AI in Hearing Care: What Hearing Aids Can—and Can’t—Do

AI can change how a hearing device processes sound, but its label does not guarantee clearer speech. See what specific evidence shows and how to assess device fit, intended use and safety.

AI can help a hearing device analyze sound and adjust how it processes it, but it is not a guarantee that speech will become clear in every noisy setting. The practical question is what a particular device’s feature does, what evidence supports it, and whether the device suits the person’s hearing needs.

What AI does in hearing care

AI is a broad label, not a single hearing-aid feature. In general, AI-enabled medical devices can analyze large or complex datasets, identify patterns and generate information that may support treatment or make a device easier to use. In hearing products, the relevant question is how a specific feature uses sound or other data to change the listening experience—not whether the product simply advertises AI.

Some noise-reduction systems use computational methods to process sound before it reaches the listener. The goal may be to make speech easier to follow against background noise, but that does not mean a device can reliably isolate a speaker in every environment. A feature’s performance depends on the product, the listening situation and the person using it.

What the label does not establish

  • It does not tell you which sounds a device prioritizes or how its processing works.
  • It does not prove that speech understanding will improve in your own daily settings.
  • It does not mean every device marketed as AI-enabled uses the same approach or has comparable evidence.

The FDA regulates medical devices, including AI-enabled devices, according to their intended use and technological characteristics. That regulatory framework is not, by itself, proof that a particular feature improves outcomes for every user.

What the evidence says about hearing speech in noise

Evidence needs to be read at the level it was collected: a particular device, feature, study and group of participants. A result for one product is not a verdict on all AI hearing aids.

A specific self-fitting example

In a 2024 announcement, the FDA described its authorization of Apple’s Hearing Aid Feature software for compatible AirPods Pro used with an iOS device. The feature uses hearing levels accessed from HealthKit for customization. In a multisite study of 118 subjects with perceived mild to moderate hearing loss, the FDA reported similar perceived benefit for people who self-fitted and those who received professional fitting of the same device. It also reported comparable amplification and a measure of speech understanding in noise; no device-related adverse events were observed in that study.

“Similar” and “comparable” do not mean that self-fitting performed better. The reported findings apply to that feature and study population; they do not establish how other products perform or predict an individual listener’s experience.

A narrow noise-reduction study

A 2023 arXiv manuscript on a mobile deep-learning denoising system streamed to hearing aids reported a 1.6 dB improvement in speech reception threshold and subjective ratings more than 40% higher on specified tests. Those are results reported by that study under its test conditions, not a general measure of AI hearing aids or proof of durable benefit in everyday listening.

What remains an open question

A 2024 NIDCD working-group summary identified real-world outcomes, affordability, privacy, accessibility, the digital divide and the possible role of AI outside the hearing aid as continuing research questions. The technology is developing, but who benefits, in which situations and at what cost are not settled by the examples above.

Hearing aids, OTC devices and PSAPs are not the same

In the United States, the FDA distinguishes hearing aids—which are medical devices intended for people with hearing loss—from personal sound amplification products (PSAPs), which are consumer electronics intended to make sounds louder in particular activities for people with normal hearing.

Category Intended users and use How it is obtained
OTC hearing aid Adults 18 or older with perceived mild to moderate hearing loss; not intended for severe or profound loss. May be bought in-store or online without a medical exam, prescription or audiologist fitting. Some models offer self-fitting through tests, software or a smartphone app.
Prescription hearing aid May be appropriate for people of any age and with any degree of hearing loss, depending on the device’s indications. Obtained through a licensed hearing-care professional, who can program it for the individual’s hearing loss.
PSAP People with normal hearing who want louder sound in particular environments or activities; not a medical hearing aid. Consumer electronic product.

These categories matter when evaluating an AI claim: a consumer amplifier is not interchangeable with a medical hearing aid, and a device intended for mild to moderate loss may not meet the needs of someone with more severe loss.

How to evaluate an AI hearing-aid feature

Use the product’s intended use and evidence—not the AI label—as your starting point. The FDA advises consumers to look at device labeling and to seek professional help when needed.

  1. Check who the device is intended for. Confirm the intended degree of hearing loss and age range. In the United States, OTC hearing aids are intended for adults 18 and older with perceived mild to moderate loss.
  2. Understand the fitting and follow-up. Find out whether the product is self-fitting, whether a professional can help program it, and what follow-up is available if settings or fit need adjustment.
  3. Identify the actual AI feature. Look for a plain explanation of what it processes or changes. Ask whether evidence concerns speech in noise, another measure, or only device usability.
  4. Check technology requirements. Some features may depend on a compatible phone, app or remote. Confirm compatibility and whether you can use the device as intended without a phone.
  5. Read the labeling and purchase terms. FDA says OTC labeling should include warnings, signs of mild-to-moderate hearing loss for which the device is intended, when to seek care, return policy, battery information and whether a phone or remote is required. Check the actual return and warranty terms; OTC rules do not require manufacturers to provide a warranty.

FDA cautions that phrases such as “FDA Registered” or “FDA Certified” can mislead consumers. Do not treat those words alone as evidence that a product is FDA-authorized or that a particular AI feature works well.

Adjustment, limitations and when to seek care

Hearing aids may improve communication in some settings, but they do not restore normal hearing. They can also amplify unwanted background noise. Getting used to a device takes time and practice; fit and program settings may need adjustment, and some people need months to adapt. If an OTC device does not provide enough benefit, consult a hearing-health professional.

Do not rely on an OTC device instead of medical evaluation for sudden hearing change, ear pain or discharge, vertigo, or hearing loss in one ear. These warning signs call for medical evaluation. An OTC device is also not intended to address severe or profound hearing loss.
