If any link is missing, the system can still produce plausible answers, but no one can verify them, correct them, or explain a failure.
The decisions that matter most are therefore not the choice of model or vector store taken alone. They are whether extraction keeps page and section locations, whether parser, chunking, and embedding changes are versioned and re-indexed in a controlled way, whether retrieval and generation are scored separately, and whether the documents themselves are treated as untrusted input. The sections below follow the order in which a team usually meets those decisions, starting with the records that have to exist before anything else is built.

What each stage has to record
The pipeline produces five artifacts, and each one derives from the one before it. Each artifact needs enough recorded metadata that it can be checked against its predecessor.

|---|---|---|
| Source document | Stable document ID, content hash or version, source location, upload time, uploader or ingesting system, approval status | Which exact file did this answer rely on, and was it allowed to be used? |
| Extraction | Parser and OCR version, page number, section heading path, extraction method for each page (embedded text, OCR, or layout parser) | Did the text come from the page we think, and how was it read? |
| Chunk and index | Chunk ID, chunker version, embedding model and version, index build ID | Which configuration produced this retrievable unit? |
| Retrieval | Query, candidate chunk IDs, scores, filters applied, including access filters | What did the system actually see before it wrote the answer? |
| Generation and citation | Model and prompt version, claim-to-passage mapping, abstention decision | Does each claim have support in a passage that was retrieved? |
A prototype usually stores only the final answer and perhaps a list of file names. Production systems need the intermediate records because an answer-level score cannot show which link in the chain broke.

Building the pipeline, stage by stage
1. Establish source identity and lifecycle
Keep the original PDF as the authoritative artifact and never let extracted text replace it. Give each logical document a stable identifier that survives re-uploads, and record the version or content hash separately so revisions can be told apart. The OWASP RAG Security Cheat Sheet recommends recording document origin and upload details, including who uploaded a document, when, from what source, and with what approval. OWASP RAG Security Cheat Sheet
Define deletion and replacement before the first load. When a document is revoked or superseded, the system needs a defined way to remove its chunks from the active index and to stop serving answers that depend on it. A withdrawn policy that remains retrievable is an evidence failure, even when the answer is accurate for an older version.
2. Extract text and structure
Separate PDFs with embedded text from scanned PDFs and image-heavy documents, because they need different handling. Scanned pages typically require OCR, and layout-aware parsing matters for multi-column pages, tables, and figures. GOV.UK’s AI Insights guidance on RAG systems makes the general point that ingesting PDFs or image files needs corresponding preprocessing:

“For instance, audio data needs a transcription pipeline to convert the audio data into text, while ingestion of PDF documents or image files requires corresponding preprocessing techniques.”

NVIDIA’s RAG documentation exposes several configurable extraction options. Read them as examples of what a configurable pipeline can expose, not as a recommendation for a particular parser or as proof that it performs best on your corpus. The documentation is published under a rolling “latest” path, so confirm option names and defaults against the release you deploy. NVIDIA RAG accuracy and performance settings

Test extraction against a hand-inspected sample that reflects the real corpus: scans, multi-column pages, tables, footnotes, diagrams, and any non-Latin text or unusual encodings. Compare the extracted output with the rendered page, not with what you expect the page to say. Include hidden text in this check. A PDF can carry text that does not appear in ordinary rendering but still appears in extracted output, where it affects both retrieval and the injection risk covered in the security section.
Treat bad extraction as an upstream evidence defect. Generation cannot restore text that was lost or misread, and a fluent answer built on a misread table value is harder to catch than an obvious failure.

