Not for autonomous high-stakes decisions, based on current evidence. General-purpose vision-language models (VLMs) can describe images, answer questions about video and draft reports, but strong performance on ordinary visual-question-answering or exam benchmarks does not establish that a system is safe for diagnosis, treatment, emergency response or autonomous driving. Suitability depends on the exact task, the consequences of each type of error, performance on representative data, subgroup and operating-condition testing, the human workflow and monitoring after deployment.
Some narrowly bounded assistive uses are promising. They still require validation in the setting where they will be used, explicit uncertainty handling and effective human oversight. A model can be useful without being trustworthy as the final decision-maker.

What “accurate enough” means in a high-stakes setting
There is no universal accuracy percentage that makes a VLM safe. A model that is adequate for sorting routine images may be unacceptable when a missed finding could delay treatment. The relevant question is not “How high is the benchmark score?” but “Can this system perform this defined task, for these people and conditions, with an acceptable and controlled risk of harm?”

Define exactly what the system will do. “Help with radiology” could mean retrieving similar cases, flagging a possible abnormality, drafting a report or recommending treatment. Those are different tasks with different error costs. A useful evaluation specifies the input available to the model, the output it may produce, who reviews it and what action follows.

Count the errors that matter
Overall accuracy gives every item equal weight. High-stakes evaluation must also examine false negatives, false positives, localization mistakes, omissions, hallucinated findings and misleading explanations. A missed spinal cord compression and an unnecessary follow-up scan are not equivalent errors. Confidence scores are valuable only if they are calibrated: a system that sounds certain when it is wrong can be more dangerous than one that abstains frequently.
Separate model performance from deployment readiness
Validity is affected by the equipment, population, institution, geography, language, image quality and workflow in which the system operates. A result obtained under carefully prepared test conditions may not survive changes in input order, phrasing, missing information or rare cases. Deployment readiness therefore includes the surrounding process, not just the neural network.

| Measure | Study result | What it means |
|---|---|---|
| Neuroradiologists’ mean accuracy | 86.2% | Performance of the participating specialists on this case set. |
| Best evaluated VLM | 35% (Gemini 2.0) | The strongest model in this evaluation, not a ranking of every current model. |
| Harmful outputs | Up to 45% of cases | Included errors such as missed pathology, incorrect localization, hallucinated findings and potential treatment delay. |
| Adding the top three differential diagnoses | Only marginal improvement | Offering more possible answers did not resolve the core reliability problem. |
The authors concluded that the systems studied were not reliable enough for neuroradiological clinical decision support without expert oversight. The cases, methods and named model versions matter: this was a 100-case evaluation, not a universal score for all medical images or future model releases. It nevertheless demonstrates why an impressive general-purpose capability does not automatically transfer to diagnostic interpretation.
Why a small diagnostic study still matters
The study exposes several failure modes that an average score can hide. A model may identify the broad body region yet miss a subtle lesion, place an abnormality in the wrong location or invent a finding that is not present. In clinical practice, those errors can change urgency, treatment or follow-up. Testing only common, clean or well-described images would understate the risk from unusual presentations and degraded inputs.
Clinical decisions involve more than reading an image
The 2024 Nature Medicine clinical-reasoning study
Makowski and colleagues evaluated large language models on 2,400 real patient cases involving four abdominal pathologies. This was not a direct VLM image-accuracy comparison, but it addresses a critical boundary: clinical decision-making requires more than answering a licensing-style question. The evaluated systems were weaker than physicians across the studied conditions and showed problems with guideline adherence, laboratory interpretation, information gathering, instruction following and sensitivity to the quantity and order of information.

For a multimodal system, adding an image does not remove those workflow problems. A model may interpret one input correctly yet recommend an unsafe action because it ignored a contraindication, misunderstood the chronology or failed to ask for a missing test. Autonomous clinical decisions therefore require evidence about the entire information-gathering and recommendation process, not just visual recognition.
A bounded assistive example: Flamingo-CXR
A 2024 Nature Medicine study evaluated Flamingo-CXR, a model fine-tuned to generate chest-X-ray reports. The evaluation used United States intensive-care data and Indian inpatient/outpatient data and involved 27 board-certified radiologists. Clinician-edited reports were judged preferable or equivalent to the original clinician reports by at least half of the panel in 53.6% of MIMIC-CXR cases and 71.2% of IND1 cases. Unedited model reports reached 44.4% and 51.2% on those datasets.
These are panel preference or equivalence rates under the study’s rubric, not diagnostic-accuracy percentages. Preferences differed by dataset and region. The authors present the work as proof of concept for assistance: a clinician can edit a draft, but the result does not show that autonomous reporting is safe or that the same performance will hold in another hospital.

