An emergency operations plan (EOP) explains how a healthcare organization prepares for and manages an incident. A business continuity plan (BCP)—often called a continuity of operations plan (COOP) in public-sector settings—focuses on sustaining or restoring essential services during the disruption. They serve different purposes but can be integrated; the Centers for Medicare & Medicaid Services (CMS) does not require one specific plan-document format.
What’s the difference between an EOP and a BCP?
| Dimension | Emergency operations plan (EOP) | Business continuity plan (BCP) or COOP |
|---|---|---|
| Main question | How will we organize and manage this incident? | How will we sustain or restore essential services and the functions they depend on? |
| Typical focus | Command and coordination, roles, incident objectives, response actions, communications, evacuation or shelter decisions, and transition toward recovery. | Essential functions and their dependencies, including staff, leadership, utilities, supplies, information systems, alternate processes or sites, mutual aid, and recovery priorities. |
| Time horizon | Preparedness and incident management through response and initial recovery. | Before, during, and after a disruption, including sustained operations and restoration. |
| Healthcare outcome | A coordinated, safe response. | Continuity and recovery of patient care and supporting operations. |
| Relationship between documents | May provide the umbrella framework for emergency management. | May be an annex, supporting plan, or integrated content; CMS does not prescribe a single documentation format. |
These are distinctions in planning purpose, not a universal rule about how many binders or files an organization must maintain. ASPR describes an EOP as part of an organization’s wider emergency-management program: it sets management structures and methods, supports incident command and coordination with external responders, and addresses information and public messaging. Continuity planning identifies how necessary services can keep operating or be restored during the disruption. Can a hospital BCP be part of its EOP?
Yes. The plans can be separate documents or connected within one planning system. CMS says it does not require a particular format for emergency-plan documentation. Its October 28, 2016 FAQ states that “Business continuity and continuity of operations have the same meaning in the context of this rule.” That clarification is limited to the CMS rule; it does not establish that every organization or field uses BCP and COOP identically. ASPR TRACIE lists hospital resources in which a continuity plan is an annex to an EOP. In practice, a facility can use the EOP to establish incident authority and coordination, then use continuity content to specify the essential services, dependencies, and workarounds that must remain available. The appropriate arrangement depends on facility structure and applicable requirements. ASPR TRACIE continuity planning collection.

What should healthcare continuity planning cover?
Continuity is broader than IT disaster recovery. Healthcare organizations need to consider the patient-care functions they must sustain, the people and resources those functions require, and how service can continue or recover if dependencies fail. ASPR’s healthcare capabilities framework includes risks to service delivery such as utilities, electronic health records, and supply chains; it is a planning framework from the 2017–2022 capability period, not a current regulation. ASPR health care preparedness and response capabilities.
- Clinical services and patient safety: identify necessary care operations and how to maintain patient safety and quality when normal processes are disrupted.
- People and leadership: plan for staffing disruptions, leadership availability, and the roles needed to sustain priority functions.
- Utilities and facilities: consider how loss of power, water, or other essential services would affect care and what alternate arrangements are available.
- Supplies and outside support: account for critical supplies, vendors, mutual aid, and other external dependencies.
- Communications and information systems: plan for disruptions to communications, EHR access, and supporting IT, including alternate ways to carry out essential work.
- Alternate processes, sites, and recovery: define workable alternatives and priorities for restoring services.
These dependencies can fail even when the facility itself is not physically damaged. A hazard affecting the surrounding community may disrupt utilities, staffing, transport, or supplies. Continuity planning should therefore consider consequences across the service network, not just damage within the building. How do EOP and continuity planning fit into CMS requirements?
For Medicare-participating providers and suppliers, CMS describes emergency preparedness as a program with four core elements—not simply a document: risk assessment and planning; policies and procedures; a communication plan; and training and testing. CMS frames emergency management across mitigation, preparedness, response, and recovery. Preparedness includes assessing the ability to continue care, training staff, testing the plan, and revising it. CMS, Effective Health Care Provider Emergency Planning.

CMS’s core-elements page says the plan is to be reviewed and updated at least annually, and calls for coordination within the facility and with other providers and public agencies. These points apply in the context of the relevant CMS requirements; they are not a claim that every organization worldwide follows the same rule. CMS, Core EP Rule Elements.

Which plan terminology should a facility use?
COOP is terminology favored by public and government entities, while BCP is more often used in the private sector, according to ASPR TRACIE. The terms can overlap, but local usage and governing requirements matter. In the CMS rule context, CMS treats business continuity and continuity of operations as having the same meaning; that should not be generalized beyond that context. CMS requirements vary by provider type, and state requirements can differ. A facility should verify the Conditions of Participation that apply to its provider category, along with state and local rules and any applicable accreditation obligations. CMS’s high-level guidance is useful for understanding the program, but it does not substitute for checking the requirements that govern a specific facility.
