The American Dental Association says either type can work effectively when used appropriately.
What the evidence says about cleaning
The main direct comparison is a 2014 Cochrane review by Yaacob and colleagues. It included 56 studies published from 1964 to 2011, with 5,068 participants randomized; 51 trials, involving 4,624 participants, contributed data to the meta-analysis. Most participants were adults, and more than half of the included studies tested rotation-oscillation brushes. The review’s evidence search was current through January 23, 2014.

On average, powered brushes reduced plaque and gingivitis scores more than manual brushing in the measured periods:

| Measured outcome | At one to three months | After three months |
|---|---|---|
| Plaque | 11% average reduction | 21% average reduction |
| Gingivitis | 6% average reduction | 11% average reduction |
These are average reductions in measured indices reported by the Cochrane review, not the share of people cured and not a direct estimate of cavities prevented. The review rated the plaque and gingivitis evidence as moderate quality, found substantial variation among trials, and said the clinical importance of the differences for long-term dental health remained unclear.

The strongest body of powered-brush evidence in that review was for rotation-oscillation brushes, which showed statistically significant reductions in plaque and gingivitis at both measured time points. The finding should not be treated as proof that every powered-brush motion or current model performs the same way. The review was published in 2014, and its search ended in January of that year; these figures should not be presented as the result of a newly conducted or up-to-date product comparison. Does the average advantage matter for you?
An average advantage in plaque or gingivitis scores is not a guarantee of a better result for an individual. Brushing thoroughly and consistently matters, and the brush you can use comfortably across all tooth surfaces is a practical choice. ADA guidance is that “Either manual or powered toothbrushes can be used effectively.”

The ADA also summarizes one four-week study of people who started as manual-brush users with gingivitis. In that study, 20% of the manual-brush group moved from gingivitis to healthy gums, compared with 46.7% of participants using an oscillating-rotating brush in sensitive mode and 60% using its daily-clean mode. Those are results from one study, not a general prediction for all users or a substitute for the broader Cochrane comparison. The ADA page notes that its article does not necessarily indicate ADA endorsement. (ADA: Electric vs. manual toothbrushes)
How to choose between an electric and manual toothbrush
Choose for comfort and control
Consider which brush you can use gently for the full two minutes and reach around the inner, outer, and chewing surfaces. A powered brush may be easier for some people with limited dexterity, disabilities, or braces, but individual comfort and ability matter more than the category label.

Weigh cost and upkeep
Powered brushes can cost more. Account for compatible replacement heads, charging or batteries, and whether replacement heads are easy to obtain. A soft-bristled manual brush avoids charging and model-specific heads; a powered brush requires a compatible replacement head when its bristles wear. Current prices and availability depend on the product and seller.
Treat features as conveniences, not proof
A built-in timer may help you brush for the recommended two minutes, but a timer by itself is not evidence of better clinical results. For powered models, follow the manufacturer’s instructions rather than assuming every brush head should be moved like a manual brush.

Check product assurance if relevant
The ADA Seal of Acceptance means a product’s safety and efficacy evidence has been reviewed under the program’s requirements. Check the ADA’s current listings for a product’s status rather than assuming a particular brush has the Seal. (ADA: Toothbrushes)
How to brush effectively with either type
- Brush twice a day for two minutes with fluoride toothpaste, as recommended by the ADA.
- Use a soft-bristled brush and gentle pressure. With a manual brush, position the bristles at about a 45-degree angle to the gumline and use short, gentle strokes, covering the inner, outer, and chewing surfaces.
- If using a powered brush, follow its product instructions and make sure you cover all tooth surfaces for the full brushing time.
- Replace the toothbrush or powered-brush head every three to four months, or sooner if the bristles look visibly matted or frayed.
People with oral-health conditions or special circumstances can ask a dentist which brush and technique suit their needs. (ADA: Home Oral Care)
