Free Bootstrap Survey Forms: Choose and Adapt a Bootstrap 5 Template

Start with a Bootstrap 5 survey-form layout, choose controls that match your questions, and connect the finished form to a real submission destination.

Free Bootstrap survey forms give you the front-end structure for asking questions, but the template alone does not collect or store responses. For a new survey, start with a Bootstrap 5 example, adapt its controls to the kinds of answers you need, and connect the form to a submission destination such as an application endpoint or form service.

What a free Bootstrap survey form includes

Bootstrap provides form controls and layout styles you can combine into a questionnaire. Its survey-form example is a third-party Bootstrap 5 and HTML starting point. Bootstrap’s own v5.3 Forms documentation explains the controls and patterns used to build and style forms.

A template is front-end code: it describes the fields and how they appear in a browser. It does not, by itself, establish that responses are sent to a server, saved, or analyzed. Those behaviors require a separate submission destination and implementation suited to your application.

Choose controls that match the questions

Match the input to the answer a respondent should provide. Use one radio group when the person must choose one option, checkboxes when multiple options are valid, and a textarea when a free-form response is genuinely useful. For other answers, select an appropriate HTML input type, such as an email input for an email address.

  • One answer: Use radio buttons with a shared question label and a distinct label for each option.
  • More than one answer: Use checkboxes and make clear that respondents may select multiple options.
  • Written response: Use a textarea, with a prompt that sets expectations about what to include.
  • Optional question: Mark it visibly as optional rather than expecting respondents to infer that it can be skipped.

Make every field accessible

Bootstrap’s v5.3 guidance states: “Ensure that all form controls have an appropriate accessible name so that their purpose can be conveyed to users of assistive technologies.” An explicit <label> associated with each input is a straightforward way to provide that name. Do not rely on placeholder text as the only label: it can disappear as someone types and does not replace a properly associated label.

When a field needs instructions or an error message, connect that text to the control with aria-describedby. Bootstrap documents this association in its v5.2 Forms documentation. For example, the input can reference both an instruction and an error element by their IDs. Ensure that any error is also communicated in a way that does not depend on color alone.

Use a semantic <form> element around the question groups. Bootstrap’s v5.0 layout documentation notes that form controls stack vertically by default, because most are block-level and full-width. Keep that layout when it makes the questions easy to follow; if you arrange fields in columns, check that the order remains clear and usable at narrow viewport widths.

A simple Bootstrap 5 survey form

This standalone example uses Bootstrap 5 classes and native HTML controls. Its action is intentionally left out: add a real submission destination and the method appropriate for your application before expecting answers to be received.

<form>
  <fieldset class="mb-4">
    <legend class="fs-5">How often do you use the service?</legend>

    <div class="form-check">
      <input class="form-check-input" type="radio" name="frequency" id="frequency-daily" value="daily">
      <label class="form-check-label" for="frequency-daily">Daily</label>
    </div>
    <div class="form-check">
      <input class="form-check-input" type="radio" name="frequency" id="frequency-weekly" value="weekly">
      <label class="form-check-label" for="frequency-weekly">Weekly</label>
    </div>
  </fieldset>

  <fieldset class="mb-4">
    <legend class="fs-5">Which features do you use? Select all that apply.</legend>

    <div class="form-check">
      <input class="form-check-input" type="checkbox" name="features" id="feature-reports" value="reports">
      <label class="form-check-label" for="feature-reports">Reports</label>
    </div>
    <div class="form-check">
      <input class="form-check-input" type="checkbox" name="features" id="feature-sharing" value="sharing">
      <label class="form-check-label" for="feature-sharing">Sharing</label>
    </div>
  </fieldset>

  <div class="mb-4">
    <label class="form-label" for="comments">What would you improve? (optional)</label>
    <textarea class="form-control" id="comments" name="comments" rows="4"
      aria-describedby="comments-help"></textarea>
    <div class="form-text" id="comments-help">Please do not include sensitive information.</div>
  </div>

  <button class="btn btn-primary" type="submit">Submit survey</button>
</form>

The fieldsets and legends identify related choices as a group; each individual choice still has its own label. Replace the sample questions with your own, and confirm that the server or form service expects the submitted field names and values shown in the markup.

Choose a layout that stays readable

Bootstrap’s default stacked form is a sensible starting point for a questionnaire because each question can occupy its own row. A multi-column arrangement can make a short form more compact, but it may make a longer survey harder to scan or awkward on a phone. Decide based on the number and type of questions, then check the layout at the viewport widths your audience uses. Bootstrap documents form layout patterns in its v5.0 layout guide.

Plan submission and validation separately

Before publishing, decide where responses should go and how your implementation handles them. The HTML template is not evidence of a working collection or analysis service.

  • Set the form’s submission destination and method for your application or chosen form service.
  • Define how submitted fields are validated, stored, and made available to whoever needs the results.
  • Decide whether the survey needs a consent notice or other explanation before collecting responses.
  • Provide clear success and error feedback, and associate field-specific instructions or errors with their controls.
  • Test keyboard navigation and the form with assistive technology; do not treat visual styling alone as proof that the experience works for everyone.

Keep Bootstrap versions and validation guidance aligned

Bootstrap documentation is versioned, and class names or validation patterns should be checked against the version actually loaded by your project. The example above is for Bootstrap 5. The v4.5 documentation records a specific limitation: its custom client-side validation styles and tooltips were not exposed to assistive technologies. That is a caveat about that documented approach and version, not a claim that every Bootstrap validation method is inaccessible.

Bootstrap’s release post dated 2021-05-05 said, “We’ve overhauled our Forms documentation and components.” Treat that as historical context; use the documentation for your project’s exact version when adapting controls or validation behavior.
