A veterinarian may consider prescription medication, while diet and supplements have more limited evidence; predictable routines and suitable enrichment can support day-to-day comfort. The right plan depends on first checking whether pain, sensory loss, or another illness is causing or worsening the changes.
What treatment can—and cannot—do
Canine cognitive dysfunction is a progressive syndrome that may affect a dog’s orientation, social interaction, sleep, house training, learning and memory, activity, and anxiety. The pattern is often described by the DISHAA domains: disorientation; changes in interaction; sleep disruption; house soiling; altered learning or memory; activity changes; and anxiety.

These signs do not establish a diagnosis on their own. Pain, declining sight or hearing, organ disease, and neurologic problems can resemble or add to cognitive changes. Cornell University’s Cognitive Dysfunction Syndrome guidance, last updated October 21, 2025, says there is no one treatment or cure. Management aims to reduce troubling signs and support quality of life, not reverse the underlying condition.
How the options compare
| Option | Veterinary involvement | Aim and evidence | Important considerations |
|---|---|---|---|
| Selegiline | Requires veterinary diagnosis, prescription, and monitoring. | The American Animal Hospital Association (AAHA) identifies it as the only drug labeled for canine cognitive dysfunction. AAHA reports it can be effective in up to 70% of dogs; this is an upper estimate, not an individual dog’s odds. | Potentially toxic interactions with antidepressants, narcotics, phenylpropanolamine, and monoamine oxidase inhibitors require veterinary review. |
| Dietary management | Discuss diet selection and changes with the veterinarian. | AAHA discusses diets with antioxidants and medium-chain triglycerides. It describes the most positive effect when diet is paired with environmental enrichment, while noting limits in research quality. | Named veterinary diets are not interchangeable endorsements; availability and formulations can change. |
| Supplements or nutraceuticals | Ask the veterinarian before starting one, including over-the-counter products. | Evidence is limited; they should not be treated as proven replacements for medication. | Product formulations vary, and safety or benefit for an individual dog cannot be assumed. |
| Routine and enrichment | Can be adapted at home; seek veterinary advice when pain, frailty, or mobility limits affect activities. | May provide useful engagement and make daily life more predictable. These are supportive management measures, not a cure. | Choose activities suited to the dog’s comfort and abilities; stop or adapt anything that causes distress or pain. |
Medication: what to ask about selegiline
AAHA’s 2023 Senior Care Guidelines for Dogs and Cats identify selegiline as the only drug labeled for canine cognitive dysfunction and say treatment should be considered early. The guideline states that management and treatment appear most effective when started early, which makes prompt assessment useful when a senior dog develops a persistent pattern of change.

The reported efficacy of up to 70% is not a guarantee that a particular dog will improve. Cornell’s owner-facing Senior dog dementia advice says some owners notice improvement within three weeks, while others may see changes after six weeks. Those observations are not a guaranteed onset time; ask the prescribing veterinarian how and when to assess a response.
Drug combinations need careful review. AAHA warns of potentially toxic interactions with antidepressants, narcotics, phenylpropanolamine, and other monoamine oxidase inhibitors. Give the veterinarian a complete list of medications and supplements before treatment. Do not start, stop, or combine medicines without veterinary direction. Anxiety or pain may also need separate attention; a medication used for one of those symptoms is not thereby an established treatment for the underlying cognitive syndrome.

Diet and supplements: options with evidence limits
Dietary approaches
AAHA discusses diets rich in antioxidants and medium-chain triglycerides and reports the most positive effect when dietary management is combined with environmental enrichment. Its guideline also cautions that the quality and rigor of evidence for diets and various supplements are limited. That supports considering diet as one part of a broader plan, not promising that a particular food will stop cognitive decline.

Supplements
Cornell mentions Senilife, Novifit (SAMe), and Aktivait, but advises consulting a veterinarian before adding a supplement. Cornell’s senior-dog guidance notes that few studies have examined supplements in senior dogs. Do not assume a product is harmless, effective, or suitable alongside prescription medication based on its being sold as a supplement.
Environmental changes that can make daily life easier
Environmental support can add engagement and predictability without claiming to reverse dementia. AAHA discusses exercise, new toys, cognitive games, tasks, and agility as enrichment approaches, particularly alongside dietary management. Cornell recommends regular interaction, walks, varied or interactive toys, familiar furniture and bowl locations, and simple food puzzles or snuffle mats for suitable dogs.

A food puzzle or snuffle mat is an optional enrichment tool, not a dementia treatment. Choose one that is easy for the dog to use and supervise its use if needed.

Why assessment comes before choosing a treatment
A veterinarian will interpret the pattern of changes and look for other causes that may be treatable. Cornell describes a workup that can include a physical examination, blood and urine tests, and sometimes MRI. Depending on the dog, the veterinarian may also consider pain, sensory changes, systemic illness, or neurologic disease.

The 2025 Canine Cognitive Dysfunction Syndrome Working Group guidelines define the syndrome through progressive changes across DISHAA domains. Their proposed Level 1 criteria include progressive signs, examinations and laboratory work to investigate alternate causes, and persistence after relevant comorbidities are managed. Proposed Level 2 criteria add MRI evidence and normal cerebrospinal fluid cell counts. These are diagnostic criteria for veterinary assessment, not a home checklist for diagnosing a dog.
A 2025 survey of 318 US veterinarians found that 97.2% had diagnosed canine cognitive dysfunction during their careers, while about 80% rarely or never referred potential cases to a specialist. Around 30% considered selegiline the most effective among the options in the survey. These are reported practice patterns and clinician opinions, not results from a controlled treatment trial or proof of medication efficacy.
Track changes and prepare for the appointment
One difficult night or accident cannot show whether a treatment is working. AAHA recommends directed questions and says client-completed questionnaires can support diagnosis and monitoring; repeat scores can help guide discussion over time.

Quick Recap
- Describe the first changes you noticed and their progression.
- Bring a current list of medications and supplements.
- Ask what other causes should be checked before attributing signs to cognitive dysfunction.
- Ask what improvement would look like, how long to allow before reassessment, and which side effects warrant a call.
- Discuss a safe activity level and whether a diet change fits the dog’s other conditions.
