How AI Vulnerability Scanners Work—and Why Findings Need Human Review

AI can help detect, explain, or propose fixes for code vulnerabilities, but a scan is not a verdict. Learn how scanners work and how to validate alerts and patches in application context.

AI can help a vulnerability scanner find, explain, classify, or propose a fix for a security issue. It cannot, by itself, prove that an alert is exploitable—or that a suggested patch is safe. Most scanners combine code-analysis techniques with varying degrees of AI assistance, and the result is evidence to investigate, not an automatic verdict.

Understanding the difference matters: conventional static analysis examines code through rules and program-analysis methods, while an AI layer may add coverage or interpret results. Both can miss flaws or flag code that is safe in its actual context. A sound review checks the reported path, the controls around it, the application’s behavior, and the consequences of any proposed change.

What an AI vulnerability scanner does

An AI vulnerability scanner examines software for patterns or behaviors associated with security weaknesses. The phrase can describe several different designs: a conventional static-analysis engine with an AI feature added to explain alerts, a system that uses AI to analyze code, or a tool that generates a possible repair after another engine detects a problem. There is no single architecture shared by every product.

Static application security testing, or SAST, analyzes source code without needing to run the application. Depending on the tool, it can look for suspicious code patterns, follow data through a program, inspect possible execution paths, and account for language or framework conventions. An AI component may extend that work or assist with interpreting its results, but an alert still needs to be checked against the application that will actually run.

OWASP describes common SAST approaches and their trade-offs in its Static Application Security Testing guide. The details vary by implementation: supported languages, frameworks, rules, and analysis depth all affect what a tool can detect.

How the scan pipeline works

A typical static-analysis pipeline turns source code into information an engine can inspect, looks for a possible weakness, and reports an alert. An AI feature may enter at one or more stages, but it does not follow that every AI-enabled scanner uses the same sequence or model.

1. Read and represent the code

The scanner receives source code, or in some workflows a pull request’s changed code. It parses that input into a representation suitable for analysis. This may include an abstract syntax tree (AST), a structured view of the code’s elements and relationships. The representation helps the engine reason about what the code does rather than merely search for a word or symbol.

For example, a rule might flag code that builds a database query by concatenating strings. That pattern can indicate a risk, but the pattern alone does not establish that an attacker can control the concatenated value or that the query is reached in a vulnerable context.

2. Apply rules and program analysis

Analysis engines use different combinations of techniques. Simple pattern checks can be fast and useful for recognizable constructs. Deeper analysis can follow how information moves through code and examine which branches or paths are possible.

  • Pattern and AST checks identify code shapes associated with a risk, such as a potentially unsafe operation or a query assembled from strings.
  • Dataflow or taint analysis tracks potentially untrusted input from a source—such as a request parameter—toward a dangerous sink, such as a sensitive operation. It can check whether a sanitizer or other control intervenes along the path.
  • Control-flow analysis examines the routes execution may take through the code, helping determine whether a suspicious operation can be reached under a given path.
  • Semantic analysis can account for language and framework conventions that affect how code behaves.

These techniques have different costs and coverage. A quick pattern check may miss a weakness that depends on a longer path through the program; a deeper analysis may take more work and still lack relevant runtime or framework context. OWASP discusses this distinction between faster pattern analysis and deeper dataflow analysis, while noting that implementations differ in their methods and coverage.

3. Create an alert

When the engine identifies a possible issue, it generates an alert with a code location and an explanation. Some alerts include a path through the program, such as the route from an input source to a potentially dangerous operation. That evidence gives a reviewer a starting point: the alert is a claim about code that should be tested against the path and conditions in the application.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

4. Add AI assistance, where the product provides it

An AI feature may receive structured alert data, relevant code, repository context, or guidance about the reported issue. It might then classify the finding, explain why it matters, or suggest a remediation. Those are different jobs: explaining an alert does not independently prove it, and generating a patch does not establish that the patch preserves the application’s intended behavior.

