How an AI Incident War Room Should Gate Findings and Sensitive Actions

An AI incident war room may gate what enters shared context without stopping sensitive tool calls. Learn the difference and what a real execution approval should enforce.

An incident war room can keep people and AI agents working from one operational record, but a gate on what enters that record is not the same as approval before an AI tool takes action. Landfall documents a gate for admitting findings into shared incident context and says people apply changes outside the product. That is a useful boundary—not proof that the system cannot be fooled or bypassed.

What an AI incident war room is meant to control

A war room is a shared operational space for coordinating responders and maintaining an incident record. Landfall describes a single event-sourced timeline where people and agents can see findings, charts, logs and decisions in a common context. The value is not simply putting a chatbot beside an alert feed: responders need to understand which information has been accepted, what remains a hypothesis, and what decisions were made.

Landfall’s documented approval gate principally controls admission to that shared context. Findings and hypotheses supplied by a responder’s harness are staged rather than automatically shown to other agents. The homepage says plain observations are rechecked against sources, while higher-impact items—such as proposed causes, correlations and directives—need a person’s approval before admission. This is a context gate: it governs what agents treat as shared incident knowledge.

According to Landfall’s product documentation, an organization administrator can directly admit a staged item with a recorded reason; the documentation search result describes gate modes called on, shadow and off, with on as the default. Because those details are available in a search result rather than a directly fetched concepts page, confirm the current product documentation before relying on mode names or defaults. These descriptions do not establish that every customer configuration uses the same settings.

Context admission is not tool-execution approval

A finding can be carefully reviewed before it becomes shared context, yet an agent could still need a separate control before it calls a sensitive tool. The distinction is about timing and consequence: context admission controls what the room accepts; execution approval must stop a tool call before the requested action runs.

Control What it gates When it acts What the cited documentation establishes

OpenAI Agents SDK human-in-the-loop example Calls to tools designated as requiring approval The run pauses before the sensitive tool call executes, pending a person’s decision The SDK documentation describes a pending approval interruption that can be approved or rejected. It is an implementation example, not evidence that Landfall uses the SDK.

OpenAI’s Agents SDK documentation describes the execution-level pattern directly: “Use the human-in-the-loop (HITL) flow to pause agent execution until a person approves or rejects sensitive tool calls.” The approval rule may apply to every call or be evaluated per call. When approval is required and no decision is stored, the agent run pauses for review. The Python guide also says callable approval rules fail closed to manual review when arguments are absent, malformed, not a JSON object, or contain nonstandard constants. This is a documented SDK behavior, not a guarantee about other agent frameworks.

A meaningful execution gate therefore needs to sit at the tool-call boundary, before the sensitive action takes place. The reviewer needs to see the specific requested action and its arguments, and the decision needs to apply to that call. Approval alone does not make a system secure: permissions, identity controls, monitoring, and a safe fallback still matter.

What Landfall says remains under human control

Landfall says agents can suggest remediations, including their blast radius and undo path, but a human applies the change using external tools. Its documentation puts the boundary plainly: “Landfall never approves or runs a change.” The product’s described role is to stage and coordinate incident information, not to execute remediation.

Landfall also says a human override wins and that context later found to be wrong is hidden from agents with the reason recorded rather than deleted. That supports a useful audit practice: preserve the history of what entered the room and why it was later corrected. It does not establish that every integration or customer configuration enforces the same protections, and the available documentation does not establish an independent security test or adversarial evaluation of the product.

What to check before trusting an approval gate

Assess the gate by the action it controls and the evidence it leaves behind, not by a broad promise that an AI cannot be faked out. During a product review or incident exercise, look for clear answers to these questions:

  • What is gated? Distinguish shared findings from tool execution, external communications and remediation. A control over one category does not automatically cover the others.
  • Does it act before the consequence? A review after an agent has already run a tool is not pre-execution approval.
  • Who can approve or override? Identify the responsible roles and authority limits. AWS Customer Incident Response Team guidance recommends defining approval authority by business impact thresholds, not just technical severity.
  • What is recorded? Preserve the incident timeline, source evidence, the relevant AI activity, the decision, and any reason for admitting or correcting context.
  • What is the containment plan? Understand the blast radius in business terms, dependencies, rollback options and the consequences of disabling a system.

Prepare the war room before an incident

OWASP’s GenAI Incident Response Guide recommends preparing secure communication channels, escalation paths, defined roles and notification procedures, as well as a plan for creating a virtual or physical war room for major incidents. It advises teams to consider system criticality, functional impact, anomaly type, and data integrity and confidentiality. This is guidance, not a binding standard or regulator requirement.

OWASP’s reporting template provides a practical record to adapt: incident ID and title; detection date and time; description; affected AI system or model; impact assessment; incident type or cause; detection method; actions taken; stakeholders notified; response team; next steps; status; and timeline. For an AI incident, keep the distinction between observed evidence and hypotheses visible so a mistaken interpretation does not silently become accepted context.

A 2026 AWS Customer Incident Response Team presentation hosted by NIST adds operational detail: map AI systems to business functions, catalog non-human identities, identify a human owner, describe blast radius in business terms, and practice response. It calls out evidence such as prompt and completion logs, model-version history, training-data access, embedding queries, agent decision traces and drift metrics. The presentation recommends prebuilt containment options that account for business cost and approval authority by impact threshold. This is AWS guidance hosted by NIST, not a NIST standard.

As Robert Saul, General Manager of AWS CIRT, puts it in the presentation: “Define approval authorities based on business impact thresholds, not just technical severity”. A high-severity alert does not by itself tell a team who can authorize a disruptive containment step; that authority should be agreed in advance.

How to judge a “can’t be faked out” claim

“Can’t be faked out” is not a security property established by the available product documentation. A more precise question is whether the control is enforced at the right boundary, what kinds of input or action it covers, who has authority to decide, and whether the decision and evidence are retained. For a context gate, ask how staged material is checked and admitted. For a tool gate, verify that execution pauses before the action, with review tied to the exact call. Then test those boundaries in a controlled exercise and document what happens when inputs are malformed, evidence conflicts, an approver is unavailable, or a proposed containment action could disrupt a business function.
