The steps describe treatment intensity—not a fixed ladder everyone must climb. A clinician chooses and reviews treatment based on symptoms, future risk, lung function, and the person’s circumstances.
How is asthma treated?
Asthma treatment aims to control symptoms and reduce the risk of serious flare-ups. Inhaled corticosteroid (ICS)-containing treatment is central to GINA’s current pathways. GINA advises against treating asthma with only an as-needed short-acting beta2-agonist (SABA), even when symptoms seem infrequent.

The right treatment depends on factors such as age, symptom pattern, past exacerbations, lung function, other health conditions, inhaler technique, ability to follow the regimen, cost, and local availability. The medicine and dose must come from an individual clinical assessment; a step number is not a prescription.
For adults and adolescents in the trials described by GINA, as-needed low-dose ICS-formoterol was associated with about two-thirds fewer asthma-related emergency-room visits or hospitalizations than SABA alone, and over one-third fewer than low-dose ICS plus as-needed SABA. These are comparative trial results for the populations and alternatives GINA describes, not a prediction or guarantee for any one person.

What are the treatment steps for adults and adolescents?
GINA 2026 presents two tracks. Track 1 is preferred and uses low-dose ICS-formoterol as the reliever throughout. Track 2 is an alternative when ICS-formoterol is unavailable, or for some people who are stable, adhere to ICS-containing maintenance treatment, and prefer to continue it. The tracks are alternatives, not a single regimen to combine.
| Step | Track 1: preferred | Track 2: alternative |
|---|---|---|
| 1–2 | As-needed low-dose ICS-formoterol. | Anti-inflammatory reliever treatment when available. If SABA is used as the reliever, GINA describes pairing it with ICS as directed. |
| 3 | Low-dose maintenance-and-reliever therapy (MART) with ICS-formoterol. | Daily ICS-containing controller treatment is added as treatment intensity increases. |
| 4 | Medium-dose MART with ICS-formoterol. | Daily ICS-containing controller treatment at a higher step of care. |
| 5 | Assess phenotype; consider add-ons such as LAMA or biologic treatment, or a trial of high-dose maintenance ICS-formoterol. | Specialist assessment and possible add-on treatment. |
At Step 5, biologic options listed by GINA include anti-IgE, anti-IL5/5R, anti-IL4Rα, and anti-TSLP treatments. Eligibility, approvals, and availability vary, so specialist assessment is needed to determine whether an add-on is appropriate.

GINA also reports that overuse of SABA—at least three 200-dose canisters dispensed in a year—is associated with increased exacerbation risk; at least 12 canisters, and possibly fewer, is associated with increased risk of asthma death. These are associations, not proof that the canisters themselves cause the outcomes. Discuss frequent reliever use with a clinician.

| Step | Options described by GINA 2026 |
|---|---|
| 1 | Anti-inflammatory reliever treatment. |
| 2 | Daily low-dose ICS. |
| 3–4 | Controller choices include medium-dose ICS, low-dose ICS-LABA, or ICS-formoterol MART. |
| 5 | Phenotypic assessment and consideration of higher-dose ICS-LABA or add-ons such as LAMA and selected biologics. |
GINA says to consider expert referral when a child requires Step 4. Its 2026 summary guide does not detail the pathway for children age 5 and younger; do not apply the 6–11 pathway to them.
Why might treatment move up or down?
GINA describes care as “Assess, Adjust, Review.” A clinician considers current symptom control and future risk, including lung function and exacerbations, then reviews the response after treatment starts or changes. Before stepping treatment up, the clinician should look for fixable reasons asthma is not controlled rather than automatically increasing the dose.

- Check adherence and practical barriers: Discuss whether the prescribed treatment is being used and whether device skills, physical ability, cost, or access make it hard to follow.
- Review risks and other conditions: Consider exacerbation risks, environmental factors, and comorbidities that may affect control.
- Include the person’s preferences: Device choice and regimen should account for patient or caregiver goals and preferences as well as clinical factors.
When asthma has been well controlled for three months or more, GINA says a clinician can consider stepping treatment down. Any reduction should be planned and reviewed; do not change or stop prescribed controller treatment on your own.

A written action plan is made with a healthcare provider and tailored to the person. NHLBI says it should explain how to avoid triggers, recognize an attack, follow usual and worsening-symptom medicine instructions, know when to contact a provider or go to an emergency department, and whom to contact in an emergency. A peak-flow meter belongs in the plan only when the provider recommends peak-flow monitoring; NHLBI notes that recorded readings can be compared over time and a low reading may warn of an attack before symptoms are noticed.

During worsening symptoms, follow the personal action plan and use the reliever as soon as symptoms start, as NHLBI advises. Seek emergency care for a serious attack or if symptoms do not improve soon after at-home medicines. Follow local emergency guidance. Emergency clinicians may deliver medicine by nebulizer; that does not make unsupervised home nebulizer treatment a general substitute for the prescribed plan.
Which guideline applies where you live?
GINA’s 2026 summary guide provides the age-specific pathways above. In the United States, the NHLBI/NAEPP clinician guide presents focused updates published in 2020 and step diagrams for adults and children ages 5–11. It emphasizes checking adherence, inhaler technique, environmental factors, and comorbidities before increasing treatment, then reassessing after a step-up. Its framework is not identical to GINA’s 2026 pathway, so a regimen should be understood in the context of the guideline and country it follows.
Medicines, device approvals, access, and emergency instructions can differ by jurisdiction and change over time. Follow the clinician-issued plan and local guidance rather than choosing a regimen from a general step chart.
