Pomegranate juice has a reported possible interaction with warfarin, but that is not proof it interacts with all heart medicines. Human studies summarized by Memorial Sloan Kettering Cancer Center did not find clinically relevant effects on the drug-metabolizing enzymes CYP3A or CYP2C9. The sources reviewed do not establish whether urolithin A interacts with cardiac medicines. Before starting either as a concentrated supplement, ask your prescriber or pharmacist to check the exact product against your full medication list.
What is known about pomegranate and heart medicines?
The clearest specific concern is a reported possible interaction between pomegranate juice and warfarin, an anticoagulant. Memorial Sloan Kettering Cancer Center advises people taking blood thinners such as warfarin to consult their healthcare provider because pomegranate juice may increase bleeding risk. Its professional summary describes a case report; that is a safety signal, not proof that pomegranate predictably affects everyone taking warfarin or other blood thinners.

Memorial Sloan Kettering also describes a reported case of rhabdomyolysis in a person taking a statin. A single case report does not establish that pomegranate caused the event or that it creates a predictable interaction with statins as a class. These reports warrant medication-specific advice rather than a blanket conclusion about all cardiac drugs.

Animal experiments suggested pomegranate juice could inhibit CYP3A and CYP2C9, enzymes involved in processing some medicines. However, Memorial Sloan Kettering’s summary says human studies did not show clinically relevant CYP3A inhibition, and a human study found no effect on CYP2C9 activity from pomegranate juice or extract. Animal findings should not be treated as established human drug interactions.

These enzyme findings also do not answer whether pomegranate is compatible with every medicine. A person’s specific drug, dose, other medicines, health conditions, and the pomegranate product all matter.
Is urolithin A safe with heart medication?
The sources reviewed do not establish whether urolithin A interacts with anticoagulants, antiplatelet medicines, statins, blood-pressure medicines, or rhythm medicines. The National Cancer Institute lists clinical trials that used urolithin A supplements, but trial listings do not establish that the supplement is safe to take alongside cardiac medication. No interaction evidence found is not the same as evidence of compatibility.

Does juice differ from an extract or supplement?
Yes. Advice about one form should not automatically be applied to another. The National Center for Complementary and Integrative Health (NCCIH) says pomegranate juice is safe and extract may also be safe, but this general guidance does not confirm safety for an individual medication regimen. NCCIH also notes that root, stem, and peel can contain substances that may be harmful in large amounts. A concentrated extract or supplement is not interchangeable with ordinary juice for interaction advice.
NCCIH’s pomegranate summary, published April 5, 2024, says limited research has not shown pomegranate to prevent or treat health problems. Neither pomegranate nor urolithin A should be used as a substitute for prescribed cardiovascular treatment.

What should you do before taking either supplement?
- Do not stop or change a prescribed medicine. Ask your prescriber or pharmacist before adding pomegranate extract, a urolithin A supplement, or a regular juice routine—especially if you take warfarin or another medicine affecting blood clotting.
- Bring the exact product information. Take the container or a clear photo of the label, including ingredients and serving directions. Mention whether you plan to use juice, extract, or another preparation.
- Share your complete medication list. Include prescription and over-the-counter medicines, vitamins, and other supplements. NCCIH advises telling healthcare providers about medicines and supplements because interactions can be serious.
- Ask for a review of your particular regimen. A pharmacist or prescriber can assess your medicines and the product rather than relying on a general claim that a supplement is safe or unsafe.
