To add product-specific questions or quote requests in WooCommerce, use the current WooCommerce Product Enquiry Form extension. It provides configurable enquiry buttons and forms, and can route enquiries through email, SMS or WhatsApp. If customers need to submit product choices that become part of an order, use a product add-on form instead; that is a different workflow.
Choose an enquiry form or a product configuration form
Start by deciding what should happen after a customer fills in the form. An enquiry asks the store a question or requests a quote. A product configuration form collects choices—such as options or personalization details—that belong with a purchase.

For a quote-led store that needs an embedded form in a call-for-price flow, WooCommerce’s Product Wise Call for Price documentation describes shortcode-based form embedding and names Contact Form 7 and Gravity Forms as examples.

Install and configure the current Product Enquiry Form extension
- Download and install it. Get the Product Enquiry Form ZIP from your WooCommerce account. In WordPress Admin, open Plugins > Add New, upload the ZIP, then install and activate the extension.
- Open its settings. Go to WooCommerce > Settings > Product Enquiry Form > General Settings. Enable the extension and choose the user roles and products for which the enquiry button should appear.
- Set product targeting rules. The extension documents inclusion or exclusion by product, category or shipping class, as well as filters for price range, stock threshold, product type, tags and attributes. Pick the rules that match your enquiry workflow, then preview the relevant product pages.
- Choose button placement and behavior. The button can appear inline with Add to Cart or below it, with documented locations including product, shop and cart pages. You can also hide the price or Add to Cart button. Hide either only if customers genuinely must enquire before buying; otherwise, keep the normal purchase path available.
- Build the form. Documented field types include text, email, number, textarea, dropdown, checkbox, radio and file upload. Name and label each field, decide whether it is required, activate or deactivate it, and set available options for choice fields. Request only information needed to answer the question or prepare the quote.
- Set spam protection and after-submit behavior. The settings document reCAPTCHA v2 and v3, with site-key and secret-key fields, plus a redirect URL option. Confirm the notification and destination behavior in your own configuration before publishing.
- Add a button with a shortcode if needed. The documented shortcodes are
[product_enquiry_button]for a single product,[cart_enquiry_button]for cart enquiries and[enquiry_button ids="1,2,3"]for specified product IDs. Replace the example IDs with the IDs of your products. - Review the customer journey. Check that the button appears only on the intended products and locations, its label makes the action clear, the fields are usable, and submitted enquiries reach the expected store workflow.
WooCommerce’s documentation describes the available configuration, but it does not establish how a particular theme, block setup, plugin combination or store version will behave. Verify those details in your own store; a live installation and submission test was not performed for this guide.

Do not confuse the current extension with the retired one
WooCommerce has a separate, older Product Enquiry Form documentation page that says its product is no longer available on WooCommerce.com and that the page will no longer be updated. That legacy extension added a Product Enquiry tab to product pages and emailed submitted details to the store owner. Its old installation instructions should not be treated as instructions for the current extension. The legacy page suggests Jetpack’s Form Block as an alternative for adding a form to a page.
