AI-agent hallucinations can put data at risk when an incorrect output—or malicious instructions hidden in material the agent reads—can trigger tools or actions with real permissions. A hallucination is a reliability problem; whether it becomes a data exposure depends on what the agent can access, what it can do, and what safeguards stand between its output and those actions.
How an agent can expose data
An AI agent may do more than generate text: it can read files, query databases, run code, or communicate through connected tools. If it has access to private information and authority to take actions, an incorrect or manipulated output can have consequences beyond a wrong answer. OWASP identifies risks including prompt injection, excessive agency, sensitive information disclosure, system-prompt leakage, and weaknesses in retrieval and embeddings in its 2025 OWASP Top 10 for LLM and Gen AI.

That does not mean hallucinations automatically cause leaks. The relevant question is whether the agent’s access and permissions let a faulty decision expose or misuse information.
External content can carry malicious instructions
An agent can ingest websites, documents, and emails that contain instructions intended to redirect its behavior. OWASP calls this prompt injection and recommends treating external content as untrusted data, with clear boundaries between instructions and content. See the OWASP AI Agent Security Cheat Sheet.

- BUILT TO LAST: Made from tough, waterproof, and crush-resistant materials. Manufactured in Sweden and programmed in the USA with the highest security standards.
NIST’s Center for AI Standards and Innovation (CAISI) describes this form of indirect prompt injection as agent hijacking. In a post dated January 17, 2025, CAISI reported that it was frequently able to induce the agents in its evaluation to follow malicious instructions in scenarios involving code execution, database exfiltration, and phishing. Its examples include an agent downloading and running a program, sending cloud files to an unknown recipient, or sending deceptive emails. These are findings from that evaluation, not a measured rate of real-world incidents or proof that every agent is vulnerable in the same way. Read NIST CAISI’s evaluation account.
More authority raises the stakes
OWASP’s LLM06:2025 Excessive Agency includes hallucination or confabulation among possible triggers of excessive agency: a faulty output becomes more consequential when the system can invoke tools or extensions with broad permissions. A read-only assistant has a different risk profile from one that can modify records or send messages.
What “safe” means for an AI agent
There is no binary guarantee that an agent’s data is safe simply because its model has been told not to reveal it. Risk depends on the particular deployment: what private data it can read, whether untrusted material enters its context, which tools can change data or communicate externally, how permissions are enforced, and whether actions and outputs are monitored.

- WORKS WITH 1000+ ACCOUNTS: Compatible with popular accounts like Google, Microsoft, and Apple. A single YubiKey 5 NFC secures 100+ of your favorite accounts, including email, password managers, and more
- FAST & CONVENIENT LOGIN: Plug in your YubiKey 5 NFC via USB and tap it, or tap it against your phone (NFC), to authenticate. No batteries, no internet connection, and no extra fees required
- MOST SECURE PASSKEY: Supports FIDO2/WebAuthn, FIDO U2F, Yubico OTP, OATH-TOTP/HOTP, Smart card (PIV), and OpenPGP. That means it’s versatile, working almost anywhere you need it
- PRIMARY & SPARE KEYS: Just like having a spare house key, we recommend buying two YubiKeys - one for daily use and one as a spare. That way you’ll never get locked out of your accounts
Do not assume that a prompt is an access-control system. OWASP’s LLM07:2025 System Prompt Leakage guidance says: “The system prompt should not be considered a secret, nor should it be used as a security control.” Prompts can be disclosed, and credentials or other secrets placed in them are at risk. Enforce authorization independently of the model.
Safeguards that reduce risk
Limit access to what the task needs
- Grant the agent only the tools and resource access required for its task.
- Scope permissions per tool and separate tool sets for different trust levels.
- Keep access to sensitive data or high-impact capabilities out of the agent’s reach unless the task requires it.
OWASP’s agent security guidance recommends least-privilege access. Smaller permissions limit what a mistaken or hijacked agent can do.
Require authorization for sensitive actions
Put sensitive operations behind explicit authorization, and require human approval for high-risk actions. The sources recommend this as a control principle; they do not prescribe a universal threshold for what every organization should classify as high risk. OWASP discusses human approval in its LLM Prompt Injection Prevention Cheat Sheet.

Keep untrusted content separate from instructions
Treat retrieved and user-provided material as data, not as trusted instructions. Use clear boundaries around that content, and consider processing or validating it separately before it reaches an agent that has tool access. OWASP’s prevention guidance describes quarantining untrusted material in a parser with no tool access as one possible defense pattern; it is a design option, not a guarantee against every attack.

Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Enforce controls outside the model
Apply authorization, privilege separation, and bounds checks deterministically and auditably in the surrounding system. Do not rely on the model to enforce its own access rules or to consistently refuse an unsafe request. Independent checks can help catch problematic outputs before they trigger consequential actions.
Protect memory and outputs
- Isolate memory across users and sessions.
- Set limits on memory size and how long it persists.
- Review memory for sensitive information before saving it.
- Filter outputs for sensitive-data leakage, and monitor actions so they can be audited.
These measures are included in OWASP’s AI Agent Security Cheat Sheet and its 2025 risk guidance. They reduce exposure opportunities; they do not promise that prompt injection or leakage can be eliminated.

How to assess an agent before trusting it with data
Use these questions to evaluate a particular deployment, rather than treating “AI agent” as one uniform security category:
- Data: What private information can it read, and is that access necessary for its task?
- Inputs: Can websites, documents, emails, or user-provided material enter its context?
- Tools: Can it only retrieve information, or can it modify records, run code, or communicate externally?
- Authorization: Are permissions scoped outside the model, and do sensitive actions require approval?
- Oversight: Are memory, outputs, and actions monitored and auditable?
A system that combines broad private-data access, untrusted inputs, and permission to act has a wider risk surface than one whose access and tools are narrowly constrained. The sources do not establish a universal rate of agent hallucinations, hijacks, or data leaks, so a general percentage would be misleading.
