Adding an antibiotic to polymethylmethacrylate (PMMA) bone cement can change its pore structure, antibiotic release, and mechanical strength. The direction and size of the change depend on the antibiotic’s form and dose, the cement formulation, specimen geometry, and the way the components are mixed. In a 2026 in-vitro comparison using vancomycin and high-viscosity Palacos R, liquid incorporation produced more porosity, while powder-mixed beads produced greater cumulative release. Increasing vancomycin loading reduced compressive and bending strength in both preparation groups.
What antibiotic incorporation changes in PMMA cement
Porosity creates pathways, but it is not the same as antibiotic release
Porosity describes voids within the cured cement. Adding an antibiotic can introduce or enlarge pores as particles dissolve, as liquid changes the mixing system, or as the powder disrupts the polymer matrix. Greater exposed surface may provide more area for fluid contact and antibiotic elution, but porosity alone does not predict the amount or duration of release.

Release also depends on the antibiotic proportion, whether it is a powder or a solution, the cement’s chemistry, how evenly it is distributed, and the specimen’s shape and surface-area-to-volume ratio. Two samples with similar measured porosity can therefore release different amounts of drug when their formulations or geometries differ.

The 2026 vancomycin comparison found a dose-dependent reduction in compressive and bending strength as the vancomycin concentration increased. This relationship occurred in both powder-mixed and liquid-mixed formulations. The result matters because a formulation that increases accessible surface area or drug delivery may simultaneously reduce the cement’s ability to carry load or resist bending.

What the 2026 vancomycin comparison found
Loh and colleagues compared 1–4 g of vancomycin per 40 g of high-viscosity Palacos R cement. One group used powdered vancomycin; the other dissolved vancomycin in water before incorporation. The investigators evaluated different specimen forms, including beads, cylinders, and blocks, and measured release for six weeks.

These findings show why “more porous” and “releases more antibiotic” cannot be treated as interchangeable conclusions. In this defined formulation, liquid mixing generated more porosity, yet powder-mixed beads released more vancomycin cumulatively over six weeks.

Why mixing and processing change the result
Powder versus liquid incorporation
Powdered antibiotic adds solid particles to the cement dough. Dissolution of those particles can leave channels or voids, but the final distribution depends on particle size, dose, and how thoroughly the mixture is homogenized. A liquid antibiotic changes the liquid phase of the cement and may alter wetting, viscosity, polymerization, and the formation of internal voids. The 2026 comparison demonstrates that these mechanisms can produce different porosity and release patterns even when the antibiotic and nominal dose are the same.
Hand mixing and vacuum mixing
A 2020 study published by the American Chemical Society reported that PMMA processing affects porosity and downstream material properties. In that study, vacuum mixing improved mechanical strength but produced less homogeneity than hand mixing. Vacuum mixing therefore cannot be treated as an automatically superior preparation method for every endpoint.

The clinical-use review also describes conflicting effects of vacuum mixing on antibiotic elution across commercial cements. The effect depends on the cement and the rest of the formulation, so a result from one product should not be transferred to another without qualification.
How to compare porosity and release studies correctly
Apparent disagreements between studies often reflect different experimental setups. Align these variables before comparing results:

- Antibiotic identity, form, and dose: record the drug, whether it was a powder or solution, and the amount per cement mass.
- Cement formulation: identify the brand or type, viscosity, powder-to-liquid system, and any formulation-specific additives.
- Mixing and homogenization: note hand mixing, vacuum mixing, mixing time, temperature, and whether the antibiotic was added before or after the cement reached a particular working phase.
- Specimen geometry: compare beads, cylinders, blocks, or other shapes separately because surface area and diffusion distance affect release.
- Endpoint and time course: distinguish measured porosity from cumulative release, release rate, compressive strength, and bending strength. A six-week cumulative value is not equivalent to an initial release-rate measurement.
What the findings mean for cement intended for clinical use
PMMA cement may serve different roles, including fixation or temporary antibiotic-loaded spacers. The acceptable balance between drug release and mechanical performance depends on that role. A temporary construct may prioritize local elution differently from a load-bearing fixation construct, but increased release must still be weighed against loss of strength and structural integrity.

Key limitations and common interpretation errors
- Generalizing across cements: Cement brands and types have different base formulations and can respond differently to the same antibiotic.
- Generalizing across antibiotics: Particle characteristics, solubility, dose, and chemical compatibility vary by drug.
- Assuming porosity guarantees better delivery: Porosity can increase exposed surface, but it does not determine release independently of geometry and formulation.
- Ignoring dose: Higher antibiotic loading may increase the potential drug reservoir while reducing compressive and bending strength.
- Equating laboratory release with clinical effectiveness: In-vitro elution over six weeks does not establish tissue concentrations or patient outcomes.
- Comparing unlike specimens: Beads, cylinders, and blocks have different diffusion paths and surface areas.
Bottom-line interpretation
Antibiotic incorporation changes PMMA bone cement as a system: its pore structure, elution behavior, and strength move together but not always in the same direction. In the 2026 Palacos R study, liquid vancomycin incorporation increased porosity, whereas powder incorporation produced greater cumulative release from beads. More vancomycin weakened both formulations in compression and bending. The practical conclusion is not that powder or liquid is universally better, but that antibiotic form, loading, cement type, geometry, and mixing process must be evaluated together for the intended application.
