AI-Enabled Breaches: A Practical Business Preparedness Plan

AI can amplify some attacks, but business readiness still depends on limiting access, protecting data, and rehearsing a clear breach response.

Your business is better prepared for AI-enabled breaches when it can limit access, spot unusual activity, and respond quickly—not when it simply buys an AI security product. AI can help attackers scale or improve some techniques, and it can create risks for the models and data businesses use. But many serious breaches still involve familiar weaknesses: vulnerable software, compromised credentials, ransomware, and gaps in response planning.

Two 2026 reports show why preparation matters, while also defining what the numbers do—and do not—say about risk to any one company.

What the latest breach figures say—and what they do not

IBM and the Ponemon Institute studied breaches at 602 organizations globally from March 2025 through February 2026. In that study, IBM reported that one in four malicious breaches were AI-enabled, a 56% increase from the prior year, and that AI-enabled malicious breaches averaged $6 million in cost. These are study findings, not a prediction that one in four breaches at every business will involve AI or that any particular breach will cost $6 million.

Verizon’s 2026 Data Breach Investigations Report (DBIR), which analyzed more than 22,000 breaches across 145 countries, illustrates why AI should not distract from established exposure. Verizon reported software vulnerability exploitation as an initial access vector in 31% of breaches, ransomware in 48%, and generative AI bolstering 15 distinct attack techniques. Those figures describe Verizon’s report, not the likelihood of each event at an individual company.

Separately, Verizon’s 2026 Breach Impact Study material analyzed approximately 70,000 cyber-insurance claims. It reported that insurable losses rose from roughly $60,000 in 2019 to around $100,000 in 2024. Those approximate figures concern the claims examined and the stated years; they are not a universal breach-cost estimate.

The practical implication is to plan for a range of incidents. AI may make some attacks faster or more convincing, but it does not replace the need to patch exposed systems, protect accounts, limit access, and rehearse a response.

Where AI changes the risk

Attackers can augment familiar techniques

Generative AI can bolster different attack techniques; that does not mean every AI-enabled attack is novel or that every breach uses AI. A business should assess the outcomes it needs to detect and contain—such as suspicious account access, unexpected data movement, or ransomware—rather than assume that an “AI attack” has one distinctive signature.

AI systems and their data can also be targets

If your business operates or relies on AI systems, include the model, the application around it, and the data used to train or operate it in your risk assessment. IBM’s 2026 report discusses risks such as AI model inversion attacks, which can involve inferring information about training data from a model. The report does not establish that every business using AI faces the same level of risk. Focus on what sensitive information the system can access, what it may reveal in outputs, and which users or vendors can change or administer it.

Build readiness around controls that reduce exposure

The FTC’s small-business guidance emphasizes controls that apply whether an attacker uses AI or not. Prioritize them according to the systems and information your business actually holds.

  • Require multifactor authentication (MFA). Start with email, administrator accounts, financial systems, remote access, and other sensitive services. Additional factors can include authenticator apps, unique passcodes, tokens, or PIV cards. A hardware security key is one possible token-based option; check that it works with your identity provider and the systems employees use.
  • Limit access by role and duration. Give each employee and vendor only the access needed for their work, and remove it when that need ends. Review privileged accounts and third-party access rather than treating vendor credentials as permanent.
  • Encrypt sensitive information. Protect it both in transit and at rest, and understand where copies are held, including with vendors and cloud services.
  • Train staff and practice reporting. Employees should know how to report a suspicious message, unexpected login prompt, or possible data disclosure promptly. A clear reporting route matters more than expecting every employee to diagnose an attack.
  • Keep software and exposure under review. Verizon’s 2026 DBIR found software vulnerability exploitation in 31% of breaches it analyzed as an initial access vector. Maintain an inventory of systems, apply security updates, and prioritize internet-facing and otherwise sensitive systems.
  • Plan for vendor incidents. Understand what access vendors have, how to reach them during an incident, and what support or notification commitments their contracts provide.
  • Write and maintain an incident-response plan. Identify decision-makers, technical contacts, counsel, communications leads, and the people authorized to isolate systems or engage outside help. Update the plan as systems and risks change.

The FTC Safeguards Rule includes written incident-response planning and maintaining an information-security program as risks change for financial institutions covered by that rule. It does not apply to every business; determine whether your organization is covered rather than treating it as a universal requirement.

Make sure the plan works in practice

A list of safeguards is not the same as operational readiness. Test whether your organization can carry out the following actions using its actual staff, systems, and vendor contacts.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Identify who has authority to declare an incident and who can isolate affected accounts, devices, or services.
  • Reach technical responders, counsel, key vendors, and decision-makers outside the systems that may be compromised.
  • Find out which data and people could be affected, including information held by service providers.
  • Preserve relevant evidence while containing further loss, and know when to bring in forensic expertise.
  • Communicate accurately with employees, customers, partners, and authorities when required.
  • Check that any insurance or incident-response service offers support suited to your needs, including clear limits, exclusions, and notification assistance; the presence of a policy or service alone does not show that a response will be covered or handled for you.

What to do when you suspect a breach

  1. Mobilize the response team. Use the contact and escalation process in your plan. Assign a coordinator so technical investigation, legal review, and communications do not proceed in isolation.
  2. Contain ongoing loss. Take prompt steps to stop further access or data exposure. Coordinate containment with technical responders; where practicable, consult forensic experts before altering systems that may hold relevant evidence.
  3. Establish what happened. Determine which systems, accounts, vendors, and information may be involved, what data may have been exposed, and how many people may be affected. Record decisions and preserve relevant evidence.
  4. Consult counsel on notification duties. The FTC says every U.S. state, the District of Columbia, Puerto Rico, and the U.S. Virgin Islands has laws requiring notification of security breaches involving personal information. Which duties apply, to whom, and on what timeline depends on the facts, the information involved, the business, and applicable law. Do not assume there is one deadline that covers every incident.
  5. Notify and communicate as required. Make required reports to authorities and affected people, and give clear, accurate information without making unsupported assurances or exposing people to additional risk.
  6. Consider relevant information sharing. CISA’s JCDC AI Cybersecurity Collaboration Playbook, released January 14, 2025, describes voluntary sharing of AI-related incident and vulnerability information and recommends integrating those processes into incident response. Consider whether its processes fit your organization and incident.

Use a risk-based readiness check

Set priorities by asking what an incident could reach, how quickly you could contain it, and what support you could call on. For each critical system or service, document:

  • Which accounts, data, and business operations it exposes.
  • Whether MFA, encryption, timely updates, and least-privilege access are in place.
  • Which vendors can access it, how that access is limited, and how to contact them during an incident.
  • Who can isolate it, preserve evidence, and approve customer or regulator communications.
  • What the response plan, relevant contracts, and any insurance actually cover—and what they exclude.

Close the most consequential gaps first, then test the response with a scenario that includes a compromised account, a vulnerable system, or a vendor outage. If the company uses AI, include a scenario involving sensitive data accessible to a model or application. The aim is not to predict the next attack technique; it is to make sure the business can limit harm and make sound decisions when facts are incomplete.
