How Can Technology Make Health Management More Accessible?

Telehealth, patient portals, apps and remote monitoring can bring parts of health management closer to home, provided people can access the tools and care teams can respond.

Technology can make health management more accessible by bringing some care closer to home: telehealth connects patients with clinicians, remote monitoring shares selected health readings between appointments, and apps and patient portals can support tracking, reminders and communication. These tools work best when they are easy to use and connected to a care team that can review information and respond. They extend care; they do not replace every in-person visit or the health system behind it.

How does technology reduce barriers to care?

Distance, travel time and mobility can make routine care difficult. Video or phone appointments let patients speak with clinicians remotely, and may help them reach specialists who are not nearby. For some follow-ups, a patient can also share information from home rather than travel to an office. Whether a remote appointment is available and suitable depends on the patient’s needs, the clinician and the services offered. The U.S. Department of Health and Human Services (HHS) describes these uses in its telehealth guidance.

Digital communication can also keep care moving between appointments. Patient portals and secure messages may give patients a way to send updates or ask questions, while care teams can use relevant information to coordinate follow-up. HHS describes hybrid care as a combination of telehealth and in-person services that can support ongoing monitoring and reduce travel burden.

What can apps, portals and connected devices help manage?

Depending on the tool and care plan, patients may use apps or connected devices to record activity, nutrition, symptoms, weight, blood pressure, blood glucose, medication routines or factors related to mental health. A portal can help share relevant information with a care team, and digital reminders can prompt actions such as appointments, vaccinations or medication routines. WHO has reported positive effects in some areas from reminders for antenatal visits and childhood vaccinations; that does not mean every reminder tool works equally well in every setting.

Tracking can make patterns easier to discuss with a clinician, but a device reading is information to interpret—not a diagnosis by itself. The usefulness of an app depends on what it measures, how the information is used and whether it fits the person’s clinical needs. The U.S. Food and Drug Administration (FDA) uses a risk-based approach to software functions that meet the definition of a medical device. A consumer wellness app should not automatically be assumed to have been reviewed as a diagnostic or treatment tool.

What is remote patient monitoring, and how does it work?

Remote patient monitoring (RPM) uses digital devices to collect and transmit patient information to a health care provider. Examples in HHS materials include blood pressure monitors for hypertension, blood glucose measurement for diabetes, digital scales, wearables and apps for tracking stress, symptoms or health habits. RPM is used in some chronic disease care and after hospital discharge.

The device is only one part of the service. HHS describes a clinical workflow in which staff review incoming data, contact a patient when a reading falls outside an expected range, verify readings when needed, explain the information and decide whether to adjust medication, arrange an appointment or refer the patient for more urgent evaluation. Without review and a response process, sending readings alone does not provide the full care-management benefit.

HHS reported in 2024 that more than 10 times as many U.S. Medicare enrollees used RPM in 2022 as in 2019. This is a comparison of Medicare use in those two years, not a current adoption rate and not evidence by itself that outcomes improved or access became equal.

Which type of digital care might fit?

Option Can help with What it depends on
Video or phone visit Remote conversations with clinicians, follow-up and access to some specialists Whether the concern can safely be handled remotely, service availability, connectivity and a suitable device
Remote patient monitoring Sharing selected readings from home for ongoing management or follow-up An appropriate device, a care team that reviews readings and a defined response workflow
Apps, portals and reminders Tracking, communication, education and prompts between appointments Usability, accessible design, privacy, reliable information and a clear role in the care plan
Hybrid care Combining remote contact or monitoring with in-person assessment and treatment Coordination between remote and face-to-face care, guided by clinical need and patient preference

WHO recommends assessing digital health interventions for benefits and harms, acceptability, feasibility, resource use and equity rather than assuming a tool will work in every context. In practice, consider whether remote care is clinically appropriate; who will review information and respond; what travel or specialist-access problem it solves; what devices, broadband and skills it requires; how privacy is handled; and whether the approach accommodates the patient’s language, disability access needs and preferences.

Who may be left out, and what can help?

Digital care can create or widen barriers when a person lacks a suitable device, reliable internet, a private place to talk, confidence using technology, accessible interfaces or support in their language. Health literacy, motivation and familiarity with the tools also affect whether they are useful. HHS advises care teams to consider technology access and experience, health literacy and motivation, then tailor telehealth to clinical needs, resources, comfort and preferences.

WHO also notes that digital health depends on infrastructure, trained health workers, coordination, privacy protections and a health system able to respond. Protecting health information and using qualified health workers matter as much as the technology itself. WHO describes telemedicine as a valuable complement to face-to-face care, not a complete substitute for it.

How to use digital tools as part of a care plan

  1. Start with the care need. Ask whether a remote visit, tracking tool or home reading is appropriate for the issue, rather than choosing a tool first.
  2. Confirm the follow-up process. For monitoring, find out who reviews readings, how often they are checked, how the care team handles unusual results and how to get help if symptoms worsen.
  3. Check practical access. Consider the required device, internet connection, language, accessibility, privacy and any support needed to use the service.
  4. Agree how it connects to in-person care. Make sure there is a way to arrange an examination or other face-to-face care when remote contact is not enough.

A home blood pressure monitor or digital scale may be relevant when it fits the condition and clinician’s plan. The evidence here supports these as device categories used in monitoring; it does not establish preferred brands, models or performance.
