The FDA record for K181878 lists Kiierr 272 and 148 lasers and gives a September 5, 2018 decision of “Substantially Equivalent (SESE).” The submission summary describes intended use for women with Ludwig-Savin I–II pattern hair loss and men with Norwood-Hamilton IIa–V, for Fitzpatrick skin phototypes I–IV. Substantial equivalence is a regulatory decision that a device is sufficiently comparable to a legally marketed predicate for the stated clearance pathway. The FDA summary says no clinical performance data were generated for this submission because the Kiierr devices were considered identical to the predicate. So the record does not amount to FDA proof that this specific model regrows hair, works for everyone, or guarantees an individual outcome; nor should “cleared” be rewritten as “FDA approved.”
What clinical evidence says about laser caps
A 2021 systematic review and meta-analysis of randomized controlled trials of FDA-cleared home-use LLLT for pattern hair loss found a pooled hair-density improvement versus sham: standardized mean difference 1.27 (95% confidence interval 0.993–1.639). The review’s encouraging category-level finding has limits. Included study durations were under 26 weeks, and the authors called for longer evaluation. They also found insufficient head-to-head evidence to determine which device is best. The reviewed sources do not establish a Kiierr-specific trial result, so the pooled figure cannot be used to promise that the 272 Premier will produce the same result for a particular wearer.

Safety checks before using it
The surfaced Kiierr manual is revision 8.6.2018. It says not to use the cap if the user is under 18, has a condition or takes medication that causes light sensitivity, is sensitive to 650 nm light, or has cancer on the scalp. Guarantee, warranty, and the real purchase decision
Kiierr’s current product page advertises a 210-day consistent-use requirement associated with its growth guarantee and a two-year warranty. The page also describes return conditions, including registration and a before photo, and says a fee may depend on the documentation. These are manufacturer terms that can change; read the live policy carefully before buying, especially what counts as consistent use, the return window, required proof, and any fees. For a buyer, the most defensible case for considering the cap is that home-use LLLT has some supportive evidence as a category and the product has a clearly stated routine. The main uncertainty is model-specific: the FDA submission does not provide clinical performance data for Kiierr, while the broader review cannot identify a best device or forecast an individual response. That makes the cap a considered option—not a proven personal outcome.
