Chronological age is how long you have lived; biological age is an estimate of how your biology compares with age-related patterns. The two can differ, but biological age is not a single directly measured, universally agreed number: its meaning depends on which measurements and model are used.
What do the two ages mean?
Chronological age is the time elapsed since birth, usually expressed in years. It is a calendar measure.

Biological age is an estimate derived from selected biological features—such as clinical biomarkers or patterns in DNA methylation—to describe how aspects of a person’s biology compare with age-related patterns. It is not a complete reading of every process in the body, and there is no single accepted biological-age value that applies across all methods.

Biomarker comparisons
One approach measures biomarkers and compares a person’s pattern with patterns observed among people of different chronological ages. The estimate depends on the measures included and the reference population. In a 2024 explanation, Columbia University researcher Daniel Belsky described the idea this way: “We use the general population as a reference, and we say, ‘The average 50-year-old looks like this, the average 60-year-old looks like this, the average 70-year-old looks like this.’” The comparison is to a population pattern, not a direct measurement of a person’s entire aging process. DNA methylation clocks
DNA methylation is a chemical marking of DNA that changes over time. Statistical models called epigenetic or DNA methylation clocks use selected patterns of these marks to estimate age-related measures. Some clocks are designed to estimate age; others aim to capture aging pace or predict health-related outcomes. Because their inputs and targets differ, their results are not automatically interchangeable. (National Institute on Aging, 2021; Nature Aging, 2025)
Can biological age be higher or lower than chronological age?
Yes. An estimate may place someone above or below the age-typical pattern used by a particular method. NIA describes “epigenetic age acceleration” as an estimate of epigenetic age above chronological age. That comparison is meaningful only in the context of the clock, its data, and its reference; it does not mean that every organ or system is aging at the same rate.

For example, NIA summarized an analysis of more than 13,000 people across 13 population-based studies. In that pooled analysis, about 5 percent of adults had epigenetic age more than 10 years above chronological age, and that group had a nearly 50 percent higher risk of death. These are group-level findings from that analysis, not a forecast of an individual’s lifespan. (National Institute on Aging, 2016)
What can an age clock tell you—and what can’t it?
A clock gives an estimate from a particular set of measurements and a statistical model. A result may be associated with health outcomes across a group without functioning as a diagnosis or reliable personal prediction. NIA reported a study examining DNA methylation-based biomarkers in more than 3,500 participants in the Health and Retirement Study, a long-term, nationally representative study of Americans aged 51 and older. That sample and finding describe that study; they do not establish clinical meaning for every clock or individual result. (National Institute on Aging, 2023)

- A higher estimate is not, by itself, a diagnosis.
- A clock score is not a guaranteed lifespan forecast.
- A change in one clock does not prove that a person has become biologically younger in every sense.
Showing that an intervention slows aging is difficult: researchers need measures that reflect meaningful change and evidence that those changes translate into health outcomes over time. A short-term movement in a clock alone cannot establish that an intervention has reversed aging. NIA’s explainer quotes epigenetic-clock researcher Steve Horvath describing the research goal: “That’s kind of the Holy Grail in my lab, to identify and validate anti-aging interventions.” (National Institute on Aging, 2021; National Institute on Aging, 2024)

Before comparing scores—or treating one as meaningful—find out what each measure represents. The label “biological age” alone does not tell you whether two results are comparable.

- Used Book in Good Condition
- Inputs: Does the model use clinical biomarkers, DNA methylation, or another kind of data?
- Reference: What population and age range were used to build or calibrate it?
- Target: Does it estimate age, aging pace, or risk linked to a particular outcome?
- Evidence: Is the result supported by group-level associations, or has it been validated for an individual clinical use?
These questions help distinguish an estimate’s intended meaning from what a score may seem to promise. The evidence summarized by NIA and Nature Aging does not establish a consumer biological-age test as a recommended choice.
