Colorlib’s Contact Form V19 is a free, minimalist HTML contact form built around a project-budget dropdown. It can help agencies and service businesses sort enquiries before reading the message, but the template is only the form visitors see: you must connect it to a backend or form service to receive submissions.
What Contact Form V19 does
Colorlib describes V19 as a minimalist form with a round call-to-action button and a budget picker. In Colorlib’s September 28, 2026 roundup, that selector sits between the email and message fields and offers example budget bands from $2,000 to over $25,000. Those amounts are examples, not recommended prices; the options can be customized.

The dropdown is the template’s main distinction. Instead of relying only on free-text messages, it lets a business ask visitors to identify a project budget—or another structured detail—at the start of an enquiry.

| Choose V19 if… | Consider a simpler form if… |
|---|---|
| You need to qualify project enquiries by budget or another selectable category. | You only need visitors to send a name, email address, and free-text message. |
| Your intake process benefits from collecting structured information before follow-up. | A dropdown would add friction or ask for information that is not useful to your team. |
Before adopting it, check that its fields fit the way your business handles leads, decide what the dropdown should ask, and confirm that you can preserve the required credit link.

What the current download includes
Colorlib’s September 28, 2026 roundup says its forms were rebuilt using dependency-free HTML and CSS plus a small amount of vanilla JavaScript. It says they can run on their own or within Bootstrap 4, 5, or 6 projects without loading jQuery, Bootstrap files, or plugins. Colorlib reports a rebuilt V19 download size of 190 KB, compared with 1.3 MB before the rebuild. These are publisher-reported specifications, not independent performance measurements.
The roundup covers 43 free contact-form templates, each with a live demo and free download. Check the current V19 package and its accompanying instructions when downloading, since implementation details can change.
How to make the form send submissions
The downloaded template provides the front end; it does not send email by itself. Colorlib says to connect the form’s action URL to a PHP mail script, a serverless function, or a form service. Its FAQ names Formspree, Basin, and Web3Forms as examples.
- Choose how submissions will be handled. Use a backend you control, a serverless function, or a form service that fits your site.
- Configure the receiving endpoint. Follow that backend or service’s setup instructions to create the endpoint that should receive submissions.
- Set the form’s
actionURL. Point the form to the endpoint you configured. The exact URL and any additional required fields depend on the provider or backend. - Test an actual submission. Confirm that the message reaches the intended inbox or system, and check how errors are reported before publishing the form.
There is no single universal endpoint or setup sequence: the receiving URL and any additional configuration come from the backend or service you select.

The V19 page labels the template CC BY 3.0. Colorlib’s current roundup says personal and commercial use is allowed under that license as long as the credit link is kept. Treat this as Colorlib’s stated license guidance, and review the license and credit requirements presented with the current download before using it.
Do not confuse it with another Colorlib v19 form
Colorlib also has a separate page at Contact Form v19. That lowercase-title variant is described as a basic message form with a Google Maps background and autocomplete; it is not the budget-dropdown form discussed here. The target template is the page titled Contact Form V19.
The target page shows a March 6, 2021 post date alongside a contradictory “Released: 2025” detail. Because those entries conflict, they do not establish a reliable release year. For current implementation claims, Colorlib’s September 28, 2026 roundup is the clearer reference.
