The publisher says it can be used standalone or placed in Bootstrap 4, 5, or 6 projects. Its front-end interactions demonstrate service and stylist selection, time-slot choices, and a running booking summary; connecting it to real appointment inventory requires a back end or booking service.
What Appointment Form V02 includes
Colorlib’s template page, dated September 25, 2026, presents V02 as a salon appointment flow. A customer can select multiple services, choose a stylist from photo cards, and select an appointment date and start time. Services have durations and prices, while choosing a senior stylist adds a per-service surcharge. A sticky summary displays the accumulating price, total duration, and expected end time; a confirmation card echoes the booking details. The page also describes rejecting closed days and inline errors that move focus to the first problem. These are features described by the publisher; the demo and download have not been independently tested. See Colorlib’s V02 page.
Is it actually a Bootstrap form?
No: the published implementation uses HTML, CSS, and vanilla JavaScript, and does not bundle Bootstrap files, jQuery, or plugins. Colorlib says it can be used on its own or in Bootstrap 4, 5, or 6 projects, so “Bootstrap-compatible” is more accurate than “built with Bootstrap.” The distinction matters if you expect Bootstrap classes, components, or version-specific validation behavior to be included out of the box.

Colorlib also lists V02 among ten free appointment form examples aimed at different business types, including clinics, salons, tutoring, and restaurants. Its roundup describes the collection as plain HTML, CSS, and vanilla JavaScript that can be used standalone or in Bootstrap projects; it does not establish that every example has V02’s salon-specific interactions. What the time and confirmation interactions do—and do not—prove
V02 is described as generating start times that fit the combined service duration before closing and around existing bookings. That is useful interface behavior, but a browser-side time choice is not evidence of live access to a salon’s schedule. The cited template description does not establish server-side appointment storage, live inventory synchronization, payment processing, calendar integration, or production security. Those capabilities need a connected back end or booking service.

Before using the form for real appointments, decide which system is authoritative for availability and how it will prevent two customers from claiming the same slot. The browser can present choices and collect details; a server-side system must validate the selected slot at submission and persist the resulting booking. Do not present a confirmation screen as a confirmed reservation unless the connected system has accepted and recorded it.
Adapting V02 in a Bootstrap project
Match the installed Bootstrap version
Start by checking which Bootstrap version the project actually loads. The relevant official guidance cited here spans Forms v5.3, Layout v5.1, and Validation v4.5; these are version-specific references, not a guarantee that every detail applies unchanged across Bootstrap 4, 5, and 6. Compare the template’s markup and classes with the documentation for the version you use.

Keep controls named and correctly typed
Bootstrap’s form guidance covers controls, selects, checkboxes, radios, input groups, floating labels, layout, and validation. Use an appropriate input type for each value and give every control an accessible name, commonly with a visible label. Consult the Bootstrap 5.3 forms documentation when adapting these controls.
Build the layout and form semantics deliberately
Bootstrap’s layout guidance notes that controls generally stack vertically because they are block-level and full-width by default; grid classes can create more complex arrangements. Keep related fields inside a <form>, and explicitly set button types so that an action button does not accidentally submit the form. See the Bootstrap 5.1 form layout guidance.

Associate validation messages with their fields
For Bootstrap 4.5, the validation documentation says invalid feedback should be associated with the relevant field through aria-describedby. Check the equivalent behavior and markup for your installed version rather than assuming that a compatible design automatically provides compatible validation. The Bootstrap 4.5 validation guidance explains that association.
