Colorlib says the layout changes from side-by-side to vertically stacked on mobile. Its current indexed description also lists vanilla HTML, CSS and JavaScript plus an included PHP mail handler, but those are publisher descriptions—not evidence that the download has been tested or will work unchanged on your site.
What Contact Form V01 includes
Colorlib describes the template as a two-panel contact page: one side contains the form, while the other displays company details. The current indexed description says the template uses plain HTML and CSS with a few lines of vanilla JavaScript, and does not require jQuery, Bootstrap files or plugins. It lists the download at 257 KB, compared with a previously listed 273 KB. These are Colorlib-reported file sizes, not independent measurements; the description identifies the newer information as a September 2026 update.

These details describe the publisher’s stated implementation; they do not establish that mail delivery, validation, or deployment will work in a particular hosting environment.

The official page contains different date fields: the article date is December 23, 2020, its details section says “Released: 2025,” and the indexed description reports an update in September 2026. Treat these as separate publisher fields rather than a single confirmed release date.

The download form asks for an email address and displays a checkbox for consent to receive updates and marketing. Read the form’s terms and make your choice based on your preferences.
Checks to make before adding it to a site
A visual template is only one part of a working contact form. Before publishing, review the code and test it in the actual environment where it will run.
- Hosting and PHP: Confirm that your hosting supports the PHP version and mail configuration required by the included handler. Identify the correct recipient setting in the PHP folder and check how the host handles outgoing mail.
- Submission behavior: Send test messages to an address you control. Check successful delivery, error handling, and what the visitor sees after submitting. The publisher’s description does not establish deliverability.
- Accessibility: Check keyboard operation, focus visibility, label-to-field associations, error announcements, contrast and behavior at narrow widths. No formal accessibility audit or conformance claim is established by the available description.
- Privacy and security: Determine what information the form collects, where it goes, who can access it and how long it is retained. Review server-side validation, abuse prevention and handling of submitted content; the page description does not establish privacy compliance, security controls or spam defenses.
- Compatibility: Test the page in your supported browsers and on mobile devices. The publisher says the layout stacks vertically on mobile, but that is not a substitute for checking your own content, styling and integrations.
Using a different form submission service
If you want submissions handled by a third-party endpoint rather than the included PHP script, expect to adapt the form’s submission configuration and follow that provider’s instructions. A Colorlib support-forum user asked how to use formsubmit.co and where to change the submit action; support directed the user to the third-party provider for implementation details. Before switching endpoints, inspect the form markup and determine how the provider expects fields, actions and responses to be configured. Then test the full submission flow, including validation, success and failure states, and the destination inbox. Do not assume that changing a button label alone changes where form data is sent.

Contact Form V01 is most relevant if you want a downloadable design that you can edit and host as part of a website, and are prepared to check the form handler and maintain the integration. If you would rather use a managed site-building service, that is a different approach: assess whether you want to edit and host code yourself or have site-building and submission handling managed through a service. The available material does not establish a feature-by-feature or price comparison with such services.