GitHub’s reported design for Copilot Autofix illustrates one such arrangement. In its engineering account, CodeQL first identifies an alert. A fix generator augments the alert’s SARIF data with source-code context, prompts a large language model, filters and post-processes the response, then presents a suggested patch. GitHub’s account was published on February 14, 2024, and updated April 7, 2025; it describes GitHub’s design at that time, not a universal blueprint for AI scanners. See GitHub’s engineering explanation of AI-assisted vulnerability fixes and its documentation on the responsible use of security and quality AI features.

What AI changes—and what it does not

AI can make a scanner more useful without turning its output into ground truth. Its role depends on the product. One feature might assist with analysis or coverage; another might interpret a conventional scanner’s alert or draft a fix. A reader should check what a particular feature actually does rather than infer its capabilities from the label “AI-powered.”

Stage or capability What it can contribute What it does not establish by itself
Static-analysis engine Finds suspicious patterns, traces data, or evaluates program paths according to its rules and analysis methods. That a path is reachable in deployment, that an attacker controls the relevant input, or that every flaw has been found.
AI-assisted explanation or classification Can provide an interpretation of an alert using code or repository context available to the feature. That the interpretation is correct or that the underlying finding has been independently validated.
AI-generated remediation Can propose a code change in response to a detected issue. That the patch closes the weakness without breaking intended behavior or introducing another defect.

GitHub’s AI Scan for pull requests is a concrete, separate example. As described in GitHub documentation accessed October 10, 2026, AI Scan is in public preview, complements CodeQL on eligible pull requests, analyzes pull-request code directly, and uses code search to gather repository context. It is advisory, runs on pull requests rather than as a full-repository scan, and GitHub says its findings may include false positives. Its supported categories and languages may change. These are product-specific details, not properties to assume of other scanners. The current description is in GitHub’s AI Scan documentation.

Why a finding can be wrong—or incomplete

Static analysis sees code, but deployed behavior also depends on context. A scanner may not know whether a reported path is reachable, whether another control protects it, or how a framework handles a particular operation. It may lack runtime state, deployment configuration, authorization policy, or business rules needed to decide whether the situation is exploitable.

Why false positives happen

A false positive is an alert that does not represent a vulnerability in the relevant application context. A suspicious code pattern may be unreachable, or an input may be constrained or sanitized elsewhere. The engine may not model a framework convention or protective control that changes the result. An AI explanation can make an alert easier to understand, but a confident-sounding explanation is not a substitute for checking the actual code path and controls.

Why scanners miss issues

A false negative occurs when a real weakness is not reported. A flaw may depend on runtime state, cross-service configuration, an authorization decision, or business logic that is not visible in the analyzed code. A weakness may also fall outside a tool’s supported language, rules, or analysis coverage. No scan should be read as proof that the code is free of vulnerabilities.

Evaluation results reinforce the need for caution. NIST’s 2023 SATE VI Report: Bug Injection and Collection says effectiveness varied by test case, bug class, and complexity; tools found lower-complexity bugs more readily than higher-complexity ones. It does not provide one accuracy percentage that can be generalized to every scanner or codebase. NIST describes static analysis as useful and advises prospective users to test tools on their own codebase before production.

How to validate an alert in your application

Review the finding as a specific claim: this input or condition can reach this operation, and the surrounding controls do not adequately prevent the stated impact. Work through the evidence rather than accepting either the alert or a dismissal based on its severity label alone.

