It should cover the model and the surrounding deployed system—not just the model weights—and connect post-deployment monitoring with incident handling, recovery, appeals, overrides, and, when necessary, decommissioning.
NIST’s AI Risk Management Framework (AI RMF) Core provides voluntary AI-specific guidance. It does not create one universal notification list or deadline. Your final escalation map must reflect the deployment country or countries, sector, data involved, contracts, and the facts of the incident.

What an AI incident response plan must accomplish
A usable plan answers seven questions before an incident occurs:
- Which AI systems, versions, use cases, operators, and user groups are covered?
- What behavior is reportable, including near misses and material negative impacts?
- How can a user, operator, customer, or third party report a concern?
- Who triages the report and who can pause, restrict, roll back, override, or retire the system?
- What evidence is collected, protected, and shared?
- Who must be informed internally and externally, and who approves each message?
- What conditions permit recovery, and how are corrective actions tracked afterward?
NIST AI RMF Manage 4.1 treats incident response as part of continuing post-deployment risk management. Its suggested activities include monitoring feedback, appeals, overrides, decommissioning, recovery, and change management. Manage 4.3 adds that incidents and errors should be communicated to relevant AI actors, including affected communities. NIST states this plainly: “Incidents and errors are communicated to relevant AI actors, including affected communities.”

The framework is not a substitute for your security incident plan, privacy breach procedure, safety process, or business-continuity plan. The AI plan should connect those processes and identify the handoffs.
Define scope and the trigger for a reportable incident
Inventory the systems in scope
List each covered application, model or model family, deployment environment, provider, operator, business owner, intended use, known limitations, and affected populations. Include supporting components such as retrieval, tools, plugins, automated decision rules, human review queues, data pipelines, and interfaces. A model may behave acceptably in isolation while the deployed system creates harm through a faulty prompt, permission, integration, or workflow.
Record version identifiers and the environments in which they run. The same model can require different thresholds when used for drafting low-risk content versus making or recommending decisions about access, safety, employment, health, housing, finance, or essential services.

Use trigger categories, then set local thresholds
Candidate triggers include:
- Materially incorrect, unsafe, discriminatory, privacy-invasive, or misleading outputs.
- Unexpected behavior, loss of a safety control, or performance degradation.
- Adversarial activity, prompt injection, data poisoning, unauthorized access, or model extraction.
- Failures in tools, retrieval, data handling, human review, or automated downstream actions.
- Near misses that could have caused significant harm under slightly different conditions.
- Repeated complaints, appeal outcomes, or monitoring signals showing a previously unknown impact.
- Any event requiring an emergency override, rollback, pause, access restriction, or decommissioning.
NIST describes monitoring as a way to identify degradation, attacks, unexpected behavior, near misses, and impacts. Your organization must define what “material” means, which events are logged for trend analysis, and which require immediate escalation. Document separate thresholds for user safety, privacy, security, discrimination, operational reliability, and legal or contractual exposure.
Make reporting easy and triage consistent
Provide more than one intake route
Give end users, operators, practitioners, customers, affected people, and relevant third parties a clear reporting route. Depending on the system, this could include an in-product report control, a monitored support channel, an on-call contact, an internal ticket queue, and a route for urgent safety or security reports. State which channel is monitored continuously and which is handled during business hours. Route every submission to a record that can be tracked to closure.

