Can AI Diagnose Medical Conditions Accurately?

AI can support narrow diagnostic tasks, but its accuracy depends on the tool, condition, data and test. Study scores do not confirm a chatbot diagnosis or guarantee real-world safety.

AI can help identify patterns and support specific diagnostic tasks, but there is no single accuracy score for “AI diagnosis.” Results depend on the tool, condition, task, patient population and test method. A general-purpose chatbot is not the same as a medical device designed for a narrow clinical use, and a strong benchmark result does not confirm that a tool is safe or accurate for your symptoms. Treat chatbot responses as general information, not a diagnosis.

What does “AI diagnosis” mean?

The phrase can describe several different things: a general-purpose chatbot answering a symptom question, software that helps a clinician interpret an image or test, a system that sorts patients by urgency, or a medical device designed for a particular task. Their results are not interchangeable. The U.S. Food and Drug Administration (FDA) says evaluation should reflect a device’s intended use and the task it performs; for example, triage and rule-out functions raise different practical considerations from software intended to improve a clinician’s diagnostic accuracy. FDA’s overview of AI- and machine-learning-enabled medical devices explains this task-specific approach.

That distinction matters when interpreting regulation, too. The FDA’s list of AI-enabled devices covers devices authorized for marketing in the United States. The agency says listed devices have met applicable premarket requirements, with review of safety and effectiveness in light of the device’s intended use and technological characteristics. An entry does not establish that a device is suitable for self-diagnosis or for a use beyond its authorization.

Regulatory classification depends on what software does and how it is intended to be used. In January 2026, the FDA issued final guidance explaining that some clinical decision-support functions may be excluded from the device definition under relevant statutory criteria, while software functions that meet the definition remain subject to applicable digital-health policies. The FDA’s Clinical Decision Support Software guidance page describes the guidance.

What do accuracy studies show?

A 2025 systematic review and meta-analysis covered 30 studies, 19 large language models (LLMs) and 4,762 cases. Across the optimal models in the included studies, reported primary-diagnosis accuracy ranged from 25% to 97.8%; reported triage accuracy ranged from 66.5% to 98%. These are ranges across heterogeneous studies, not estimates of how accurately an AI tool will diagnose an individual patient. The authors judged most studies to have a high risk of bias, commonly because they tested diagnoses using known cases. The review’s PubMed record reports the findings and limitations.

A separate 2024 study in Nature Medicine found that the evaluated state-of-the-art LLMs did not diagnose accurately across all pathologies, performed significantly worse than physicians in that evaluation, and were sensitive to small changes in prompt wording. That result applies to the systems and test design the researchers evaluated; it does not establish how every model, later version or clinical AI device performs. The study in Nature Medicine provides its methods and results.

These findings do not support a universal claim that AI is either as accurate as a doctor or less accurate in every situation. A model given a carefully prepared clinical vignette is facing a different task from a chatbot interpreting an unscripted description of symptoms. Accuracy can also differ by medical specialty, the information supplied, the model version, case selection and the way a study defines the correct answer.

Why a test score may not reflect real-world performance

  • The task determines the meaningful metric. Detection, classification, triage and other tasks have different consequences for false positives and false negatives. Overall accuracy alone may conceal clinically important errors. The FDA says evaluation metrics should fit the task and the way data are collected and presented. FDA’s AI/ML device overview discusses these distinctions.
  • The patients and setting matter. Results can change when patient populations, clinical practice, data quality or health-care infrastructure differ from those represented in testing. The FDA identifies these as factors that can affect real-world performance. Its request for public comment on real-world performance monitoring explains why evaluation cannot stop at a static test.
  • Retrospective tests have limits. Tests on stored cases can establish a baseline, but they cannot predict every behavior in a changing clinical environment. After deployment, performance may shift; the FDA discusses ongoing, systematic monitoring to detect drift and help maintain safety and effectiveness. FDA’s real-world performance discussion covers these issues.
  • Training data may not represent everyone equally. The World Health Organization (WHO) warns that health AI can inherit poor-quality or biased data, including underrepresentation related to race, ethnicity, ancestry, sex, gender identity and age. FDA, Health Canada and the U.K.’s Medicines and Healthcare products Regulatory Agency identify transparency about populations that may be underrepresented as an important principle. Their transparency principles address what users should be able to understand about these systems.
  • Users may over-trust a fluent answer. WHO warns that people may overlook errors or hand off difficult decisions to an automated system. Generative AI can produce false, inaccurate, biased or incomplete health information that sounds plausible. WHO’s 2024 statement on AI for health describes these risks.

How to assess an AI diagnostic claim

When comparing tools or reading an accuracy claim, look for the details behind the number. FDA guidance emphasizes intended use, task-appropriate evaluation, real-world performance and transparency about the human-AI team. These questions help distinguish evidence relevant to a particular use from a broad marketing claim:

  1. What is the intended use, and who is the user? Identify the condition and task the product is designed for, and whether it is intended for a clinician, patient or caregiver.
  2. How was it validated? Check whether testing used retrospective cases, external data, prospective patients or routine clinical deployment, and what reference standard defined the correct result.
  3. Do the study population and setting match the intended use? Look for differences in patient groups and clinical conditions, as well as disclosed subgroup limitations.
  4. What errors does the score hide? For the relevant task, look for sensitivity, specificity, false-negative and false-positive results. Overall accuracy is not enough when the consequences of each type of error differ.
  5. Can a human review the result? Determine whether a clinician can examine the evidence and limitations behind the output, and whether the tool is intended to support or make a decision.
  6. Is the information current? For a U.S. medical device, check the exact product and intended use in the FDA’s current AI-enabled device list. For any deployed tool, ask how real-world performance is monitored for changes.

Can you use a chatbot to assess your symptoms?

A chatbot may help you organize questions or understand general health information, but its response is not a clinical examination and cannot confirm what is causing your symptoms. WHO recognizes diagnosis and patient-guided symptom investigation as possible uses for large multimodal models while warning about inaccurate, biased or incomplete outputs and over-reliance on automation. WHO’s guidance on responsible health AI sets out these potential uses and risks.

For personal symptoms, contact a qualified health professional. Use emergency services for urgent or severe symptoms according to local guidance. Do not delay care because an AI tool says a condition is harmless, and do not start or stop treatment based only on an AI-generated answer.
