Can AI Make Your Doctor’s Second Opinion Obsolete? What the Evidence Shows

No—not on the evidence available. AI can help clinicians consider a diagnosis, but current studies do not show that a patient-facing chatbot can safely replace another doctor’s assessment. A 2025 review found generative AI performed worse than expert physicians on diagnostic tasks, and research on doctors working with AI does not prove that AI-only second opinions improve patients’ health.

What the strongest comparison found

A systematic review and meta-analysis by Hirotaka Takita and colleagues, published in npj Digital Medicine on March 22, 2025, assessed 83 studies of generative-AI diagnostic tasks. Across those varied evaluations, pooled diagnostic accuracy was 52.1%. AI performed significantly worse than expert physicians; the overall differences from physicians and non-expert physicians were not statistically significant. Read the meta-analysis.

That is not evidence of equivalence. A non-significant difference does not establish that two approaches perform equally well, and the pooled figure is not a prediction for a particular chatbot, specialty, patient, or second-opinion service. The review combined evaluations of different models and tasks, with studies published from June 2018 through June 2024. Its results describe research performance, not the safety or effectiveness of a consumer service in routine care.

Three very different things called “medical AI”

An AI answer shown directly to a patient is not the same intervention as a clinician using AI during an assessment. Nor is either automatically equivalent to an independent second opinion from another clinician. The FDA emphasizes that a system’s intended use matters: a tool for triage or ruling something out raises different considerations from one meant to help clinicians improve diagnostic accuracy. Clinician using AI A randomized study reported better clinician diagnostic accuracy than conventional resources in its tested AI-collaboration workflow. Read the workflow study. The study does not show that a patient can substitute a chatbot for a second clinician, or that the workflow improves patient outcomes in routine care.
Another clinician’s opinion A second clinician can assess the case as a clinician; the cited AI studies do not test whether AI can replace this role in ordinary patient care. The sources do not compare every form of human second opinion with every AI service or establish which choice is best for an individual case.

Why a correct diagnosis may still be a warning sign

In a medical quiz study involving clinical images and short text summaries, physicians evaluating the AI’s answers reported errors in image descriptions and explanations—even when the final diagnosis was correct. On the most difficult questions, physicians using external resources did better than the AI. The NIH’s account, published July 23, 2024, describes a quiz setting, not a trial of patient care; its value here is showing why a correct-looking answer alone may not demonstrate dependable reasoning. What an accuracy percentage can—and cannot—tell you

Diagnostic accuracy means agreement with a reference standard: the benchmark used to decide what the correct result was. It is not the same as proof that using a tool leads to better decisions or outcomes for patients. When assessing a performance claim, look for the details behind the percentage:

  • The task: Was the AI asked to suggest a diagnosis, triage a case, rule out a condition, or help a clinician?
  • The cases: Were they realistic and representative of the patients and specialty where the tool would be used?
  • The information available: Did the system see the relevant medical history, examination findings, images, and test results?
  • The comparator and benchmark: Was performance compared with experts, other clinicians, or a reference standard—and how was that benchmark chosen?
  • The outcome: Was the study measuring answer accuracy, clinician performance, or patient health outcomes?
  • The intended use and oversight: Who is meant to use the tool, what limitations are disclosed, and is a licensed clinician responsible for the assessment?
  • Privacy and updates: What happens to the information entered, and how are changes to the model or its intended use evaluated?

Could AI still change the second-opinion process?

Possibly, but the evidence points more clearly to AI as a potential clinical aid than as a replacement for a second doctor. The randomized clinician-AI workflow study suggests that collaboration can improve diagnostic accuracy in the setting it evaluated. It does not answer whether an AI service used by a patient alone provides a safe substitute, or whether any accuracy gain translates into better health outcomes.

For a reader seeking another view, the practical distinction is who is accountable for interpreting the case. A chatbot may help organize questions or explain terms, but its output should not be treated as an independent clinical assessment. If considering any AI tool, check its intended user and use, what records it can assess, relevant validation, clinician oversight, privacy practices, and whether patient outcomes—not just quiz scores—have been studied. The cited sources do not establish a particular patient-facing service on those measures.

So, is a doctor’s second opinion becoming obsolete?

No. The evidence reviewed does not show that AI can replace a second clinician’s assessment. It supports a narrower possibility: AI may assist clinicians in some diagnostic workflows, while performance varies by model, task, and evaluation. Until patient-facing systems are shown to work safely in routine care and improve meaningful outcomes, an AI answer is not an evidence-based substitute for another doctor’s judgment.
