That result comes from one open-label study, not a verdict for every patient. A separate type 2 diabetes trial compared lower doses and found a different weight-loss result, so the two comparisons should not be merged.
CagriSema is an investigational fixed-dose combination of cagrilintide and semaglutide, administered once weekly by injection in the reported trials. Tirzepatide is the comparator in two company-reported trials summarized below. The available announcements do not provide a complete current label-to-label comparison of dosing, warnings, or uncommon serious side effects.

What are CagriSema and tirzepatide?
CagriSema combines cagrilintide, a long-acting amylin analogue, with semaglutide, a GLP-1 receptor agonist. Novo Nordisk describes it as an investigational, once-weekly subcutaneous fixed-dose combination. It is not another name for semaglutide alone. Novo Nordisk’s CagriSema announcement

Which produced more weight loss in the direct obesity comparison?
REDEFINE 4: the closest head-to-head evidence
In REDEFINE 4, an open-label Phase 3 trial, 809 adults with obesity and at least one weight-related complication were randomized to once-weekly CagriSema 2.4 mg/2.4 mg or tirzepatide 15 mg for 84 weeks. The trial’s primary non-inferiority endpoint was not met: CagriSema was not shown to be non-inferior to tirzepatide. Novo Nordisk’s REDEFINE 4 announcement
| REDEFINE 4 measure at week 84 | CagriSema 2.4 mg/2.4 mg | Tirzepatide 15 mg |
|---|---|---|
| Weight loss, treatment-regimen estimand | 20.2% | 23.6% |
| Weight loss, trial-product estimand | 23.0% | 25.5% |
The treatment-regimen estimate reflects outcomes regardless of treatment discontinuation or other therapy, as defined for the trial. The trial-product estimate is an idealized estimate assuming participants remained on assigned treatment and did not add other weight-loss therapies. Keep these measures distinct: they answer different questions about what happened during the study. Both are sponsor-reported results.

The open-label design means participants and investigators knew which treatment was assigned. The comparison is directly relevant to the studied population, doses, and duration, but it does not establish that tirzepatide is better for every individual or in every clinical setting.
Why the diabetes comparison has a different result
REIMAGINE 5 was a separate Phase 3 trial in adults with type 2 diabetes inadequately controlled on metformin, an SGLT2 inhibitor, or both. At week 60, Novo reported 12.4% weight loss with CagriSema 1.0 mg/1.0 mg and 9.1% with tirzepatide 5 mg. HbA1c fell by 1.71% and 1.67%, respectively; CagriSema was reported as non-inferior for HbA1c reduction. Novo Nordisk’s REIMAGINE 5 announcement

This finding does not overturn REDEFINE 4. REIMAGINE 5 involved people with type 2 diabetes, lower doses, a different duration, and a different clinical objective. It should not be treated as a comparison of the full-dose obesity regimens.
What other trial results add
Placebo-controlled weight-loss evidence
In REDEFINE 1, a 68-week Phase 3 trial involving 3,417 adults with obesity or overweight plus a weight-related complication, without diabetes, Novo reported 20.4% weight loss with CagriSema and 3.0% with placebo under the treatment-policy estimate. Under the trial-product estimate, the figures were 22.7% and 2.3%. This trial provides context for CagriSema’s effects but did not compare it with tirzepatide. Novo Nordisk’s REDEFINE 1 announcement

Results in type 2 diabetes versus semaglutide
In REIMAGINE 2, 2,713 adults with type 2 diabetes inadequately controlled with metformin, with or without an SGLT2 inhibitor, received CagriSema 2.4 mg/2.4 mg or semaglutide 2.4 mg for 68 weeks. For the efficacy estimand, HbA1c changed by −1.91% with CagriSema and −1.75% with semaglutide, while weight changed by −14.2% and −10.2%. Under the treatment-regimen estimate, weight changed by −12.9% and −9.2%. This is a comparison with semaglutide, not tirzepatide. Novo Nordisk’s REIMAGINE 2 announcement

The announcements summarized here do not provide a complete comparison of current warnings, contraindications, uncommon serious events, or discontinuation rates for both medicines.
How are they dosed?
The direct obesity trial used once-weekly subcutaneous injections of CagriSema 2.4 mg/2.4 mg and tirzepatide 15 mg. REIMAGINE 5 used CagriSema 1.0 mg/1.0 mg and tirzepatide 5 mg. Those are study doses, not directions to start, increase, or switch medication. Novo describes CagriSema as once-weekly subcutaneous treatment, but the announcements do not establish a complete current titration schedule for either drug.
For dosing decisions, use the current prescribing information that applies in your country and follow a prescriber’s instructions. Do not infer an escalation schedule from trial endpoint doses.

Is CagriSema approved?
Novo Nordisk reported submitting a U.S. New Drug Application for CagriSema for weight management in December 2025 and said at that time that it was not approved in the United States or European Union. In a September 2026 announcement, the company said an FDA decision was expected in the fourth quarter of 2026. These announcements do not establish a final FDA decision or an approved label; check the relevant regulator and local product information for current status. Quick Recap
How to interpret the evidence
- The most relevant direct obesity comparison in the reported evidence is REDEFINE 4, where tirzepatide 15 mg had the larger average weight-loss estimate and CagriSema missed non-inferiority.
- The favorable CagriSema weight-loss result in REIMAGINE 5 belongs to a lower-dose type 2 diabetes comparison and cannot be generalized to the obesity trial.
- The numerical results are sponsor-reported; the announcements are not a substitute for a full peer-reviewed head-to-head report or current product labels.
- Trial averages do not determine which medicine is appropriate for an individual. Eligibility, health history, tolerability, other medicines, and local approval all matter in a clinician-led decision.