3. Normalize, chunk, and attach metadata
Keep the section hierarchy when splitting content, so that a chunk about a late fee in section 4.2 carries its heading path rather than floating as a bare paragraph. Attach the document identifier, version, page range, and section path to each retrieval unit. NVIDIA’s custom-metadata documentation describes page number as processing metadata that can be used in retrieval filters and in citations. NVIDIA custom metadata documentation
An illustrative chunk record is shown below. It is a generic schema written for this example, not the format of any particular product:
{
"chunk_id": "pol-2231-v3-p014-c02",
"document_id": "pol-2231",
"document_version": "3",
"page_start": 14,
"page_end": 15,
"section_path": ["4 Fees", "4.2 Late payment"],
"extraction_method": "embedded_text",
"chunker_version": "2026-09-a",
"embedding_model": "example-embed-v2",

}
Tune chunk size and splitting strategy against your own evaluation set. They are trade-offs, not constants. Smaller units can sharpen retrieval but may strip the context needed to interpret a clause or figure. Larger units keep more context but dilute topical focus and enlarge what is passed to the model. NVIDIA’s documented defaults belong to its own blueprint and should not be read as an optimum for another corpus. NVIDIA RAG accuracy and performance settings

A 2026 arXiv preprint reports one such test. Its benchmark covers 36 Portuguese administrative documents totaling 1,706 pages and roughly 492,000 words, a manually curated set of 50 questions, and 19 pipeline configurations. The authors report that metadata enrichment and hierarchy-aware chunking contributed more to answer accuracy than the choice of conversion framework. That is a finding about one language, one document type, and one evaluation. It does not establish a universal parser or chunk size. If your corpus differs in language or document type, treat the result as a hypothesis to test, not a default. 4. Embed and index as a versioned build
Record the parser, normalization step, chunker, embedding model, and index configuration as one versioned build. GOV.UK states the operational consequence directly: “During this step, the selection of the underlying embedding method is also crucial, as altering the chunking as well as the embedding strategy necessitates re-indexing all chunks.” GOV.UK AI Insights, RAG systems
Plan rollouts around that rule. Keep the previous index build queryable until the new build passes regression evaluation, and log which build served each answer. A rollback is then a pointer change, not a rebuild under pressure.
5. Retrieve and assemble evidence
At query time, retrieve candidate passages and apply the user’s access constraints as part of the retrieval query. Rank or filter the candidates before assembling the context window. Reranking, hybrid search, and query decomposition are available features in NVIDIA’s RAG documentation. Adopt each one only where evaluation shows it helps your real query distribution, not because it is offered. NVIDIA RAG documentation index
Each passage that reaches the context window must keep a stable link to its source document, version, and location. That lets the answer cite what was actually retrieved rather than what the model later assumes was there.

