It interprets a goal, proposes a plan, inspects files or other inputs, edits code, runs permitted checks, evaluates the results, and either revises the work or asks a person to decide. The model is only one part of the system: the harness, tools, runtime boundaries, permissions, and human review determine what the agent can actually do and how risky its actions are.
The agent loop: from request to verified result
A conventional chatbot usually responds to a prompt with one answer. A coding agent operates a control loop. It maintains enough context to decide what to do next, calls tools that produce observable results, and uses those results to update its approach. The loop can end when the task is complete, when a tool or permission boundary blocks progress, or when the agent needs clarification or approval.

- Interpret the goal. The system turns a request such as “fix the failing login test” into an operational objective, while identifying constraints supplied by the user, project instructions, or the runtime.
- Plan. It proposes substeps, such as locating the authentication code, reading the failing test, identifying a likely cause, and deciding which checks would validate a change.
- Act through tools. It may list directories, read files, search for symbols, edit a file, run a test command, or call an application-provided service. A proposed action has no effect until an enabled tool executes it.
- Observe. The agent receives tool output: file contents, compiler diagnostics, test results, exit codes, command errors, or a permission denial.
- Adjust. It compares the observation with the goal. A failed test can lead to a revised edit, a narrower diagnosis, or a request for information.
- Finish or check in. It reports the result, leaves a change for human review, or pauses before an action that requires permission.
This is self-directed behavior, not a guarantee of correctness. Anthropic describes the distinction as an agent that directs its own processes and tool use instead of following a fixed script. Its description of the practical loop is: “it plans, acts, observes the result, adjusts, and repeats until the task is done or it needs to check in for human input.” A generated plan is a hypothesis about how to solve a problem; it is not evidence that the code is safe or that the plan will work.
What makes an agent more than a model
Calling a language model an “agent” without describing its surrounding system hides the parts that control its reach. Anthropic identifies four core components, while OpenAI’s architecture descriptions add the application server that connects the model to a running workflow.

| Component | What it contributes | Questions to ask |
|---|---|---|
| Model | Interprets language, reasons over available context, proposes actions, and evaluates returned results. | Which model is used, what context does it receive, and how does it handle uncertainty? |
| Harness | Supplies system instructions, project rules, state management, guardrails, and the protocol for calling tools. | Which instructions have priority, and how are denials, retries, and time limits handled? |
| Tools | Expose concrete operations such as reading files, applying patches, running commands, or calling services. | Can a tool only read, or can it write, delete, deploy, send, or change permissions? |
| Environment | Determines the files, processes, network destinations, credentials, and compute resources the tools can reach. | Is execution hosted, self-hosted, local, isolated, or unavailable? |
| Application server | Submits work, handles function calls, returns progress and results, and can enforce application-specific policy. | Where are calls executed, logged, retried, and translated into user-visible events? |
The same model can behave very differently in two products because these surrounding components differ. One integration may allow only repository searches and read-only analysis. Another may provide a writable workspace, a terminal, package installation, and selected network access. “The agent can code” is therefore incomplete unless it also states where it runs and what it is authorized to touch.
How planning and tool calls interact
A plan is useful because software tasks contain dependencies. The agent may need to understand a failing test before editing a source file, and it may need to run a formatter before running the full test suite. Plans also create a point for review: a person can reject an unsafe approach before any command executes.
Planning is not always a single, visible document. Some systems expose a step-by-step plan and progress events; others keep intermediate reasoning inside the harness and show only concise updates. Context can be retained across turns or reconstructed from files, tool results, and application state. Ask what state is actually persisted rather than assuming that every agent has durable memory.

Consider a hypothetical request to correct a regression in an order-total calculation. This example explains the mechanics; it is not a claim that every product follows these exact steps.
- The agent reads the issue description and identifies the affected package and expected behavior.
- It searches the repository for the calculation and opens the relevant implementation and tests.
- It proposes a change, noting assumptions such as whether tax is applied before or after a discount.
- It edits the smallest appropriate set of files through a patch tool.
- It runs the targeted test command available in the environment.
- If the test fails, it reads the assertion and output, revises the implementation or test interpretation, and tries an appropriate check again.
- It reports changed files, commands run, results, and unresolved uncertainty for a human to inspect.
The observable outputs matter as much as the plan. A clean targeted test does not prove that unrelated behavior remains correct, and a successful command can still have changed data or contacted a service. Reviewers should examine both the patch and the actions taken to produce it.

