Demystifying Grounded RAG: Reducing LLM Hallucinations with a Local Vector Store

Grounded RAG gives a language model passages to answer from, which reduces hallucinations but cannot eliminate them. Here is how a local vector store fits, what local does and does not cover, and how to test retrieval and groundedness separately.
Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Grounded retrieval-augmented generation (RAG) reduces LLM hallucinations; it does not eliminate them. RAG gives the model passages from your own documents to answer from, which improves the odds of an accurate answer. A local vector store changes where the search index lives and runs. It does not, by itself, make retrieval correct, make the model use the evidence faithfully, or make the whole pipeline private. This guide explains each part of the system, what “local” does and does not cover, and how to test the two things that decide quality: whether the right evidence is retrieved, and whether each answer claim follows from it.

How RAG connects a model to your documents

RAG is a pattern, not a product. The language model is not retrained on your documents. Instead, when a question arrives, the system finds relevant material and places it in the prompt alongside the question. Google Cloud describes grounding as connecting model output to verifiable sources, and AWS’s Prescriptive Guidance on grounding and RAG describes the same retrieve-then-generate sequence.

A working pipeline has four stages. The breakdown below is a practical summary; the vendor guides describe the same pattern in different words rather than publishing a formal standard.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Prepare the sources. Collect documents and extract their text. PDFs, HTML pages, and office files each need parsing. Remove duplicates and superseded versions before anything is indexed.
  2. Chunk and index. Split each document into passages, convert each passage into an embedding (a numeric vector that represents its meaning), and store the vectors with metadata such as source, date, and version.
  3. Retrieve. Embed the user’s question with the same embedding model, search for the nearest passages, and apply any metadata filters.
  4. Generate. Send the question and the retrieved passages to the language model with instructions to answer from them, and return the answer with references to the passages it used.

Each stage can fail on its own, and a later stage cannot repair an earlier one. A model given the wrong passages will answer from the wrong passages, however carefully it is prompted.

What a local vector store is, and what it is not

A vector store holds embeddings and returns the stored passages whose vectors are closest to the query’s vector. It is one retrieval component. Document parsing, chunk size, the embedding model, metadata filters, prompt construction, and the generating model all shape the final answer. A vector store that returns the right neighbours is necessary for a grounded answer, but it does not check whether those neighbours are true.

Semantic similarity is the wrong test for truth. A passage can be about the same topic as a question without answering it. It can be outdated, or it can contradict another passage in the same index. The retriever returns what looks similar, and the system must decide whether that is enough.

Why RAG reduces hallucinations but cannot eliminate them

The strongest case for RAG comes from OpenAI’s API documentation, in its guide “Optimizing LLM Accuracy.” The guide says: “RAG is an incredibly valuable tool for increasing the accuracy and consistency of an LLM – many of our largest customer deployments at OpenAI were done using only prompt engineering and RAG.” That is OpenAI describing the value of the approach, based on its own customer deployments. No measured accuracy figure accompanies the statement, so read it as a statement of value, not a guarantee.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The same guide also warns that wrong context, or too much irrelevant context, can impair an answer and cause hallucinations. That is the central limit of the approach: RAG is only as reliable as what it retrieves and how the model uses it. The residual failure modes are predictable:

  • The corpus is stale, incomplete, or contradictory, so the model receives weak or conflicting evidence.
  • Chunking splits an answer across two passages, or merges unrelated text into one chunk, so the relevant part is diluted or lost.
  • The retriever returns related passages that do not contain the answer, and the model fills the gap from its training.
  • The right passage is retrieved, but the model misreads it, mixes it with prior knowledge, or adds a claim the passage never states.
  • A citation is attached to a sentence that the cited passage does not support.

What “local” covers in a local vector store

“Local” is used loosely. It can describe where the index is stored, where the database process runs, or whether the data persists after the process stops. Qdrant’s documentation describes several arrangements, summarised below. The Edge option carried a beta label on its documentation page in early October 2026.

Mode What runs where Persistence Network exposure Typical fit
In-memory client (Qdrant Python client local mode) Vectors are held in the application’s memory Held in RAM; not kept once the process stops None beyond the application process Experiments and tests
On-disk local client (same client, pointed at a directory) Vectors are stored in files on the local disk Persists between runs None beyond the application process Single-machine applications
Local server (Qdrant local quickstart, Docker) A Qdrant server container on your machine, with storage mounted from a host directory Persists in the mounted host directory Reachable from other machines if the port is published; the default local configuration has no encryption or authentication Development and team testing, after hardening
Embedded, in-process (Qdrant Edge) The engine runs inside your application process Not stated on the Edge documentation page checked in early October 2026 No background service or network is needed for local retrieval Applications that must ship retrieval inside a single process; beta label as of early October 2026

The local server is a development setup by default

Qdrant’s local quickstart runs the server in Docker and mounts persistent storage from a host directory, so the index is kept on the host rather than inside the container. The same quickstart notes that the default local container configuration has no encryption or authentication, which makes network exposure the main risk. Treat it as a development setup, not a production recipe. Before any network exposure:

  • Keep credentials for any hosted embedding or model service out of source code and configuration files committed to version control.

