Choose the register-call API when your application needs to keep control of Twilio webhooks and routing. Use SIP trunking when you want to connect a PBX or another SIP-enabled phone system.
Choose an integration path
| Path | Best fit | What you manage | Documented constraint |
|---|---|---|---|
| Native Twilio integration | Standard inbound and outbound calls using a Twilio number | ElevenLabs manages more of the connection setup | Verified caller IDs can be used for outbound calls, but cannot receive inbound calls or be assigned to an agent. ElevenLabs native Twilio integration |
| Register-call | Existing Twilio workflows or custom routing before a call reaches the agent | Your application handles webhooks and returns TwiML to Twilio | Call transfers are not available through this pattern. ElevenLabs register-call guide |

Connect a Twilio number with the native integration
This is the most direct documented route when you do not need a custom Twilio call flow. The integration links a Twilio number to an ElevenLabs agent and supports inbound and outbound calls. A purchased Twilio number supports both directions; a verified caller ID is outbound-only and cannot be assigned to an agent. Check the number’s type and calling capabilities before planning inbound service.

- Confirm the number. In Twilio, identify whether you have a purchased number or a verified caller ID, and confirm it has the capability required for your intended call direction.
- Open ElevenLabs’ phone-number settings. Follow the current native Twilio integration guide to import the Twilio number and link it to the intended agent.
- Test the direction you plan to use. Place an outbound test call and, for a purchased number configured for inbound calling, call it from another phone to verify that it reaches the correct agent.
The managed setup is simpler than building a register-call flow, but it offers less control over custom Twilio infrastructure and routing.
Keep custom call routing with register-call
Register-call is for applications that need Twilio to run their own webhook logic before connecting a caller to an ElevenLabs agent. In an inbound flow, Twilio calls your webhook; in an outbound flow, your application initiates the Twilio call. Your server sends the agent ID and phone numbers to ElevenLabs, receives TwiML, then returns that TwiML to Twilio so the call can connect.

- Mid-level phone, ideal for professionals and managers with moderate call load
- Ergonomic design with adjustable display
- Built-in Bluetooth, Wi-Fi
- Expose a webhook endpoint. Configure Twilio to send the inbound call webhook to your application, or have your application initiate the outbound call through Twilio.
- Request the TwiML from ElevenLabs. Send the required
agent_id,from_number, andto_number. The optionaldirectionparameter acceptsinboundoroutboundand defaults toinbound. See the register-call API reference for the current request schema. - Return ElevenLabs’ response to Twilio. Pass the returned TwiML back as the response to Twilio’s webhook request so Twilio can connect the call.
- Add call-specific context if needed. The flow supports optional conversation-initiation data for dynamic variables and configuration overrides; consult the register-call guide for supported fields.
This arrangement gives the application more responsibility for TwiML routing and audio configuration. Call transfers are not available through register-call because ElevenLabs does not have direct access to the Twilio account credentials. Keep credentials on the server, store them securely, and avoid exposing them in browser code or webhook responses.
Connect an existing PBX with SIP trunking
SIP trunking is intended for existing phone infrastructure rather than a new Twilio-number connection alone. ElevenLabs lists Twilio as a compatible SIP trunk provider, alongside Vonage, RingCentral, Sinch, Infobip, Telnyx, Exotel, Plivo, and Bandwidth. That compatibility does not mean provider settings or features are interchangeable; validate the configuration for your own account and trunk.

- Supports 4 SIP accounts and 4 multi-purpose line keys
- Swappable faceplate to allow for easy logo customization
- GRP2612W includes built-in dual-band Wi-Fi support. Ethernet cord must be disconnected to enable Wi-Fi capability
- HD audio supporting all major codecs, including wideband codecs G.722 and Opus Up to 16 digital BLF Keys
- Enterprise-level protection including secure boot, dual firmware images, and encrypted data storage
Check prerequisites before configuring the trunk
- A SIP-compatible PBX or phone system, phone numbers to connect, and administrative access to trunk settings.
- Support for G.722 or G.711 (PCMU or PCMA), or the ability to resample audio.
- Firewall rules that permit the signaling and media traffic required by the chosen configuration.
- A plan for signaling and media security. The guide recommends TLS signaling and required media encryption for production.
- An authentication method: digest authentication or source-IP access-control lists (ACLs). The guide recommends digest authentication over relying on IP allowlisting when dynamic IP addresses complicate operations.
Choose transport and confirm eligibility
The SIP guide provides URI patterns for TCP and TLS. It describes UDP as experimental for testing, not production. Enterprise static-IP SIP endpoints are described as an Enterprise feature, including trials; confirm eligibility and regional configuration with ElevenLabs before depending on them. A URI example is not a complete provider setup: match its details to the actual trunk and PBX configuration. See the SIP trunking guide and SIP outbound-call API.

Plan tests and costs around your actual setup
Test call behavior and failure points
- Test inbound and outbound separately; a number or caller ID that supports one direction may not support the other.
- For register-call, verify that Twilio reaches the public webhook, that the server sends the correct call numbers and agent ID, and that it returns ElevenLabs’ TwiML response to Twilio.
- For SIP, check codec negotiation, firewall behavior, authentication, transport, and media encryption with the provider’s real configuration.
- Confirm how the application handles webhook failures and calls that cannot connect before routing live traffic.
Estimate costs only after choosing destinations and volume
The integration documentation does not establish an all-in cost. Check current Twilio and ElevenLabs pricing for the destinations, call volume, plans, and features you expect to use; the total depends on those details.

When older outbound-call instructions appear
ElevenLabs’ older outbound-call guide says outbound calling is now supported natively and recommends using the native integration instead. For a standard outbound setup, begin with the native Twilio guide; use register-call only when retaining application-controlled Twilio routing is part of the design.
