Culture grows bacteria from a specimen and can provide a viable isolate for susceptibility testing and further study. MALDI-TOF mass spectrometry often identifies organisms after they have been cultured, while culture-independent tests detect pathogen-associated genetic or antigen targets without first recovering the organism.
What does bacterial culture contribute?
In a culture-based workflow, a laboratory attempts to grow organisms from a patient specimen. If bacteria grow, the laboratory can work with the resulting isolate: identify it, test its response to antimicrobial drugs, and use it for further characterization. Culture therefore answers more than whether a target was detected; it can produce biological material for follow-up testing.

A 2002 perspective by Pierre Houpikian and Didier Raoult described culture’s role in antigenic studies, antibiotic susceptibility testing, experimental models, and genetic studies. That is a historical perspective, not a current clinical guideline, but it illustrates why recovery of an organism can matter beyond initial identification. Read the CDC-hosted perspective.

How does MALDI-TOF fit into identification?
MALDI-TOF mass spectrometry analyzes a microorganism’s mass-spectrometry profile to help identify it. In the workflow described by the FDA, these systems identify microorganisms cultured from human specimens: they are not substitutes for obtaining an organism when the method requires a cultured sample, and they are not home diagnostic devices. The result depends on the organism, the system’s reference database, and the laboratory’s validated workflow.

Quality assurance and integration into laboratory procedures matter. The FDA recognizes CLSI M58, a standard addressing performance, reporting, quality assurance, user verification, and workflow integration for MALDI-TOF identification of cultured microorganisms. FDA record for the recognized standard.
How is a culture-independent test different?
Culture-independent diagnostic tests, or CIDTs, detect a gene or antigen associated with a pathogen rather than growing the organism first. CDC says these tests can be faster, easier, and cheaper to use than traditional culture tests. Their speed can help answer a focused question about whether a pathogen-associated target is present, but a positive result does not necessarily provide a viable isolate.

That distinction can affect what happens next. CDC notes that CIDTs generally do not provide some information used in patient care and public-health practice, including antimicrobial response and information that helps determine whether a pathogen has been seen in other ill people in a way that may signal an outbreak. | Method | What it detects or produces | Viable isolate? | Key use and limit |
|---|---|---|---|
| Culture | Growth of organisms from a specimen | Can provide one if growth succeeds | Supports identification and downstream testing, including susceptibility testing and further characterization. |
| MALDI-TOF | Mass-spectrometry profile of a cultured microorganism | Depends on culture to supply the organism | Identifies cultured microorganisms; result depends on organism coverage, database, and validated laboratory workflow. |
| Culture-independent diagnostic test | A pathogen-associated genetic or antigen target | Does not necessarily provide one | Can deliver faster target detection, but may not supply isolate-level information used for susceptibility work or surveillance. |
What historical MALDI-TOF results can—and cannot—show
A clinical microbiology laboratory in Marseille, France, reported 351,937 non-duplicated bacterial identifications by MALDI-TOF from January 2012 through March 2018. In that laboratory’s data, 28,391 identifications (8.1%) were unidentified or misidentified overall; the yearly unidentified rate fell from 17.7% in 2012 to 3.6% in 2018. The authors reported correct identification of 744 unique bacterial species.

These figures document one laboratory’s experience over a historical period, including changing performance as its experience and database evolved. They are not universal accuracy or sensitivity estimates, and they should not be treated as a current benchmark for every organism, instrument, database, or laboratory. Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Which method should a laboratory use?
There is no universally best method. The choice depends on the specimen and the question the test needs to answer: rapid detection of a particular target, organism identification, antimicrobial susceptibility, strain characterization, or recovery of an isolate for public-health work. A faster result is not automatically a more complete answer.

- TRUSTED TAP SCORE EXPERTISE Backed by Tap Score water quality expertise and scientific standards. Includes guidance for result interpretation and next steps, with optional follow-up testing through certified laboratories.
- When a target must be detected quickly: a CIDT may provide useful speed, depending on the test and specimen.
- When an isolate or follow-up testing matters: culture can provide the organism needed for susceptibility testing and further characterization.
- When an organism has been cultured and needs identification: MALDI-TOF can be part of the laboratory workflow, subject to database coverage and local validation.
These approaches can complement one another. A laboratory may use a rapid test to detect a target and culture when recovery of an organism is needed; the exact approach depends on the clinical or public-health purpose and the laboratory’s validated procedures. CDC’s general description of CIDTs and the FDA’s scope for MALDI-TOF do not establish one algorithm for every pathogen or specimen.