Capture the minimum facts at intake
The first record should include, where available:
- Time received, time observed, reporter, and preferred contact method.
- System name, deployment, model and configuration version, and user or operator role.
- Prompt, input, retrieved material, output, tool action, or other artifact relevant to reproducing the event.
- What was expected, what occurred, and whether the behavior is repeatable.
- People, communities, organizations, data, services, or physical processes that may be affected.
- Any action already taken, such as disabling a feature, contacting a person, or preserving a copy.
Do not collect more personal or sensitive data than the investigation requires. Restrict access to incident records and define how confidential, regulated, or security-sensitive evidence is handled.
Run a documented triage sequence
- Acknowledge and protect. Confirm receipt when possible, address immediate danger, and preserve the original report and relevant artifacts.
- Verify the event. Check whether the report concerns the covered system, a connected component, a user error, or an external service.
- Estimate severity. Assess actual and potential harm, number and vulnerability of affected people, duration, reversibility, exploitability, geographic reach, and whether the system is still operating.
- Assign an owner. Name an incident lead and record the decision, rationale, and next review time.
- Escalate. Engage security, privacy or legal, safety, product operations, communications, and accountable leadership according to the threshold reached.
- Set update times. Record when the next internal assessment and stakeholder update are due, even if facts remain incomplete.
Severity labels are useful only when linked to actions. For example, a low-impact reproducible defect may receive a normal engineering ticket, while an active exploit or ongoing threat to people may require immediate access restriction and executive notification. Define these actions in advance rather than relying on a label alone.

Assign roles and decision authority
The plan should name people or functions, not merely departments. Maintain an on-call roster and backup contacts outside the incident document if the document could become unavailable during an outage.
Core internal roles
- Incident lead: coordinates the response, maintains the timeline, sets meeting cadence, and records decisions.
- Model or AI engineering owner: diagnoses model, prompt, data, evaluation, and configuration behavior; prepares rollback or replacement options.
- Security and operations: investigates compromise, access, infrastructure, logging, and availability; applies technical containment.
- Product or system owner: understands the user workflow, dependencies, service commitments, and safe operating modes.
- Privacy, legal, compliance, or safety reviewers: assess obligations, affected rights, contracts, evidence handling, and required notices.
- Communications or customer support: prepares consistent user, customer, employee, regulator, and public updates.
- Accountable leadership: resolves high-impact trade-offs, authorizes major service restrictions, and accepts residual risk when appropriate.
Define who can independently pause a feature or block a risky output path, who can approve a rollback, who can override an automated action, and who can authorize decommissioning. If approval is unavailable, specify an emergency authority and the retrospective review required afterward. NIST’s guidance supports responsibility assignments, appeal and override processes, and decommissioning, but it does not prescribe a universal organizational chart.
Contain the event while preserving evidence
Choose the least harmful effective containment
Possible controls include disabling a tool or integration, narrowing user or geographic access, switching to human approval, reducing permissions, adding a block or validation rule, reverting to a known-good model or configuration, rate-limiting an abuse path, or taking the service offline. State who can apply each control, what dependencies it has, and how users are informed. A containment action that silently moves risk to a different workflow is not a complete fix.

Preserve an investigation record
Capture the incident timeline, relevant prompts or inputs, outputs, retrieved context, tool calls, model and configuration versions, access records, monitoring signals, user reports, decisions, approvals, containment changes, and communications. Protect evidence from alteration and document its source and collection time. Keep collection proportional to privacy, security, employment, confidentiality, and data-retention obligations. The cited NIST material supports tracking incidents, errors, response, and recovery; it does not mandate one universal AI evidence schema or retention period, so define those controls with your legal, privacy, and security functions.

Start with the incident lead or on-call response channel, then notify the role owners needed for the specific risk. A practical internal escalation path is:
- System or model owner and the operations team responsible for immediate controls.
- Security when compromise, abuse, unauthorized access, or adversarial activity is possible.
- Privacy, legal, compliance, or safety when personal data, regulated decisions, contractual duties, discrimination, physical risk, or rights may be involved.
- Product, customer support, or account teams when users or customers need guidance or remediation.
- Communications when external messaging, media interest, or a broad user update is possible.
- Accountable leadership when the event exceeds the documented severity threshold, requires prolonged shutdown, or creates material organizational exposure.
Record the notification time, recipient, information shared, requested action, and next update. Keep the roster current as people, vendors, systems, and reporting lines change.
Who to notify outside the organization
Affected users and communities
Notify people who experienced or may experience the impact when doing so is safe, lawful, and useful. Explain what happened, what is known and unknown, what actions they should take, how to appeal or request correction, and how to obtain updates. Use accessible language and channels appropriate to affected communities; do not assume that a notice to the contracting customer reaches every affected person.
Customers, deployers, providers, and partners
Coordinate with the customer or deployer operating the system, the model or infrastructure provider, data partners, and other AI actors whose systems or obligations are implicated. Contracts may specify notice windows, evidence-sharing duties, service suspension rights, or responsibility for communicating with end users. Follow those terms while checking that they do not conflict with legal requirements or the need to warn people promptly.

