Can Engineered Antibodies Make Chemotherapy Safer? What ADCs Can—and Can’t—Do

Antibody-drug conjugates are designed to target cancer cells with potent drugs, but they do not guarantee fewer side effects. Evidence shows a mixed safety profile.

Engineered antibodies may help direct chemotherapy drugs toward cancer cells, but they do not make treatment harmless or reliably safer for everyone. Antibody-drug conjugates (ADCs) are designed to improve drug delivery; clinical evidence shows a mixed pattern of side effects, and risks vary by medicine, cancer, and patient.

What is an antibody-drug conjugate?

An ADC combines a monoclonal antibody that recognizes a target associated with cancer cells with a potent cell-killing drug, often a chemotherapy payload. The antibody is intended to carry that payload toward cells bearing the target, concentrating treatment where it is needed and potentially widening the therapeutic window.

That targeting is not a guarantee that only cancer cells are exposed. Targets may also occur on healthy cells, and an ADC’s payload can contribute its own characteristic toxicities. The National Cancer Institute explains that targeted therapies act on proteins involved in cancer-cell growth, division, and spread, but can still cause serious side effects: NCI: Targeted Therapy for Cancer.

Do ADCs have fewer side effects than chemotherapy?

There is no class-wide answer that ADCs are safer than conventional chemotherapy. In a 2023 systematic review and meta-analysis, Suzuki and colleagues pooled results from 20 randomized trials involving 10,075 patients. Compared with control treatments in those trials, ADC-based treatment was associated with higher odds of several all-grade adverse events, lower odds of febrile neutropenia, and no overall increase in high-grade adverse events.

Selected pooled findings from the review are below. Odds ratios (ORs) compare the odds of an event in the ADC groups with those in control groups; they are not absolute probabilities for an individual or a particular drug.

All-grade event Pooled odds ratio (95% confidence interval) Interpretation in the included comparisons
Fatigue 1.25 (1.08–1.45) Higher odds with ADC-based treatment
Anorexia 1.36 (1.09–1.69) Higher odds with ADC-based treatment
Nausea 1.46 (1.09–1.97) Higher odds with ADC-based treatment
Sensory neuropathy 2.18 (1.27–3.76) Higher odds with ADC-based treatment
Febrile neutropenia 0.46 (0.22–0.96) Lower odds with ADC-based treatment
Thrombocytopenia 2.07 (1.00–4.31) Higher odds with ADC-based treatment
Elevated alanine aminotransferase 2.51 (1.84–3.40) Higher odds with ADC-based treatment
Elevated aspartate aminotransferase 2.83 (2.04–3.93) Higher odds with ADC-based treatment

The review’s pooled results combine different medicines, cancers, and control treatments, so they do not establish that one ADC—or ADCs as a whole—outperforms every chemotherapy option on safety. A sound comparison needs to match the cancer and target, treatment setting, dose and schedule, comparator, and severity of the specific adverse events. It also needs to consider cancer control, not toxicity alone. See the review by Suzuki et al. in JNCI Cancer Spectrum: Toxicity profiles of antibody-drug conjugates for anticancer treatment.

What side effects can ADCs cause?

Side effects depend on the ADC’s antibody, linker, payload, target, dose, and the patient’s circumstances. The pooled findings above include symptoms and laboratory abnormalities, but they cannot predict what a particular person will experience. Tarantino and colleagues note that ADCs can have on-target and off-target effects, as well as potentially life-threatening effects that are not yet fully understood. They write: “Despite being designed with the rationale of expanding the therapeutic indices of conventional chemotherapies, most ADCs have a toxicity profile similar to that of their cytotoxic payload.”

General information about monoclonal antibodies lists possible reactions and complications such as allergic reactions, cardiovascular effects, inflammatory lung disease, and cytokine release syndrome. These are possibilities across antibody treatments, not a list of effects that applies to every ADC. For a specific medicine, its current prescribing information and the oncology team’s advice are the relevant guides. See the National Cancer Institute’s overview: Monoclonal Antibodies.

How are researchers trying to make ADCs safer?

Improving tolerability is an active design and treatment goal, not a settled guarantee. Strategies described by Tarantino and colleagues include optimizing doses and schedules, changing the antibody, linker, or payload, and developing biomarkers and diagnostic tools to anticipate or detect toxicity. These approaches may help refine treatment, but they do not establish that an ADC will be free of serious side effects for an individual patient.

How does biomarker testing affect treatment choice?

The target matters: an ADC can only be a relevant option when the cancer and its target fit the treatment’s intended use. Biomarker testing can help determine whether a targeted therapy may be appropriate. An example or development in one cancer subtype should not be taken as evidence that the same treatment is suitable for another. The oncology team can interpret test results in the context of the diagnosis and available treatment options. The NCI discusses targeted therapies and biomarker testing at Targeted Therapy for Cancer and gives breast-cancer-specific research context at Advances in Breast Cancer Research.