Outbyte PC Repair FREEClear out junk files and repair common Windows errorsFree Scan →Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →Evaluate retrieval and generation separately
Build an evaluation set from real user questions, with expected answers or the passages that should support them wherever you can establish them. Score the stages separately rather than blending them into one number. NVIDIA’s evaluation documentation lists answer accuracy, context relevancy, response groundedness, and context recall as distinct measures, and each points to a different failure. NVIDIA RAG evaluation documentation
| Measure | Stage it diagnoses | A low score usually means |
|---|---|---|
| Context recall | Retrieval | The supporting evidence was not retrieved at all |
| Context relevancy | Retrieval | The retrieved passages contain material that does not bear on the question |
| Response groundedness | Generation | The answer makes claims the retrieved context does not support |
| Answer accuracy | End to end | The final answer disagrees with the expected answer |
Four measures are not enough for a production system. Add checks that fit the application:
- Citation correctness: whether the cited passage supports the specific claim it is attached to.
- Abstention behavior on questions the corpus cannot answer.
- Stale-document hits, meaning answers that rely on a superseded or withdrawn version.
- Permission violations, meaning retrieved passages a user should not have been able to see.
- Latency and cost under the expected workload.
Re-run the evaluation after any material change to parsing, OCR, chunking, embeddings, retrieval, reranking, prompts, or model versions. Keep trace data that records which source versions and passages were retrieved for each answer, so a regression can be attributed to a stage instead of to the whole system.
Make the model cite claims, not documents
A citation that points to a relevant document is weaker than one that points to the passage supporting the specific claim. Prompt the model to ground factual statements in the supplied passages, then check the claim-to-passage relationship after generation rather than trusting the prompt alone. When retrieval does not support an answer, the system should say so instead of producing a best guess.
Return the answer as structured data: each claim with its supporting chunk IDs, plus an explicit abstention state. A practical validation checklist:
Best Value
- Every factual sentence maps to at least one retrieved chunk ID.
- The cited document version, page, and section match the chunk’s stored location.
- Numbers, dates, and table values in the claim match the passage text exactly.
- If no passage supports the answer, the response is an abstention, not an estimate.
Treat documents as a security boundary
The OWASP RAG Security Cheat Sheet covers risks and controls across ingestion, embedding generation, vector storage, retrieval, response generation, output validation, and downstream agent integration. Its central requirement for this design is that retrieved documents must be treated as potentially adversarial input. A PDF can contain instructions aimed at the model, and those instructions reach the model through the same extracted text as legitimate content. OWASP RAG Security Cheat Sheet
Controls that follow from that requirement:
- Enforce access checks on the source data, and apply them before any passage reaches generation, so that a user cannot receive text they could not open in the source system.
- Record provenance for every document, including uploader, time, source, and approval, and show it in traces.
- Restrict who can add documents to the corpus, and log each approval.
- Validate model output before it reaches users or any downstream tool, particularly when an agent acts on the answer.
Neither the OWASP cheat sheet nor the GOV.UK guidance page shows a publication year, so check the current revision of each before citing it in a design review.
Choosing implementation components
Compare candidate components on the same axes, measured on your corpus:
- Extraction fidelity for embedded text, scans, tables, charts, multi-column layouts, and your languages.
- Preservation of headings, page locations, and the metadata needed for useful citations.
- Retrieval recall and answer groundedness on representative questions.
- Update, deletion, re-index, and rollback behavior.
- Access control, tenant isolation, and auditability.
- Latency, operating cost, deployment constraints, and operational effort under the expected workload.
Component categories you will likely evaluate include PDF parsing and OCR tools, managed RAG services, vector retrieval stores, and reference pipelines such as NVIDIA’s RAG Blueprint. A neutral, current head-to-head comparison across these vendors is not available in the sources cited here. No single parser, embedding model, vector store, or chunk size can be named as the best choice without a benchmark on your own workload. Treat any vendor’s published benchmark as a claim to verify against your corpus.
Diagnosing failures: which stage broke
When an answer is wrong, start with the trace rather than the prompt. The table maps common symptoms to the stage where they most often originate.
| Symptom | Likely stage | First check |
|---|---|---|
| The correct passage never appears in retrieval results | Retrieval or chunking | Low context recall. Check whether the answer spans a chunk boundary and whether a metadata filter excluded the passage. |
| The passage was retrieved, but the answer ignores or contradicts it | Generation | Low response groundedness. Review the prompt and the order of passages in the context window. |
| A table value or figure is wrong or missing | Extraction | Compare the extracted text with the rendered page at the same page number. |
| The citation names the right document but the wrong page | Metadata mapping | Compare the stored page range with the rendered page, and confirm that one page-numbering convention is used across the pipeline. |
| An answer relies on a superseded or withdrawn version | Lifecycle | Confirm that the old document ID and version are absent from the active index build. |
| Quality dropped after a chunker or embedding change | Versioned change | Confirm that the whole index was rebuilt with the new configuration, then re-run the regression set. |
| Confident answers to questions the corpus cannot answer | Abstention | Test with unanswerable questions and confirm the abstention state triggers when no passage supports the claim. |
The Bottom Line
If you are converting a PDF prototype, make three changes first, in this order: attach page and section locations to every chunk, version the index build so it can be rolled back, and log the retrieved passages for every answer. Evaluation, claim-level citation checks, and the security controls can then be layered onto records that already show where each answer came from.