Local retrieval does not mean a private pipeline

A local index keeps stored vectors and similarity search on your own infrastructure, but a RAG pipeline has several other components, and each one may or may not run locally. A vector index can sit on your machine while the embedding model is a hosted API. Or the vectors can be local while answer generation calls a hosted model. Qdrant’s inference documentation separates client-side local inference from externally hosted model options, which is the same distinction to make for your own stack.

Component Can it run locally? Question to answer for your deployment
Source documents and parsed text Yes, if stored on your systems Where do copies, caches, and exports end up?
Embedding model Depends on the choice: a local model or a hosted embedding API Are the question and every passage sent to a third party to be embedded?
Vector store Yes, in the local modes described above Who can reach its port, and are backups encrypted?
Language model for answers Depends: a local runtime or a hosted API Do prompts, retrieved passages, and answers leave the machine?
Logs, traces, and evaluation tools Depends on the tool Are prompts and passages written to logs or to a hosted dashboard?

Describe the pipeline component by component rather than with a single “private” label. A claim about data staying on the device holds only for the parts that actually run there.

Choosing a store on the constraints that matter

No vector store is the universal best choice. Compare candidates on these six axes, using your own requirements for each:

  • Back Up Multiple Computers & Devices – NAS Navigator management utility and PC backup software included. NAS Navigator 2 for macOS 15 and earlier. You can set up automated backups of data on your computers.
  • Deployment boundary. Is the store embedded in the application, a local library or client, a local server, or a managed remote service? Decide whether other devices or services need access.
  • Persistence and recovery. Is a memory-only prototype acceptable? Decide whether you can rebuild the index from source documents or must restore it from backup, and test that path.
  • Workload. Consider document count, vector dimensions, filter use, concurrency, and how often documents change. The documentation does not give a universal size threshold, so measure memory, disk use, and latency on your own corpus.
  • Retrieval features. Vector similarity search, metadata filters, and sparse or keyword-style matching are different capabilities. Product codes, personal names, and error strings often need exact-term matching, so test whether pure semantic search finds them.
  • Framework and language fit. Check client and integration support for your stack, and confirm the same approach works in development and in production. Qdrant’s LangChain integration documentation is one example of an integration page to check.
  • Privacy and operations. Consider authentication, encryption, network access, backups, and monitoring, along with the data-handling points in the previous section.

Testing retrieval and groundedness separately

Before tuning anything, build a fixed evaluation set: a list of representative questions, each paired with the source passage or passages that should answer it. Keep the set stable so that changes can be compared against the same baseline.

Then run two different checks:

  1. Retrieval check. For each question, did the expected passage appear among the returned chunks, and at what rank? Microsoft’s RAG evaluators measure retrieval against retrieved documents and relevance labels, so you need labelled relevant passages to score it.
  2. Groundedness check. For each generated answer, does every claim align with the passages supplied to the model? Microsoft treats groundedness as a separate evaluation from retrieval. Google Cloud’s grounding check works by comparing candidate text with reference facts.

A strong retrieval result does not show that the answer is grounded, and a fluent answer does not show that the right evidence was retrieved. Score both, and read the failures one by one.

Observed failure What it means Where to fix it
The expected passage is not in the top results Retrieval missed the evidence Chunk boundaries, the embedding model, metadata filters, exact-term matching for identifiers, and whether the corpus contains the answer at all
Retrieved passages are off-topic or contradict each other Retrieval precision or source quality problem Remove stale and duplicate documents, tighten filters, and store dates or versions as metadata so they can be filtered
The right passage was retrieved, but the answer misreads or ignores it Generation problem Prompt instructions to answer only from supplied passages, less irrelevant context in the prompt, and a different or better-tested generating model
The answer makes a claim that no passage supports Ungrounded generation Require a citation for each claim, allow an explicit “insufficient evidence” answer, and run a groundedness check
A citation does not support the sentence it is attached to Attribution failure How citations map back to chunks, and whether passages are truncated before they reach the model

Changing the vector database alone will not fix the generation and attribution rows. Make one change at a time, rerun the same evaluation set, and record which component you touched, so that a fix in one stage does not hide a regression in another.
