Start by deciding what business decision the survey will inform; then ask a small set of clear, neutral questions, make response options fit the experience, and explain how results will be reviewed and used.
Start with the decision the survey should support
Before writing questions, define what you need to learn and what could change as a result. Specify the customer group, the product or service interaction being assessed, and the period respondents should consider. A survey about a recent support conversation should name that conversation; a survey about an ongoing relationship should state the time period.

Collect only information relevant to that objective. This keeps the questionnaire focused and makes it easier to interpret the answers. The Government of Canada’s online-survey standards likewise call for minimizing respondent burden and optimizing questionnaires for desktop, tablet, and smartphone. Its 15-minute average-duration standard applies to government surveys and is not a recommended target for every commercial customer survey.
Write questions customers can answer
Ask about observable experiences, use familiar language, and make each question about one idea. Respondents should know which team, interaction, order, or service they are rating. CAHPS guidance for patient-experience surveys emphasizes both identifying the organization or provider and asking about experiences respondents can report; those principles transfer, but healthcare questionnaires should not be copied into another setting without checking their fit.
#1 Best Overall
Use balanced wording and response choices. Avoid questions that praise the service before asking for a rating, presume an experience the customer may not have had, or combine separate issues such as staff courtesy and speed. Offer “not applicable” or “don’t know” when a customer may reasonably lack the experience needed to answer. The UK Magenta Book suggests checking: “Can the respondents understand the question – and do they understand it in the same way that you do?”
Adaptable customer satisfaction survey questions
Use the following as starting points, replacing bracketed text with a specific service, task, or interaction. Keep response scales consistent within a survey and label them clearly.
| Purpose | Example question | Design note |
|---|---|---|
| Overall satisfaction | Overall, how satisfied were you with [specific product, service, or interaction]? | State exactly what experience the rating refers to. |
| Ease | How easy or difficult was it to [complete the task]? | Ask about one task, not several steps or outcomes at once. |
| Timeliness | How satisfied were you with the time it took to [receive a response or complete the task]? | Specify the relevant response or task. |
| Clarity | How clear was the information you received about [specific topic]? | Name the information being assessed. |
| Staff interaction | How satisfied were you with the help you received from [team or person]? | Use a team or person the respondent can identify. |
| Problem resolution | Was your issue resolved during this interaction? | Include “not applicable” for customers who did not have an issue. |
| Open comment | What, if anything, could we have done better? | Make the question optional if a comment is not essential. |
These examples cover distinct experience dimensions—such as clarity, ease, timeliness, staff interaction, and resolution—rather than forcing customers to compress several judgments into one score. The Government Analysis Function also uses the neutral prompt, “What, if anything, did you find difficult about this activity?”
Choose a useful mix and keep the survey manageable
For many feedback tasks, a small number of focused rating questions can track defined aspects of the experience, while one optional open-response question can help explain a rating or identify a specific friction point. Include only items that contribute to the decision you defined. Avoid asking customers to report on matters outside their knowledge.
Test the questionnaire before sending it broadly. Ask representative customers or colleagues to complete it and describe what they think each question means. Cognitive testing can reveal mismatches between intended and interpreted meaning; a pilot can uncover practical problems with the full survey, including unclear instructions, awkward mobile display, or response choices that do not fit. Revise questions where testers interpret them differently than intended.
When to send a customer satisfaction survey
After a specific interaction
For feedback on a support exchange, purchase, visit, or other discrete transaction, invite customers close enough to the event that its details remain available to them. The exact delay depends on the journey and the decision the survey supports; the reviewed guidance does not establish a universal number of hours or days.
For an ongoing relationship
Choose a repeat interval based on how often customers interact with the service and how frequently a meaningful change could occur. A survey sent too often can add burden without producing new information. There is no single best cadence supported for all businesses, so align the interval with the service, customer contact pattern, and intended use of the findings.

