It can develop in different settings—including an acute infection, a chronic infection, a diabetic foot wound, or infection in the bones of the spine—so symptoms, tests, and treatment vary. Pain, warmth, swelling, drainage, or fever can be warning signs, but symptoms alone cannot confirm the diagnosis.
What is a bone infection?
Osteomyelitis is the medical term for infection of bone. It is not one uniform condition: the suspected cause, location, duration, and a person’s health all shape how clinicians investigate and treat it. Guidance for a diabetic foot wound, for example, is not interchangeable with guidance for infection in a vertebra or an acute infection that has spread through the blood.

What signs can osteomyelitis cause?
Possible features include localized bone pain or tenderness, warmth, redness, swelling, drainage, fever, or reduced mobility. Presentation varies, and some people may not have all of these signs. None of them alone proves that bone is infected; assessment may require examination, imaging, and microbiological testing. CDC/NHSN surveillance definitions combine specified clinical, microbiological, pathology, and imaging findings for surveillance classification; they are not a complete bedside diagnostic rule.

How do clinicians investigate a suspected bone infection?
Imaging depends on the suspected site
Clinicians use imaging to assess bone and the extent of possible infection. The American College of Radiology’s 2022 appropriateness update describes MRI as highly sensitive for osteomyelitis and discusses alternatives when MRI or CT cannot be obtained. ACR Appropriateness Criteria

Diabetic foot wounds need particular care
NICE advises considering osteomyelitis when a person with diabetes has a local foot infection or a deep or chronic wound. Normal inflammatory markers, a normal X-ray, or a negative probe-to-bone test do not necessarily exclude it. NICE states: “If osteomyelitis is suspected in a person with diabetes but is not confirmed by initial X‑ray, consider an MRI to confirm the diagnosis.” NICE diabetic foot problems recommendations
Samples can help identify the organism
Blood, tissue, or bone samples may help identify the organism and guide treatment. Whether a biopsy is appropriate—and when it should be done—depends on the setting, existing test results, and whether another reliable microbiological source is available. For chronic nonspinal osteomyelitis in adults, a Society of Academic Bone Radiologists consensus describes radiography and MRI as initial steps when feasible, and considers image-guided bone biopsy in selected cases, particularly when MRI suggests infection and surgery or another microbiological source is unavailable. For adults with suspected native vertebral osteomyelitis, IDSA recommends image-guided aspiration biopsy when clinical, laboratory, and imaging evidence supports the suspicion and a microbiological diagnosis has not already been established through specified blood cultures or serologic tests. The guideline describes circumstances in which empiric antimicrobial treatment may be deferred until a diagnostic sample is obtained. This is not a blanket reason to wait: urgent instability or neurologic compromise changes the clinical urgency and requires prompt clinician-directed care. IDSA native vertebral osteomyelitis guideline
IDSA reported that aspiration established a microbiologic diagnosis in 50%–90% of patients in cited studies of native vertebral osteomyelitis when patients were off antibiotics. That range applies to this specific population and context; it is not a general estimate of the yield of all bone biopsies. IDSA native vertebral osteomyelitis guideline

How does the type of osteomyelitis affect care?
- Acute or chronic: Duration and clinical course affect evaluation and treatment. Guidance for acute infection should not automatically be applied to chronic disease.
- Diabetic-foot-associated: A deep or chronic wound may raise concern even when some initial tests are normal; NICE’s guidance addresses this specific setting.
- Native vertebral osteomyelitis: For adults, IDSA’s pathway emphasizes establishing a microbiological diagnosis when appropriate and accounts for urgency, including instability or neurologic compromise.
- Nonspinal chronic osteomyelitis: Imaging and, in selected circumstances, image-guided biopsy can help guide decisions; multidisciplinary care may be relevant.
How is osteomyelitis treated?
Treatment is individualized. It commonly involves antimicrobial medicines chosen for the suspected or identified organism, the infection site, and the patient’s circumstances. Some cases also require debridement or other surgery. Sampling can influence which treatment is selected, and relevant specialists may need to coordinate care.
Antibiotic choices and durations in local protocols are intended for defined clinical settings, not as universal prescriptions. For example, NHS Highland’s guidance covers acute osteomyelitis and septic arthritis caused by hematogenous spread and explicitly excludes other settings; it recommends early specialist advice and says definitive treatment should usually be guided by aspirate or bone biopsy where possible. NHS Highland acute osteomyelitis and septic arthritis guidance NHS Lothian guidance calls for blood cultures and bone or tissue samples, discourages superficial swabs, and notes that debridement may be needed. When should you seek medical assessment?
Arrange medical assessment for persistent or worsening localized bone pain, warmth, swelling, redness, or drainage, particularly if you also have fever or a deep or chronic foot wound and diabetes. Seek urgent care for severe illness, new weakness or other neurologic symptoms, or signs of instability. These symptoms have many possible causes; only a clinician can assess whether osteomyelitis is present and what care is appropriate.