What “using tools” actually means
Tools are typed interfaces between the model and the outside world. A file-reading tool might accept a path and return text. A patch tool might accept a structured edit. A command tool might accept a command, working directory, environment variables, timeout, and input policy. A service connector might expose narrowly defined operations rather than an unrestricted network client.
Tool results become new context for the next decision. The harness can reject malformed arguments, truncate oversized output, impose timeouts, or require approval before execution. A denial is itself an observation: the agent can choose a different route, explain that it is blocked, or ask the user to change the permission.
Tools can be provided by a managed service or by the application hosting the agent. In an SDK integration, the application may execute functions in its own infrastructure and return their results. In a managed runtime, the service may execute within a hosted workspace. A direct API integration can combine model responses with an environment selected by the developer. These arrangements affect where source code, logs, credentials, and network traffic reside.
Runtime choices and their consequences
There is no universal coding-agent runtime. Common arrangements include:
| Runtime arrangement | Typical capability | Boundary questions |
|---|---|---|
| Hosted sandbox | A provider supplies an isolated workspace and selected tools for the task. | Which paths are writable, how is outbound network access controlled, and how long does the workspace live? |
| Self-hosted environment | The organization runs the agent’s tools and compute in its own infrastructure. | Which identity, secrets, internal services, and monitoring systems are reachable? |
| Local runtime | Tools execute on a developer workstation or a controlled local machine. | Can the agent read personal files, alter the host, install software, or use the user’s credentials? |
| Application-provided functions | The agent requests operations that an application server performs on its behalf. | Does each function enforce authorization independently, or does it trust the model’s request? |
| No compute environment | The system can reason about supplied text but cannot run commands or inspect a live workspace. | How will code be tested, and which claims are based only on pasted context? |
OpenAI’s agent documentation distinguishes a managed Agents API runtime, an SDK used inside an application, and direct Responses API integration. Those are integration choices, not promises that all tools or safety controls are identical. Always inspect the particular product’s execution model.

Human oversight belongs at several points
Oversight is not a single approval dialog. It can happen before work starts, while actions are being selected, and after execution.
Before execution: define the task and plan
A person can limit the repository or directories in scope, specify success criteria, and review a proposed approach before allowing tools to run. This is valuable when an apparently simple request could involve migrations, production configuration, or deletion. Plan review shifts attention from approving dozens of routine commands to evaluating the overall strategy.
During execution: permissions and approvals
Each action can be classified as allowed, approval-required, or blocked. Reading files inside a designated workspace might be automatic. Writing outside that workspace, accessing a production endpoint, installing a package, or sending a message might require a confirmation. Operations that are categorically unsafe can be blocked regardless of what the model requests.
After execution: inspect and audit
Review the resulting diff, test output, tool-call history, and any files or services touched. Logs and progress events make it possible to reconstruct what happened when a result is surprising. Recovery is safer when the system can identify which action failed, preserve the prior state, and retry only the permitted portion.
Anthropic notes that asking for confirmation on every action can become burdensome on long tasks. Excessive prompts can cause users to approve reflexively or disable protections broadly. A useful policy makes low-risk work smooth while reserving deliberate review for actions with meaningful consequences.

Sandboxing, network policy, and observability
Technical isolation and human approval solve different problems. A sandbox can restrict writable paths, process privileges, and network destinations even if the model makes a bad request. An approval policy can stop a permitted tool call until a person reviews its intent. Neither replaces the other.
- Filesystem boundaries: expose only the repository or temporary directories needed for the task; keep credentials and unrelated personal data out of reach.
- Process boundaries: restrict privilege escalation, package installation, background daemons, and access to the host operating system.
- Network boundaries: allow only necessary destinations and methods; distinguish read-only dependency retrieval from arbitrary outbound requests.
- Identity boundaries: use scoped credentials and short-lived tokens rather than a developer’s broad account.
- Telemetry: record tool requests, approvals, denials, outputs, timestamps, and relevant resource identifiers without exposing secrets in logs.
- Recovery: use version control, snapshots, dry runs, and reversible operations so a mistaken edit or command can be undone.
OpenAI’s account of running Codex safely presents sandboxing, approvals, managed network policy, and agent-native telemetry as complementary controls. These are implementation examples, not universal defaults. A product that advertises a sandbox may still differ substantially in network reach, persistence, and logging.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Prompt injection and misunderstood intent
An agent can be manipulated by instructions embedded in material it reads. A comment in a source file, a documentation page, an issue body, or a fetched web response might tell the agent to reveal secrets, ignore its rules, or perform an unrelated action. This is prompt injection: untrusted content is treated as if it were an authorized instruction.
Misunderstood intent is a separate risk. A request to “clean up old data” might mean deleting temporary files to one person and removing historical records to another. Because the agent can act repeatedly and at speed, a small ambiguity can have a larger consequence than it would in a one-shot answer.

