How to Document AI-Generated Code So a Team Can Maintain It

Document AI-assisted code through normal engineering records: explain intent, disclose material AI help, name human owners, record real validation, and preserve maintenance context.

Document AI-generated code as an ordinary engineering change, with a clear record of its purpose, the AI’s material contribution, human ownership, and the checks actually performed. A label alone does not explain how the code works or make it maintainable; the team still needs to understand, review, test, and support what it accepts.

What to record for each AI-assisted change

Use the team’s existing pull request (PR), commit, review, test, and design-document workflow. A PR description or equivalent change record should give future maintainers the context they need without claiming that every line was generated by AI or requiring a separate paperwork system.

  • Intent: State the requirement or problem the change addresses and the expected behavior.
  • AI assistance: Identify which parts were materially generated or modified with AI, using the team’s agreed convention. The U.K. Home Office engineering standard offers [AI-assisted] in a commit message as one example; it is not a universal requirement. Home Office standard
  • Human ownership: Name or identify the accountable owner and the reviewer or approver, following the team’s normal review record. The Home Office says teams retain full accountability, and OWASP states that AI-generated code must have a human owner. Home Office standard · OWASP Cheat Sheet Series
  • Validation: List the tests, build checks, static analysis, security scans, and dependency checks actually run, with outcomes or relevant failures. Do not imply that a check passed if it was not performed.
  • Maintenance context: Capture important assumptions, constraints, design choices, edge cases, and known limitations that are not obvious from the code.
  • Dependencies and provenance: Identify new or changed packages and note the result of the usual checks for existence, suitability, maintenance, security, and license compatibility.

This is a practical pattern, not a universally mandated template. The sources support traceability and understandable code, but do not require an AI label on every generated line or prescribe one documentation format.

Put each explanation where it will remain useful

Pull request: explain this change

Keep change-specific intent, AI assistance, ownership, review, validation results, and limitations in the PR or equivalent change record. This makes the decision and evidence inspectable alongside the code being reviewed.

Commit: make the history searchable

If it fits the team’s conventions, use a commit marker such as the Home Office’s [AI-assisted] example. A marker can help people find relevant history, but it does not replace the PR’s explanation or review evidence.

Project documentation: preserve decisions that outlast the PR

Put durable architectural decisions, system-wide assumptions, or contribution rules in the relevant project documentation or an architecture decision record. Avoid copying a one-off PR narrative into permanent documentation unless it explains an ongoing design choice.

Code comments: explain non-obvious behavior

Use comments for implementation details that a maintainer cannot reasonably infer from the code, not as a substitute for readable code or a change record. The goal is to preserve useful reasoning close to where it will be needed.

Review and validate the code before accepting it

AI assistance does not lower the team’s normal bar for review or testing. The Home Office standard calls for review and approval before production and testing under existing engineering standards. GitHub advises running automated tests and static analysis first; Microsoft Learn says to read and understand every change before accepting it and to test AI-generated code at least as thoroughly as hand-written code.

  1. Check comprehension: Read every material change and ensure a human owner can explain what it does. Do not accept code that the team cannot maintain.
  2. Check fit: Compare behavior with the requirement, architecture, project conventions, naming, readability, and documentation. GitHub’s guidance also cautions against accepting code that is hard to follow or would take longer to refactor than to rewrite. GitHub Docs: Review AI-generated code
  3. Build and test: Compile or otherwise validate the build, run relevant automated tests, and inspect new warnings. Add integration or edge-case checks where the change calls for them.
  4. Inspect security and dependencies: Look for hallucinated APIs, ignored constraints, suspicious package suggestions, and unsafe behavior. Run the security, static-analysis, dependency, and integration checks appropriate to the change.
  5. Record evidence: Put the reviewer and owner, checks performed, outcomes, and remaining limitations in the change record. The DoD AI4SDLC rulebook describes evidence such as PR review, test acceptance, scan results, dependency review, and provenance review. DoD AI4SDLC rulebook

Scale the record to the change’s risk

A small, low-impact change may need only a concise PR record and the team’s ordinary checks. A security-sensitive or high-impact change calls for more scrutiny and more inspectable evidence, such as deeper security and dependency review or explicit provenance review. The DoD rulebook is aimed at U.S. defense software acquisition and governance, so its evidence model is particularly relevant to high-assurance settings—not a universal requirement for every team. The Home Office standard likewise applies within its U.K. departmental context; other teams should adapt its examples to their own policies.

Whatever the risk level, keep the record proportionate: prioritize information that helps someone understand the decision, verify the result, or maintain the code later. A team can use a commit marker, a PR template, or a broader AI-use register, but the cited guidance does not establish one universally required approach.
