To create a fillable PDF form in Adobe Acrobat, open All tools > Prepare a form. Start with a blank page, an existing document, or a paper scan; then check the detected fields, add or correct what is missing, preview the form, and save it. A browser-based web form uses a separate Acrobat workflow.
Choose the kind of form you need
Use Prepare a form when you want an interactive PDF that people can fill in. Acrobat can begin with a blank page, an existing digital document, or a scan. If you need an online web form instead, use Acrobat on the web; it is not the same as adding fillable fields to a PDF.

Create a form from an existing document or scan
- Open Acrobat and select All tools > Prepare a form.
- Select an existing file or scan a paper document, then choose Create form.
- Review the fields Acrobat detects. Move, resize, rearrange, or delete fields as needed, and add any missing fields.
- Preview the form to check how it works, then save it.
Automatic field recognition is a starting point, not a final quality check. Inspect every field and its placement before sharing the form. Adobe’s instructions for converting documents to PDF forms describe this workflow.

- Open the form-creation workflow. Adobe’s current desktop instructions use Menu on Windows or File on macOS, then Prepare a form.
- Select Start from blank page and create the form.
- Add the static text and labels that explain what people should enter.
- Insert the interactive fields, preview the result, and save it.

Choose fields that match the answers
Acrobat provides controls including text boxes, drop-down lists, radio buttons, checkboxes, and list boxes. Match the field to the response: use checkboxes when people may select multiple answers, and radio buttons when they should choose one option from a set. Field properties can also make a field required or read-only, and configure behaviors such as dates or calculations.
- Use a text field for a written response.
- Use a drop-down list or list box when people should choose from a defined set of options.
- Use checkboxes for choices that may have more than one selection.
- Use radio buttons for mutually exclusive choices.

Make fields easier to use with assistive technology
Add a clear description in each field’s Tooltip box. Screen readers read that description aloud, helping people understand what information a field requests. Consider accessibility in the source document as well as in field order and navigation. Adobe’s accessible PDF guidance explains these considerations.

Create a browser-based web form
In Acrobat on the web, choose E-sign > Create a web form, select a file to use as the template, and configure participants and fields. This feature requires a valid Acrobat Pro or Acrobat Pro for teams subscription, according to Adobe’s page last updated April 1, 2026. See Adobe’s web-form instructions.

Preview, save, and collect responses
Before sharing a PDF, preview it and verify that fields appear where intended and accept the expected answers. Acrobat supports distributing PDF forms and collecting responses through email or a server; Adobe also documents tracking and managing response data in its PDF forms guide. A fillable PDF is not automatically a browser-based web form.
