What Documentation Do Universities Require for ADHD Exam Accommodations?

Universities vary in the ADHD documentation they accept. Learn what records may help, why functional impact matters, and what to ask before paying for a new evaluation.

U.S. universities do not use one universal ADHD documentation checklist. Most ask for evidence of a diagnosis and, crucially, an explanation of how current ADHD-related limitations affect academic testing and why each requested accommodation would address them. Accepted documents and recency rules vary, so check your own university’s disability-services requirements before arranging a new evaluation.

What documentation is commonly requested?

Universities may accept different kinds of records, but documentation is most useful when it establishes both the student’s current needs and the connection between those needs and the requested exam support. Depending on the school, the file may include:

  • A clearly stated ADHD diagnosis and the evaluator’s relevant qualifications.
  • Current symptoms and relevant clinical, developmental, educational, or treatment history.
  • An explanation of functional limitations in college or testing settings, rather than diagnosis alone.
  • A specific rationale connecting each requested accommodation to a limitation or access barrier.
  • Accommodation recommendations, with reasons for each recommendation.
  • The evaluator’s signature and contact or credential information; some schools specify dated letterhead or signed documentation.

These are common themes, not a mandatory national checklist. The University of Tampa, for example, describes provider documentation covering diagnosis, current symptoms, assessment and contact history, functional impact, and accommodation rationale (University of Tampa documentation guidelines). The University of Colorado Boulder lists several possible documentation types, while Stanford asks for a comprehensive evaluation for ADHD (Colorado Boulder documentation guidelines; Stanford ADHD guidelines).

Why do requirements vary by university?

Each disability-services office sets its own documentation process and considers requests in context. Some campuses accept a clinician letter or provider form; others request a detailed psychoeducational or neuropsychological evaluation. A diagnosis or a record that supports one accommodation does not automatically establish the need for a different one. The office may consider the student’s functional limitations and the specific course, program, or testing situation.

Campus examples illustrate the range; they are not a national rule:

University Example of its ADHD documentation approach Recency information stated
University of Virginia Its criteria address current functioning, qualified clinicians, symptoms consistent with diagnostic criteria, academic limitations, treatment impact, previous accommodation history, and specific recommendations. It advises students not to delay contacting its disability center because paperwork is incomplete. Evaluations submitted for a service request should have been conducted within five years.
University of Colorado Boulder Lists multiple possible records, including a clinician letter, medical or education records, evaluation reports, and proof of standardized-test accommodations; review is individualized. Records from the last three to five years may be requested for ADHD and some other conditions.
University of Tampa Describes a signed provider document with diagnosis, current symptoms, assessment and contact information, functional impact, and accommodation rationale. Not stated in the cited guidance.
University of Delaware Its evaluation guidance describes a comprehensive report with diagnosis, symptom and academic history, prior accommodations, relevant medical and psychosocial history, an interpretive summary, and appropriate assessment instruments. Not stated in the cited guidance.
Stanford Calls for a comprehensive psychoeducational or neuropsychological evaluation with appropriate norms and postsecondary recommendations. Not stated in the cited guidance.

Details above come from the universities’ guidance: Virginia, Colorado Boulder, Tampa, Delaware, and Stanford. Policies may change, so confirm current instructions with the campus office.

Will an IEP, 504 plan, or prior testing approval be enough?

For campus accommodations, high-school records and prior testing accommodations can help document history, but universities differ on whether they are sufficient. Colorado Boulder includes IEPs and Section 504 plans among possible records. Stanford says those plans alone do not satisfy its ADHD documentation process, though they may add useful context when submitted with formal documentation. Virginia describes state-specific provisions concerning IEP evidence; that is not a rule to apply to every state or university.

Prior plans therefore may support a request without guaranteeing approval. Ask the campus office whether it accepts the records you already have and whether it needs additional or more current information.

How recent does an ADHD evaluation need to be?

There is no single recency window across universities. In the examples above, Virginia specifies that submitted evaluations should be within five years of the service request, while Colorado Boulder says records from the last three to five years may be requested for ADHD and some other conditions. Other cited university guidance does not state a time limit. Ask the relevant office how it applies its policy to an existing evaluation before assuming it has expired or arranging a new one.

Are university exams different from the SAT, GRE, LSAT, MCAT, or licensing tests?

Yes. A university’s process for exams within its courses is separate from an external testing entity’s process for an admissions, standardized, licensing, or certification exam. The U.S. Department of Justice’s guidance concerns testing entities covered by the ADA testing rules, not a single documentation checklist for every campus. It says documentation requests should be reasonable and limited to the need for the requested accommodation (DOJ testing-accommodations guidance).

For external tests, DOJ guidance says testing entities should give considerable weight to accommodations received in similar testing situations and those in an IEP or Section 504 plan. If a candidate documents receiving the same accommodation on a similar standardized exam and certifies a continuing need, the entity should generally grant it without asking for further documentation. Check the specific testing program’s current rules; those provisions should not be confused with campus exam procedures (DOJ testing-accommodations guidance; ADA Title III regulations).

What should you do before requesting accommodations?

  1. Find the current documentation page for your university’s disability-services office. Look for an ADHD form, provider instructions, and a description of how to submit a request.
  2. Ask whether your existing records are current and sufficient before paying for a new assessment. Be specific: ask whether the school accepts a treating-provider letter, an older evaluation, an IEP or 504 plan, or records of prior exam accommodations.
  3. Tell your provider which exam support you are requesting. Ask them to describe the relevant functional barrier and explain how the requested support addresses it, rather than submitting only a diagnosis.
  4. Submit the request through the campus office and follow its review process. A clinician’s recommendation informs the request but does not by itself determine the university’s decision.
  5. If the exam is administered by an outside testing organization, check that organization’s accommodation process separately.

Some campuses may discuss next steps even when a student’s paperwork is incomplete. Virginia, for example, says students should not delay meeting with its disability center and describes limited provisional accommodations in some circumstances. Ask your own university whether it offers anything similar; availability and terms are institution-specific.

What to check on your university’s policy page

  • Which document formats are accepted, such as provider letters, forms, or comprehensive evaluation reports.
  • What evaluator qualifications are required.
  • Whether there is a recency standard.
  • What evidence of current academic or test-taking impact is requested.
  • Whether prior plans or exam accommodations count, and whether they are sufficient alone.
  • Whether the provider should explain the rationale for each requested accommodation.
