An engineered antibody can bind sperm and make them stick together, potentially slowing or stopping them before they pass through cervical mucus. Early laboratory, animal-model and human postcoital-test results are promising, but none has yet shown that the approach prevents pregnancy in routine use.
How an antibody could stop sperm
Antibodies normally bind specific targets. In this approach, researchers use antibody fragments that recognize a target on the sperm surface, then attach multiple sperm-binding fragments to an antibody scaffold. A single engineered antibody can therefore bind more than one sperm at a time, cross-linking cells into clusters, or agglutinates.

The intended effect is local: after vaginal delivery, the antibody would rapidly clump sperm and impede their movement through cervical mucus. This is a non-hormonal strategy; it aims to act on sperm rather than alter hormone levels. The proposed mechanism is not the same as physically trapping sperm in a conventional barrier method.

What the engineered-antibody studies have found
Laboratory and sheep-model results
In a 2021 paper in Science Translational Medicine, Shrestha and colleagues described sperm-binding antibodies carrying six to ten Fab antigen-binding fragments, derived from an antibody isolated from an immune-infertile woman. In the reported assays, the engineered constructs agglutinated sperm at least 10 to 16 times more potently and rapidly than the parent IgG.

In a sheep-vagina model, 33 micrograms of the 10-Fab construct reduced progressively motile sperm by more than 99.9%. That is a preclinical result in an animal model, not a measurement of pregnancy prevention in people. Agglutination in a laboratory assay and reduced motile sperm in an animal model are useful evidence about biological activity, but they do not establish how often a contraceptive would prevent pregnancy in real-world use.
A later construct: LamH10
A 2025 study reported laboratory and sheep-model work on LamH10, a decavalent sperm-binding antibody construct. Its preclinical findings are a separate line of evidence from the human study of the HC4-N antibody in the ZB-06 film. LamH10 has no human efficacy result established by the studies described here.

What the first-in-woman ZB-06 study measured
ZB-06 is an investigational vaginal film containing HC4-N, a human contraceptive antibody. The study protocol describes the trial formulation as an intravaginal film containing 20 mg of HC4-N drug substance; that figure describes the investigational formulation, not a consumer product or recommended dose.
Thurman and colleagues’ 2023 Phase 1 study in the American Journal of Obstetrics and Gynecology assessed safety and a postcoital-test surrogate: the number of progressively motile sperm found in ovulatory cervical mucus after intercourse. Twenty women enrolled, but eight couples completed all study visits. After a single film use before intercourse, the mean was 0.04 progressively motile sperm per high-power field, compared with 25.9 at baseline without the product. At a follow-up visit without product use, the mean was 47.4.

Those measurements indicate activity on the study’s sperm-count endpoint, and the follow-up count shows sperm were again present when the product was not used. They are not pregnancy rates. The study was small, and a postcoital sperm count cannot by itself tell readers how effective the film would be at preventing pregnancy over months of typical use. The authors concluded that ZB-06 was a candidate warranting further development and testing—not that contraceptive effectiveness had been demonstrated.

For a changing development status, consult the latest trial-registry entry or developer update. Until pregnancy-prevention effectiveness and appropriate use are established, the investigational antibody film should not be treated as an available or proven contraceptive.

- Pregnancy outcomes: A study must measure pregnancies, not only sperm in cervical mucus, to establish contraceptive effectiveness.
- Use beyond a single test: Further studies need to assess repeated and typical use, including whether the formulation works reliably when used as intended.
- Safety and tolerability: Phase 1 work assessed safety, but a small study cannot answer every question about broader use.
- Clear product-specific evidence: Results for one antibody or film should not automatically be attributed to another construct, such as LamH10.
The underlying idea is biologically distinct from hormonal contraception, and the reported studies show that engineered antibodies can strongly affect sperm movement. The key unanswered question is whether a specific formulation can prevent pregnancy safely and reliably in people.