Regulators, law enforcement, and other authorities
Determine whether a regulator, supervisory authority, sector body, certification organization, or law-enforcement agency must be contacted. The correct recipient depends on where the organization and affected people are located, the sector, the data involved, the nature of the harm, and applicable contracts and laws. NIST AI RMF does not establish one recipient list for every AI incident.
Do not promise a universal deadline. The official NIST materials used here do not establish a single AI-specific notification period. Have qualified counsel or the compliance lead map applicable privacy, cybersecurity, consumer-protection, employment, safety, critical-infrastructure, and sector rules before an incident occurs.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Write communication that remains accurate as facts change
Prepare an approval path for initial, updated, and closing messages. Every message should distinguish confirmed facts from hypotheses and state when the next update will be provided. Include:
- The affected service, time window, and incident type in plain language.
- What users or partners may have seen or experienced.
- Whether access was restricted, the current operating status, and immediate protective steps.
- What information may be involved, without exposing sensitive investigation details.
- Available support, appeal, correction, refund, safety, or remediation routes.
- A contact point and update schedule.
Coordinate messages across support, status pages, customer contacts, regulators, and public channels. Avoid claiming that an incident is resolved merely because a model was rolled back; verify downstream effects and outstanding user remedies first.
Recover, remediate, and close the incident
- Set recovery criteria. Define the tests, approvals, monitoring results, and user protections required before restoring normal service.
- Restore cautiously. Use a limited rollout, additional human review, reduced permissions, or other safeguards when appropriate.
- Monitor after recovery. Watch for recurrence, delayed effects, abuse migration, appeal volume, and performance changes.
- Provide remedy. Correct records or decisions, reconsider affected cases, answer appeals, and communicate any continuing risk.
- Find root causes. Examine model behavior, data, prompts, tools, access, human processes, incentives, and monitoring gaps rather than stopping at the first technical symptom.
- Track corrective actions. Assign owners and due dates for fixes, evaluations, controls, training, contract changes, and documentation.
- Update the plan. Change thresholds, contacts, runbooks, inventories, and escalation paths based on what failed or proved unclear.
NIST Manage 4.1 and 4.3 connect recovery with change management, tracking, response, and communication. Treat closure as a documented decision, not the disappearance of alerts.
Best Value
Maintain and exercise the plan
Publish the current plan where designated responders can reach it during an outage, including an offline or separately hosted copy if appropriate. Review it after model, deployment, vendor, organizational, legal, or contact changes and after every significant incident. NIST SP 800-171 Rev. 3 assessment material calls for distributing the incident response plan and updating it for changes or implementation problems. The sources do not set a universal exercise frequency, so choose a schedule based on system risk and change rate.
Exercises should test more than a technical rollback. Include a false-positive report, a privacy-sensitive event, an adversarial attack, an affected-community communication, an unavailable decision-maker, and a disagreement about whether to resume service. Record decisions, timing, missing permissions, broken contact routes, and evidence gaps, then assign fixes.

How NIST guidance fits
NIST AI RMF 1.0 was released on January 26, 2023. NIST describes the framework as voluntary and says its Playbook offers suggestions rather than a mandatory checklist. NIST’s AI RMF page also states that the framework is being revised, so check the current edition before adopting a long-lived policy.
Use the AI RMF Core to align monitoring, response, recovery, communication, appeals, overrides, and decommissioning. Use your organization’s legal, privacy, security, safety, continuity, and contractual controls to set the actual deadlines and recipients. NIST’s SP 800-171 Rev. 3 assessment material is useful for plan responsibility, distribution, and updating, but it is not an AI-specific notification law.