Are VLMs ready for autonomous driving?
DVBench’s safety-critical video test
Zeng and colleagues introduced DVBench in a 20 April 2025 arXiv preprint to test difficult driving scenarios, including rare safety-critical events that ordinary driving datasets may underrepresent. The benchmark contains 10,000 human-annotated multiple-choice questions. Fourteen vision-language models were evaluated, and none exceeded 40% accuracy under the paper’s GroupEval strategy.
The authors report improvements after fine-tuning selected models, but those gains do not demonstrate safe road operation. DVBench measures video understanding and reasoning through questions; it is not an end-to-end trial of sensing, prediction, planning, vehicle control, redundancy or public-road safety. A model can answer a clip-based question correctly while failing to react reliably within the timing and uncertainty constraints of a real vehicle.

| Evaluation area | Questions to answer | Evidence to require |
|---|---|---|
| Task and consequence | What exact decision is supported, and what happens after an error? | A written intended-use statement, severity analysis and defined unacceptable failures. |
| Realism and external validity | Do test cases represent the people, devices, institutions, geography and operating conditions of deployment? | Independent, representative test data and evaluation outside development conditions. |
| Error profile | How often does the system miss, overcall, mislocalize, omit or hallucinate? | False-positive and false-negative rates, severity breakdowns, calibration and abstention results. |
| Subgroups and distribution shift | Does performance change for relevant demographic groups, rare findings, new equipment or altered contexts? | Disaggregated results and stress tests using changed populations, inputs and environments. |
| Human-AI interaction | Do reviewers catch errors, or does automation bias make them less likely to check? | Prospective workflow studies measuring human decisions, time, overrides and missed model errors. |
| Lifecycle control | What happens when performance drifts or the model encounters an unfamiliar case? | Monitoring, incident reporting, escalation, rollback and a tested human fallback. |
NIST’s Artificial Intelligence Risk Management Framework 1.0, published in 2023, is voluntary general guidance rather than a VLM approval standard. It emphasizes validity and reliability, safety, robustness, documentation and risk management throughout design, deployment and use. NIST also notes that “Accuracy and robustness contribute to the validity and trustworthiness of AI systems, and can be in tension with one another in AI systems.” The framework is being revised, so organizations should check current NIST material and sector-specific regulations before treating it as an operational requirement.

How to validate a high-stakes VLM before deployment
- Write the intended-use boundary. State the exact input, output, user, setting and decision. Prohibit uses that were not evaluated, such as turning a report-drafting tool into an autonomous diagnosis system.
- Assemble a representative test set. Include normal and difficult cases, rare but consequential conditions, incomplete or noisy inputs and the demographic, geographic and equipment variation expected in practice. Keep an untouched external set for final evaluation.
- Predefine safety metrics. Set separate targets for sensitivity, specificity, false alarms, missed events, localization, calibration and abstention. Weight errors by clinical or operational harm instead of relying on one mean score.
- Run stress and shift tests. Change image quality, camera position, phrasing, information order, language, lighting, hardware and institutional protocols. Record where performance falls outside the validated range.
- Evaluate the complete workflow. Measure how trained users interact with the output, whether they verify source data, how often they override it and whether the tool changes decisions or delays. A “human in the loop” is a hypothesis to test, not a safety guarantee.
- Define uncertainty and escalation. The interface should make limitations visible, allow abstention and route uncertain or out-of-distribution cases to a qualified person. Specify who has authority to make the final decision.
- Monitor after launch. Track error reports, near misses, subgroup performance, input drift, response latency and override patterns. Set thresholds that trigger investigation, model restriction, rollback or suspension.
- Document changes. A new model version, prompt, preprocessing step, sensor or data source can change performance. Revalidate material changes rather than assuming the original evidence transfers.
Common mistakes that create false confidence
Using a general benchmark as a safety certificate
Visual question answering, academic exams and curated demonstrations test selected capabilities. They do not establish reliability under the consequences, time pressure and messy information conditions of a clinical or driving workflow.
Reporting only an average
An average can conceal a dangerous subgroup, a rare-event failure or a high rate of confident hallucinations. Require distributions and error categories, not just a single percentage.
Assuming a person automatically fixes the model
Reviewers can miss errors, defer to fluent explanations or become less vigilant when automation appears reliable. Measure the combined system with realistic users and workload, including false reassurance and alert fatigue.
Recommended Free Tools
Ignoring operational drift
Hospitals change scanners and protocols; fleets change cameras, software and weather exposure; populations and case mix change over time. A one-time validation result is not ongoing evidence.
What the current evidence supports
| Use case | Evidence-based position |
|---|---|
| Autonomous diagnosis or treatment recommendation | Not supported by the cited evidence for general-purpose VLMs; requires task-specific validation, governance and qualified final oversight. |
| Radiology report drafting with clinician editing | A plausible assistive workflow, with study-specific benefits and limitations demonstrated for chest X-rays; not proof of autonomous safety. |
| Rare-event driving decisions from video alone | Current benchmark results do not establish road readiness or end-to-end safety. |
| Administrative or low-consequence visual classification | May be appropriate when errors are reversible and the system is tested for the actual population and workflow. |
Questions an organization should answer before buying or building
- Which exact model version and input pipeline produced the claimed results?
- Were the test cases independent of the data used for development or tuning?
- What are the false-negative, false-positive, abstention and calibration results for the intended population?
- How does performance vary by subgroup, institution, device, language and operating condition?
- What evidence measures the human workflow rather than the model in isolation?
- Who is accountable for the final decision, and what is the documented fallback when the model is uncertain or unavailable?
- What monitoring, incident response, audit trail and version-control processes operate after deployment?
Until those questions have defensible answers, a high benchmark score should be treated as an invitation to investigate, not permission to automate a consequential decision.
