods can identify organisms or detect selected resistance markers, including from a positive culture.
- Measure susceptibility. When bacteria have been recovered, the laboratory can expose them to antibiotics and measure whether they grow.
- Interpret the result. The laboratory compares measurements with organism- and drug-specific clinical breakpoints, which help assign categories such as susceptible or resistant.
- Choose or revise treatment. The care team combines the laboratory findings with the infection site and severity, the patient’s allergies and risk of drug toxicity, and other clinical considerations.
Testing and treatment can proceed in stages: clinicians may start with an initial plan based on the clinical situation, then use results as they arrive to change the drug or narrow coverage. The laboratory result informs that decision; it is not itself a prescription.

Culture and susceptibility testing versus rapid marker tests
These approaches answer different questions. Conventional culture and antimicrobial susceptibility testing (AST) provide measured responses from recovered bacteria; a rapid molecular test looks for the organisms or resistance markers included in that assay.

- THE ORGINAL DIY BACTERIA TEST KIT that is certified accurate by Good Housekeeping Labs, American Red Cross, Underwriter Labs, and Merck Labs. Designed and developed by our EPA certified scientists. Made in the USA.
- PATENTED TECHNOLOGY: Detects 50 different species of coliform bacteria (including E. coli), as low as 1-organism. Test any freshwater supply.
- 3RD PARTY STERILIZATION: Every Safe Home kit is sterilized by a certified facility (see attached certificate) and guaranteed to be “free from bacteria” before you test. This process assures zero “false positive results” from product contamination, as may be experienced with other kits who skip this sterilization process.
- FAST RESULTS- Get results as quickly as 6 hours. May take up to 72 hours based on incubation conditions, and concentrations of bacteria in the water being tested.
- YOUR PURCHASE MAKES A DIFFERENCE in people’s lives. Safe Home donates a portion of every test kit sale to clean water initiatives around the globe. See attached images with this listing, to learn more.
| What to compare | Culture plus phenotypic AST | Rapid molecular or marker testing |
|---|---|---|
| What it can show | Recovered and identified bacteria, with measured susceptibility to the antibiotics tested. | Whether selected organisms or resistance genes or markers targeted by the assay are detected. |
| Breadth | Can provide a drug-specific susceptibility profile, subject to the laboratory’s panel and method. | Limited to the assay’s targets; not detecting a marker does not establish susceptibility to every antibiotic. |
| Timing | Requires bacterial growth and additional testing. Reference broth microdilution takes multiple days. | Can provide targeted information sooner. A cited ASM review reports that some assays performed on positive blood cultures return results in under two hours; this does not describe every test or the time to a final susceptibility profile. |
| Main role | A foundation for identifying pathogens and measuring susceptibility. | Complementary information that may support an earlier targeted treatment change. |
| Important limit | Laboratory conditions do not perfectly reproduce the conditions inside a patient’s body. | A detected marker is not automatically a complete susceptibility profile or proof that the organism is causing active infection. |
CDC describes selected public-health laboratory testing that can include organism confirmation by MALDI-TOF, carbapenemase tests, and PCR for particular carbapenemase genes. Those examples illustrate targeted testing; they are not a universal set of tests used for every infection.
How laboratories determine whether an antibiotic is likely to work
Phenotypic AST tests the bacteria themselves. The laboratory exposes an isolate to antimicrobial agents and measures its growth response. Methods described by the CDC include broth microdilution, Kirby-Bauer disk testing, and gradient diffusion strips. Broth microdilution is a reference method, but conventional testing can take multiple days.

The measured result is interpreted using a clinical breakpoint: a threshold for a particular organism and antibiotic that helps connect laboratory measurements to clinical categories. Depending on the organism, drug, and applicable standard, a report may use categories such as susceptible, intermediate, susceptible-dose dependent, or resistant. These categories are not a universal ranking of all antibiotics. Breakpoints can differ by organism and medicine and may be revised as evidence and standards change, so the report needs to be interpreted in context.

How doctors use the result to choose treatment
Clinicians consider whether the detected organism is causing the patient’s illness and whether a reported antibiotic is appropriate for the infection site and severity. They also weigh allergies, toxicity risks, and other clinical factors. A laboratory category is important evidence, but it does not account on its own for every factor that affects a treatment decision.

As results become available, the team may narrow broad initial coverage, switch from a drug that is not expected to work, or ask a specialist for advice. The appropriate choice is specific to the infection and the patient; there is no single antibiotic recommendation for “resistant bacteria.”
When a difficult-to-treat infection needs expanded testing
For selected resistant organisms or hard-to-treat infections, public-health laboratory networks may offer confirmatory or expanded testing. CDC’s Antibiotic Resistance Laboratory Network describes testing for selected carbapenem-resistant isolates that can include organism confirmation, carbapenemase testing, targeted PCR, and broader AST. CDC also describes expanded susceptibility testing for selected hard-to-treat infections; its 2021 information reports some results within three business days. That timing is specific to the service described in that publication, not a general promise for routine testing. Eligibility and current service details vary, so clinicians determine whether a case qualifies.

Some specialized services answer a different question from routine AST. CDC’s AR Lab Network page, accessed in 2026, reports results within two business days for its PCR colonization screening service after the specimen is received. That figure applies to the described screening service, not routine testing of an active infection or a final antibiotic susceptibility report.
What a resistance test cannot tell you on its own
- Whether bacteria are causing disease. Detecting bacteria or a resistance marker does not by itself prove active infection; the finding must be considered with the specimen and clinical picture.
- Whether every antibiotic will work. A marker test covers its defined targets, while a susceptibility profile covers the drugs and methods used by the laboratory.
- Exactly how treatment will perform in the body. AST measures bacterial response in laboratory conditions, which do not perfectly reproduce the infection environment.
- Which medicine you should take. A clinician must connect the result to the infection and the patient’s circumstances.
A 2024 ASM clinical pilot of GoPhAST-R reported 95% overall agreement with standard AST in 42 positive blood cultures. The study authors said larger and more diverse validation is essential, so this pilot result should not be treated as proof that the method can generally replace standard susceptibility testing.
