How I Built Incident Memory with Hindsight

MemoryOS puts Hindsight retrieval inside an AI-assisted incident investigation, bringing past experience into view while keeping diagnosis and action subject to engineering verification.

MemoryOS puts a memory lookup inside an incident investigation: before analyzing a new problem, it retrieves relevant past experiences from Hindsight, then gives that context to an AI model alongside the current incident. The goal is to help engineers ask, “Have we seen something like this before, and what did we learn?” It is a development project described by its author, not an independently evaluated production incident-management system.

How MemoryOS uses incident memory

V Chaitanya describes MemoryOS as an incident workspace and copilot. An engineer supplies a service, symptoms, and logs. The application asks Hindsight for relevant prior experiences, combines the returned context with the current incident, and asks Groq to produce a structured investigation. Useful findings can then be retained for later investigations. The intended loop is:

  1. Capture the current incident’s details.
  2. Recall relevant prior experience from Hindsight.
  3. Reason over the current evidence and recalled context.
  4. Produce an investigation with a possible root cause, supporting evidence, and potential actions.
  5. Retain useful outcomes so a later investigation can draw on them.

The design treats memory as part of the investigation, rather than as a separate archive engineers must remember to search. It gives the model historical context; it does not make that context complete or guarantee a correct conclusion.

What Hindsight contributes

Hindsight’s official overview describes an agent memory system with separate memory banks, typed facts and observations, and knowledge pages. It presents three general operations: retain, recall, and reflect. The same overview lists managed cloud, embedded local operation, and deployment to a user’s own cluster as options. Those are vendor descriptions and deployment choices, not independent evidence that Hindsight improves incident response.

In Chaitanya’s example implementation, a request sends the current incident context to a memory recall endpoint, asks for experience and observation memories, and limits the returned context with a token budget. That code illustrates this project’s approach; it should not be taken as a guarantee of the current Hindsight API or as the only way to use the system.

What the example investigation found

Chaitanya reports a demo incident named “Product Search Degradation” affecting catalog-api, with slow searches and intermittent failures. MemoryOS associated it with an earlier inventory-api incident involving database connection-pool exhaustion. Its generated analysis pointed to upstream timeouts and requests waiting for inventory responses.

The demo suggested checking connection-pool metrics, considering a temporary capacity increase where appropriate, restarting if needed, adding circuit-breaker logic, and investigating slow queries. These were reported outputs from the project, not independently verified findings or operational instructions. Engineers would need to validate the evidence and assess the risks in their own environment before taking action.

Chaitanya says the latest investigation recalled 26 memories. That is a figure from one author-reported demo run in 2026, not a benchmark or evidence of better incident outcomes.

What the project does not automate

Chaitanya describes MemoryOS as a development project, not a complete enterprise incident-management platform. Incidents are entered through the app rather than collected automatically from production monitoring. The usefulness of an investigation depends in part on whether the recalled memories are relevant. Engineers must verify evidence before acting, and MemoryOS does not automatically change infrastructure. The implementation is configured for development with locally running services.

The author names direct observability integrations, monitoring alerts, and chat workflows as possible future directions. They are not capabilities established for the described project.

Why retaining outcomes can matter

Incident memory is useful only if it preserves information that can help with later work. Google Cloud describes a postmortem as a written record of an incident’s impact, mitigation or resolution actions, root causes, and follow-up actions, with learning rather than blame as its goal. That provides a useful operational context for keeping investigation outcomes, but it does not establish that MemoryOS captures postmortems accurately.

For broader operational practice, Google’s Site Reliability Workbook resource page describes the book as a practical companion to SRE principles. Google also says its SRE books are available free online and points readers to material on incident management and postmortem culture. Neither resource is a component of MemoryOS.

How to interpret the project

MemoryOS demonstrates a focused idea: put retrieval of prior incident experience directly into an AI-assisted investigation. Its reported example shows how a past service problem might provide a useful lead when a related symptom appears elsewhere. It does not establish diagnostic accuracy, production readiness, or improved incident outcomes. Chaitanya’s summary captures the aim: “Investigate once. Remember the resolution. Use that experience when the next incident happens.”