Layered defenses
- Minimize access. Give the agent only the files, tools, credentials, and network destinations needed for the stated task.
- Separate data from instructions. Treat repository text, tickets, and web content as untrusted inputs; do not let them silently change the system policy.
- Require confirmation for impact. Gate deletion, publication, deployment, external communication, secret access, and irreversible database operations.
- Constrain execution. Use isolated workspaces, read-only mounts where practical, resource limits, and explicit command allowlists or denylists.
- Expose evidence. Show the proposed action, relevant arguments, and result so a reviewer can spot a mismatch.
- Validate independently. Run tests, static checks, security scans, and human review appropriate to the code’s risk.
No single layer makes an agent infallible. More tools and a more open environment increase the possible consequences of both malicious instructions and ordinary mistakes.
Best Value
Reducing approval fatigue without removing accountability
Teams often face a tension: frequent prompts interrupt routine work, but automatic execution can hide consequential decisions. One design pattern described by OpenAI Alignment uses a separate review agent to assess some boundary-crossing actions asynchronously. Its purpose is to reduce synchronous interruptions while retaining a review stage.
That pattern should be treated as a specific implementation choice, not proof that automated review is equivalent to human judgment. Define which actions may be delegated to automated review, what confidence or evidence is required, who receives escalations, and how a person can override or investigate the decision. Human accountability still matters for production changes, sensitive data, and ambiguous intent.
How to evaluate a coding-agent system
When comparing systems, focus on operational behavior rather than model names alone.
| Evaluation axis | Evidence to request |
|---|---|
| Planning and state | Does it maintain context across steps? Can users see the plan, progress, assumptions, and pending decisions? |
| Tool access | Which read, write, command, service, and deployment operations exist? Are arguments validated? |
| Execution environment | Is it hosted, self-hosted, local, or absent? What persists after the task ends? |
| Permission model | Which actions run automatically, which require approval, and which are blocked? |
| Network and data boundaries | Which hosts, secrets, repositories, and internal services can be reached? |
| Observability and recovery | Are tool calls and results logged? Can failed actions be retried safely or rolled back? |
Ask for a demonstration using a disposable repository and non-sensitive credentials. Observe not only whether the agent produces a plausible patch, but also whether it explains uncertainty, respects denials, stops at approval gates, and leaves a useful audit trail.
What published accuracy figures do—and do not—tell you
Official sources do not establish a general accuracy, productivity, or adoption rate that applies to all coding agents. A narrow internal evaluation from OpenAI Alignment, published April 30, 2026, reported a 99.1% auto-approval rate on escalated actions in its internal Codex sandbox-escalation traffic and 90.3% recall on synthetic overeagerness cases. Those figures describe one organization’s mechanism and a synthetic safety evaluation; they are not cross-vendor benchmarks or guarantees of safe behavior in ordinary coding work.
For a real deployment, measure the outcomes that matter to your workflow: change failure rate, test coverage of agent-authored patches, rollback frequency, approval volume, blocked-action rate, time to diagnose a failure, and incidents involving data or permissions. Keep the evaluation environment and task mix explicit so results are not mistaken for universal properties.
A practical oversight checklist
- Write the goal, scope, and definition of done in terms a reviewer can verify.
- Start in a disposable or version-controlled workspace with no unnecessary secrets.
- List the tools the task actually needs; leave unrelated tools disabled.
- Set filesystem, process, network, and identity boundaries before execution.
- Classify actions as allowed, approval-required, or blocked.
- Review the plan when the task could affect production, data retention, dependencies, or external communication.
- Inspect both the final diff and the tool history, not just the agent’s summary.
- Run independent tests and security checks appropriate to the change.
- Preserve logs and a rollback path, while preventing secrets from entering telemetry.
- Record unresolved assumptions and have a person decide when intent remains ambiguous.
Why the human remains responsible
An agent can shorten the path from an issue to a tested patch, but it does not own the project’s intent, risk tolerance, or legal obligations. The model may misunderstand a requirement, follow hostile text, choose an incomplete test, or make a technically valid change that is wrong for the business. Human oversight defines the boundary of acceptable action and supplies judgment where the system lacks context.

The useful configuration is task-dependent, but the principle is stable—minimize unnecessary access and make consequential actions visible and reviewable.