Decide which customers are eligible, how they will be invited, and whether the chosen channel reaches the intended group. Track the survey mode and the volume of eligible interactions alongside invitations and responses. OECD describes government user-experience surveys that collect feedback at the point of service interaction; this supports timely collection as a useful approach, not a universal requirement for every commercial survey.

Improve response quality without pressuring customers
A large number of responses does not by itself show that the results represent all customers. Record how many people were invited and how many responded, then consider whether respondents may differ from nonrespondents. Be cautious about generalizing results beyond the group and period actually covered.
Set a reminder approach that respects customer choice and applicable policy. In US federal agency contexts, GSA policy requires an opt-out in survey invitations and reminder emails and disallows targeted individual follow-up without customer consent. This is a government policy, not a blanket rule for every business; organizations should apply the rules and consent requirements relevant to their own operations.
Analyze answers and turn them into improvements
Report results with context
Present the number invited and the number who responded with the results. State the survey period, mode, customer group, and relevant limitations so readers can understand what the findings do and do not represent. Examine response distributions and differences across relevant journeys or customer groups rather than relying only on one overall average.
Use open comments carefully
Read written responses as evidence about customers’ reasons and specific friction points, not as a substitute for the full response set. If comments are grouped into themes, apply categories consistently and retain enough context to avoid stripping away what the customer meant.
Rank #4
Assign ownership and check whether changes helped
For each improvement, name an owner, identify when it will be implemented, and decide what evidence will show whether it helped. The goal is not merely to collect ratings: it is to use them to diagnose a service issue and make a change that can be evaluated.
Choose a survey method or platform that fits the job
Whether using an online survey platform or another administration method, compare options against the practical requirements of the survey. Consider whether the method reaches the intended customers at the right point in their journey, works on the devices they use, supports suitable sampling and response tracking, presents accessible questions, handles data appropriately, and makes findings usable in reporting and improvement workflows. Match the method to audience access, survey complexity, and the evidence needed.
AHRQ guidance covers sampling, collection, response tracking, and reporting while leaving survey users to select an appropriate sampling and administration methodology. GSA documents Qualtrics as its platform for external surveys unless another platform is approved by its Office of Customer Experience; that agency policy is not an independent endorsement or proof that the platform suits every organization.
Common mistakes to avoid
- Asking without a decision in mind: Unused questions add burden and make analysis less focused.
- Combining several judgments: Separate questions about speed, courtesy, clarity, or resolution so respondents can answer precisely.
- Assuming everyone had the same experience: Include an appropriate “not applicable” or “don’t know” choice.
- Using a universal cadence or response-rate target: The reviewed official guidance establishes neither a single best timing interval nor a universal customer survey response-rate benchmark.
- Treating an overall average as the whole story: Review distributions, relevant customer journeys, and limitations as well as summary scores.
- Collecting feedback without follow-through: Assign an owner and a way to assess whether the resulting change improved the experience.
Frequently Asked Questions
What should I ask in a customer satisfaction survey?
Ask a few focused questions about the specific experience you want to improve—for example, overall satisfaction, ease, timeliness, clarity, staff interaction, or whether a reported issue was resolved. Add an optional open question such as “What, if anything, could we have done better?”

When should I send a customer satisfaction survey?
For feedback on a specific transaction or support interaction, invite customers while the experience is still fresh. For ongoing relationships, set an interval that reflects interaction frequency and how often meaningful changes can occur; there is no universal cadence supported for every business.
How many questions should a customer satisfaction survey have?
There is no universally supported number. Include only questions relevant to the decision the survey will inform, keep rating items focused, and test the survey with representative respondents to identify unnecessary burden or confusion.
What is a good customer satisfaction survey response rate?
The reviewed guidance does not establish a universal target. Report invitations and responses, examine whether respondents may differ from nonrespondents, and avoid treating response count alone as proof that results represent the whole customer group.
Should customer satisfaction surveys be anonymous?
The guidance summarized here does not prescribe anonymity for every survey. Decide whether identifying respondents is necessary for the stated purpose, explain how responses will be used, and follow the privacy and consent rules that apply to your organization.
