Most people can have a routine cholesterol test without fasting. A fasting and a nonfasting lipid panel measure the same main cholesterol and blood-fat components; recent food mainly affects triglycerides. If your result is high or your clinician needs to investigate a specific condition, they may ask you to repeat the test fasting. Follow the preparation instructions from the clinician or laboratory that ordered your test.
What does a cholesterol test measure?
A standard lipid panel usually reports total cholesterol, HDL cholesterol, LDL cholesterol, and triglycerides. “Fasting” and “nonfasting” describe whether you ate before the blood draw, not two different kinds of panel. The American Heart Association’s patient guidance says a lipid panel may be done either way depending on the person’s situation.

An advanced lipid panel may include additional measures, such as apolipoprotein B. That is a separate distinction from whether the sample was fasting.

Triglycerides are the routine lipid measure most affected by recent food. A 2016 joint consensus from the European Atherosclerosis Society and European Federation of Clinical Chemistry and Laboratory Medicine summarized observational comparisons after habitual meals. It reported these maximum mean changes 1–6 hours after eating:

| Measure | Maximum mean change after a habitual meal | Source and qualification |
|---|---|---|
| Triglycerides | +0.3 mmol/L (26 mg/dL) | 2016 EAS/EFLM consensus; mean change observed 1–6 hours after habitual meals. |
| Total cholesterol | −0.2 mmol/L (8 mg/dL) | 2016 EAS/EFLM consensus; mean change observed 1–6 hours after habitual meals. |
| LDL cholesterol | −0.2 mmol/L (8 mg/dL) | 2016 EAS/EFLM consensus; mean change observed 1–6 hours after habitual meals. |
| HDL cholesterol, apolipoprotein A1, apolipoprotein B, and lipoprotein(a) | No effect from fasting status reported | 2016 EAS/EFLM consensus. |
These are averages from observational comparisons, not predictions for an individual. The consensus found that fasting and nonfasting concentrations vary similarly over time and have comparable usefulness for predicting cardiovascular disease. Its recommendation is to routinely use nonfasting samples for lipid profiles; see the 2016 EAS/EFLM consensus.
Is a nonfasting cholesterol test accurate?
For routine lipid assessment and cardiovascular risk estimation, guidance accepts either a fasting or nonfasting profile. The 2018 AHA/ACC guideline says that for adults aged 20 or older who are not taking lipid-lowering therapy, either type is effective for estimating atherosclerotic cardiovascular disease (ASCVD) risk and documenting baseline LDL-C. It recommends a fasting repeat if the initial nonfasting triglyceride result is at least 400 mg/dL (4.5 mmol/L). The recommendation appears in the AHA/ACC guideline materials.

A meal does not automatically make a cholesterol test inaccurate or unusable. The practical difference is that triglycerides can be higher after eating, so a clinician may need a fasting result to clarify an elevated value or answer a diagnostic question.
When might a fasting repeat be requested?
Guidelines use different triglyceride levels as a point to consider fasting follow-up. They should not be collapsed into one universal cutoff:

| Guidance | When fasting follow-up is indicated or considered | How to read the threshold |
|---|---|---|
| AHA/ACC, 2018 | Repeat fasting when the initial nonfasting triglyceride level is at least 400 mg/dL (4.5 mmol/L). | Specific recommendation for follow-up after a nonfasting result. |
| EAS/EFLM, 2016 | Fasting sampling may be considered when nonfasting triglycerides exceed 5 mmol/L (440 mg/dL). | Advisory threshold in the European joint consensus. |
A clinician may also order fasting sampling to investigate a very high triglyceride result, a suspected inherited lipid disorder, or another context-specific concern. The European consensus treats fasting and nonfasting profiles as complementary options, rather than assuming everyone needs a fasting test.

The American Diabetes Association’s 2026 Standards use different thresholds according to sampling status: hypertriglyceridemia is defined as fasting triglycerides above 150 mg/dL or nonfasting triglycerides above 175 mg/dL. These are clinical interpretation thresholds, not instructions that everyone should fast. For adults above them, the Standards recommend addressing lifestyle factors, secondary causes—including diabetes, chronic liver or kidney disease, and hypothyroidism—and medications that can raise triglycerides. See the ADA Standards of Care in Diabetes—2026.

A triglyceride result alone does not determine whether treatment is needed. A clinician interprets it alongside your health history, other test results, and the reason the test was ordered.
Quick Recap
What should you do before the blood draw?
- Check the order or appointment instructions. The clinician or laboratory may specify fasting for your particular test or diagnostic question.
- If the instructions are unclear, contact the ordering clinician or laboratory. Ask whether to fast and what to do about water and medicines.
- Do not change medication or water intake on your own. Preparation directions depend on the order; the cited guidance does not establish one universal fast duration or medication rule.
- If you ate despite being told to fast, tell the laboratory or clinician. They can decide whether to proceed or reschedule based on the test and reason for testing.
Fasting and nonfasting panels at a glance
| Question | Fasting panel | Nonfasting panel |
|---|---|---|
| What it measures | Standard lipid components such as total cholesterol, HDL, LDL, and triglycerides. | The same standard components. |
| Typical role | May be useful after a high triglyceride result or for a specific diagnostic question. | Suitable for most routine testing and cardiovascular risk assessment under the cited guidance. |
| Main interpretation consideration | Uses fasting triglyceride thresholds. | Recent food may raise triglycerides; a high result may prompt fasting follow-up. |
| Preparation | Follow the instructions attached to your specific order. | Often avoids scheduling around a fast; the EAS/EFLM consensus cites improved compliance as a reason to use nonfasting profiles. |
