Enterprise RAG: Build Governance Into the Retrieval Architecture

A production RAG system needs governance across ingestion, storage, retrieval, generation, evaluation, and audit—not just vector search and an LLM.

Enterprise RAG is not just vector search followed by an LLM. A production system must govern the whole information flow: how sources enter the knowledge base, who can retrieve them, how fresh and traceable they are, how the model uses them, and how failures are evaluated and investigated. Retrieval supplies context; it does not, by itself, guarantee truth, authorization, safety, or compliance.

What changes when RAG moves from a demo to a governed system?

Retrieval-augmented generation (RAG) retrieves information from an external or enterprise source and supplies it to a model as context for an answer. That gives a model a way to use current or domain-specific material without storing that material in its parameters. It also creates new points where data can be exposed, misused, become stale, or lose its provenance.

A narrow implementation treats RAG as a retrieval task: split documents into chunks, find likely matches, and pass them to a model. A governed implementation treats ingestion, storage, retrieval, generation, and feedback as one operational system. Controls must follow the data across that boundary—not live only in a prompt or a model safety setting.

What does the end-to-end architecture look like?

A useful reference design separates the work that prepares knowledge from the work that answers queries. Google Cloud’s RAG reference architecture describes these as ingestion and serving subsystems, with design trade-offs involving security and compliance, reliability, cost, and performance. Its architecture page was last reviewed on 2025-11-10 UTC.

Ingestion and indexing

Ingestion validates source material, applies classification and identity metadata, prepares content for search, creates embeddings where used, and builds or updates an index. The index is a derived representation of sources, so it needs a lifecycle: source changes, deletions, and permission changes must be reflected in the searchable data.

Serving and response generation

Serving accepts a query, identifies the caller and their permissions, retrieves eligible material, supplies selected context to the model, and returns an answer. The system should preserve the relationship between the answer and the source material used, so a user or investigator can inspect what grounded the response.

Governance across both paths

Ingestion and serving are connected by shared controls: identity, classification, access policy, provenance, freshness expectations, logging, evaluation, and incident response. AWS guidance on grounding and RAG also calls out source quality, traceability, access control, reindexing, prompt observability, audit logging, feedback, memory controls, and token-budget optimization as operational responsibilities.

Which controls belong at each stage?

Stage Controls to design for Failure to prevent or detect
Ingestion Validate and filter sources; record provenance and versions; treat contributed content as untrusted. Poisoned or malicious material entering the knowledge base unnoticed.
Storage and identity Encrypt data; apply least privilege; attach classification and authorization metadata for roles, users, and tenants. Data being available to a caller who lacks permission, or boundaries being lost between tenants.
Retrieval Enforce authorization and metadata filters before context reaches the model; retain source identity; measure relevance. Irrelevant, unauthorized, or untraceable chunks being used as evidence.
Generation Constrain unsafe or out-of-policy responses; check for sensitive disclosure; make supporting evidence inspectable. A response exceeding the evidence, exposing sensitive material, or following hostile instructions in a source.
Operations and lifecycle Define freshness expectations; reindex changed or deleted content; capture useful traces; provide a route to report bad grounding. Stale knowledge persisting, or an incident being impossible to reconstruct.

These are design responsibilities, not guarantees supplied by the RAG pattern. AWS identifies risks including exfiltration, poisoned sources, unauthorized access, uncontrolled disclosure, and weak provenance. The exact controls depend on the system’s sources, identity model, and operating environment.

Why must retrieved documents be treated as untrusted?

Retrieved content is data, not an instruction channel. A document, forum post, or support ticket can contain text intended to manipulate the model. If the model treats such text as authoritative directions, an attacker may influence behavior indirectly through material that enters the knowledge base rather than through the user’s prompt.

A 2026 Mandiant special report describes a customer-service agent whose knowledge base included untrusted forum comments and support tickets, creating a path for hidden malicious instructions in retrieved material to be interpreted as system-level directions. The defensive implications are practical: validate sources, keep retrieved content distinct from trusted instructions, isolate data by role or tenant, limit tool privileges, and apply runtime guardrails. A prompt telling the model to ignore malicious content is not a substitute for access controls and constrained capabilities.

How should a team evaluate RAG quality?

Measure retrieval and generated answers separately. If an answer is wrong, the cause might be missing or irrelevant context, a model that misused good context, or both. One overall score can conceal which stage needs repair.

Evaluate retrieval

Use representative queries and, where available, labeled relevance judgments to assess whether the system retrieves the expected documents or passages. Inspect context quality as well as ranking: the right source can still produce poor context if chunk boundaries, filters, or selection are wrong.

Evaluate the answer

Assess groundedness, relevance, completeness, correctness, and utilization of retrieved context as distinct dimensions. Groundedness asks whether claims are supported by the retrieved material; it does not establish that the answer includes every important point or answers the question. Microsoft’s RAG evaluator guidance describes groundedness as a precision concern and completeness as a recall concern. Its Azure evaluation guidance also names relevance, correctness, and utilization among useful dimensions.

Test with representative user questions and source documents, then diagnose failures at the appropriate stage. Because model responses are nondeterministic, Microsoft advises considering target ranges rather than treating one score as a universal pass/fail threshold. Re-run evaluations and monitor quality when sources, indexing, retrieval configuration, prompts, or model versions change. No single metric is a complete quality score.

What evidence should logs preserve for an investigation?

Logs should make it possible to reconstruct what the system did without collecting more sensitive data than necessary. Microsoft’s governance guidance describes preserving the initiating identity, prompts, retrieved context, model and version, safety decisions, tool calls, approvals, outputs, and correlation identifiers in tamper-resistant stores. Its page was last updated on 2026-08-01.

Decide what to retain, who may inspect it, and how long it remains available. Trace records can themselves contain confidential prompts or retrieved documents, so auditability must be balanced with privacy, access restrictions, and retention requirements.

How should teams compare implementation approaches?

There is no universal architecture or vendor ranking established by the cited guidance. Compare options against the workload and controls the organization actually needs:

  • Permissions and isolation: Can access rules follow identity, role, and tenant boundaries through retrieval and into the context sent to the model?
  • Source lifecycle: How are provenance, versioning, freshness, deletion, and reindexing handled?
  • Security controls: Can teams configure ingestion validation, encryption, retrieval filtering, output checks, and audit logs?
  • Evaluation: Can retrieval and answers be measured separately, including against labeled ground truth where available?
  • Operational fit: How do reliability, latency, performance, and cost behave for the expected workload?
  • Governance evidence: Can an incident be reconstructed from records of identity, prompts, retrieved context, model version, decisions, and output?

Managed services and self-managed components can differ across these dimensions. Evaluate the actual integration path—including identity propagation, deletion behavior, observability, and policy enforcement—rather than assuming a product’s RAG capability settles governance.