  1. Locate the reported code and understand the claim. Read the alert explanation, affected lines, and any supplied path. Identify the alleged weakness and the condition the scanner says makes it possible.
  2. Trace the path. Follow the relevant data from its source through transformations to the operation identified as the sink. Confirm whether the path is possible in the application, rather than assuming that code appearing in the alert is necessarily executed.
  3. Inspect guards and transformations. Look for validation, sanitization, authorization checks, or other controls between the source and sink. Determine whether they apply to this specific path and whether they are appropriate for the operation.
  4. Check framework and configuration behavior. Confirm how the relevant framework, libraries, and deployed configuration handle the operation. Do not assume a control exists just because it is common, or assume it is absent because the scanner did not describe it.
  5. Assess realistic impact. Consider who can reach the path, what data or capability is exposed, and whether the scenario violates the application’s security requirements. Runtime and business context can change the significance of an otherwise plausible code pattern.
  6. Decide and record the result. If the alert is valid, fix the underlying cause and verify the change. If it is not applicable, document the concrete reason—such as an unreachable path or an effective control—so a future reviewer can evaluate that decision.

This process applies whether the finding comes from a conventional rule or an AI-assisted feature. The reviewer is validating the claim in the target application, not judging whether the tool’s prose sounds persuasive.

How to review an AI-generated patch

A suggested patch is a proposal, not an approved change. It may address the alert but alter behavior elsewhere, apply a control in the wrong place, or fail to handle another path to the same operation. GitHub says developers must evaluate suggestions and verify that intended behavior is maintained.

  • Read the complete diff, including surrounding code and any changes beyond the reported lines.
  • Check that the change addresses the cause of the vulnerability rather than only suppressing the alert.
  • Confirm that existing callers, error handling, and expected application behavior remain valid.
  • Run relevant automated tests and add or update tests for the security condition where appropriate.
  • Re-run the relevant analysis and review any remaining or newly reported findings.

Passing tests are useful evidence, but they do not prove that a patch is secure in every runtime or deployment context. Treat the patch review as part of the same verification work as the original alert.

Use scanning as one layer of verification

A static scan addresses only part of software verification. NIST’s Guidelines on Minimum Standards for Developer Verification of Software recommends combining techniques, including threat modeling, automated tests, static scanning, heuristic secret detection, built-in protections, black-box and structural testing, historical test cases, fuzzing, web-application scanning where applicable, and review of included libraries, packages, and services.

These checks answer different questions. Static analysis examines code for possible weaknesses; tests exercise selected behavior; fuzzing explores inputs; dependency review looks at included components; and threat modeling helps identify security concerns in the system’s design. Choose the mix according to the system and its risk. A clean scan does not replace the other checks, and no single listed technique covers every source of risk.

How to evaluate a scanner before relying on it

Do not select a scanner solely by the number of alerts it produces or by an advertised AI capability. A useful evaluation asks whether its evidence and workflow help the team find and resolve relevant issues in its own software. NIST recommends testing tools on the intended codebase before production use.

  • Language and framework coverage: Check whether the tool supports the languages and frameworks the project actually uses, including the versions and conventions relevant to it.
  • Vulnerability and query coverage: Determine which classes of issues and rules are addressed, and identify important gaps for the application’s risk profile.
  • Analysis depth: Understand whether findings rely on patterns, dataflow, control-flow, semantic information, or a combination, and what trade-offs that entails.
  • Evidence and explainability: Look for a clear location, a useful explanation, and—where available—a traceable path that a reviewer can verify.
  • False-positive burden: Pilot the tool against representative code and measure the review effort required to distinguish useful alerts from non-applicable ones. Do not assume results from another project predict yours.
  • Workflow fit: Check how scanning fits into code review and CI, and whether teams can investigate findings before they become a source of unmanageable noise.
  • Fix reviewability: If the tool proposes patches, assess whether changes are understandable, limited, and straightforward to test and review.

A pilot should include more than an easy demonstration case. Use representative parts of the codebase and have developers inspect both reported and apparently clean areas in the context of other verification work. The aim is to learn what the tool covers, where its output needs human effort, and how it fits the team’s review process—not to infer a universal accuracy rate from one run.

Interpreting a scan result responsibly

A useful mental model is that the scanner supplies a lead: a location, an explanation, and sometimes a path or suggested repair. The team supplies the missing application context, determines whether the risk is real, and verifies any fix. That division of work preserves the speed and reach of automated analysis without asking it to establish facts it cannot see.
