What an AI system can reasonably be expected to do depends on its intended use and the evidence for that particular device; a detection flag is not automatically a diagnosis.
How AI contributes to disease detection
AI-enabled medical software analyzes data such as images or other clinical information and returns an output for a defined task. Depending on the device, that output may identify a feature for review, sort cases by urgency, help rule out a condition, or provide information to a clinician making a diagnosis.

The practical role is set by the device’s intended use—not by the broad label “AI.” Screening, triage, rule-out, diagnostic support, prognosis, and prediction of treatment response are distinct tasks. They involve different decisions and require evidence suited to the task.

Different tasks, different clinical roles
| Task | How the output may be used | What the output does not establish by itself |
|---|---|---|
| Screening | Flag a person or image for further assessment. | That the person definitely has the disease, or that a negative result rules it out in every circumstance. |
| Triage | Help prioritize which cases need attention sooner. | A completed diagnosis or proof that care is improved simply because cases are sorted faster. |
| Rule-out | Support a decision that a condition is sufficiently unlikely for a defined use. | That the condition is impossible or that the result applies beyond the device’s intended population and setting. |
| Diagnostic support | Provide information that a clinician can consider alongside other evidence. | That the software independently confirms a diagnosis or is superior to clinicians in general. |
| Prognosis or treatment-response prediction | Estimate future risk or likely response for a specified use. | That a predicted outcome is certain or that the tool has been validated for another task. |
Where AI is being used
The FDA gives examples of specific AI-enabled devices that analyze retinal images to detect diabetic retinopathy and imaging systems that provide diagnostic information for skin cancer. These illustrate how software can contribute to disease detection; they do not show that all AI systems diagnose autonomously or perform equally across diseases.

Medical imaging is a prominent area, but AI-enabled devices can involve different inputs and clinical workflows. The FDA’s regulatory-science overview says most AI-enabled devices currently on the market are diagnostic. Emerging uses include risk assessment, prognosis, treatment-response prediction, therapy, improved image acquisition, and multiclass classification. Systems that combine radiology, physiology, pathology, demographic, or health-record data also raise questions such as how to harmonize differing data and handle missing information.
What FDA authorization means in the United States
The FDA regulates medical devices, including AI-enabled devices; it does not regulate “AI” as a standalone category. Its risk-based approach considers a device’s intended use and technological characteristics. Depending on the device, a U.S. premarket pathway may include 510(k) clearance, De Novo classification, or premarket approval.

The FDA’s AI-enabled device overview reported more than 1,600 devices authorized for marketing in the United States as of September 2026. This is a count of devices, not an accuracy rate, a measure of how widely clinicians use them, or evidence of improved patient outcomes. “Authorized” is the appropriate general term because the count spans different regulatory pathways; it should not be recast as “1,600 FDA-approved devices.”

According to the FDA, devices on its AI-enabled list have met applicable premarket requirements, including a focused review of overall safety and effectiveness and whether supporting studies were appropriate for the intended use and technological characteristics. That assessment applies to the particular device and specified use. It does not establish performance for every patient, setting, workflow, or different disease.

How to judge whether a diagnostic AI claim fits
A headline claim such as “detects disease” is not enough to tell you what a system does. The relevant question is whether the evidence matches the precise task and conditions in which the device is meant to be used.
- Intended use: Is the software meant to screen, triage, rule out a condition, or support diagnosis? Evidence for one task does not automatically support another.
- Input and setting: Check the data type or imaging modality, clinical setting, and population covered by the evidence. A result for one image type or population cannot simply be carried over to another.
- Study design and reference standard: Consider how performance was evaluated and whether the comparison standard and study conditions fit the proposed use. FDA review considers whether studies are appropriate for a device’s intended use and characteristics.
- Human workflow: Establish who reviews the output and how it informs a decision. A time-saving or prioritization claim is not, on its own, evidence of better diagnostic accuracy or patient outcomes.
- Metrics suited to the task: A measure useful for image detection may not answer whether a prognosis or treatment-response prediction is reliable. Newer AI uses can require different metrics and reference standards.
There is no single comparable statistic in the cited official material showing how much AI improves diagnostic accuracy across all diseases. A number from one device, task, or study should not be presented as a general measure of medical AI.

Why performance and safety require ongoing attention
Evaluation does not end when a device reaches the market. Development, validation, deployment, monitoring, maintenance, and modification can all affect how a system functions over time. Changes to software or the conditions in which it is used may matter, so lifecycle management is part of the safety picture.
Transparency matters, too: healthcare providers, payors, and others interacting with a device need information relevant to risks and outcomes. In June 2024, the FDA, Health Canada, and the UK MHRA published guiding principles for transparency of machine-learning-enabled medical devices. FDA Digital Health Center of Excellence director Troy Tazbaz said in the accompanying FDA announcement that AI can be applied to prevention, diagnosis, treatment, and administrative tasks. The breadth of possible applications is not evidence that any specific device is appropriate for every one of them.

Governance and evidence-generation are also part of responsible health AI. The World Health Organization’s 2024 guidance addresses ethics and governance for large multimodal models in health; its 2021 framework addresses generating evidence through training, validation, and evaluation for AI-based medical devices.
What AI is—and is not—changing
AI gives healthcare teams new ways to analyze complex data and surface patterns that may otherwise require review. In disease detection, its contribution is specific to the device, intended use, supporting evidence, and place in the clinical workflow. A system can support a screening or diagnostic process without replacing the clinician’s broader assessment, and regulatory authorization does not turn a bounded claim into a universal guarantee.
