In a phase I study, all nine participants with high-risk clear cell kidney cancer had no reported recurrence at a median 40.2 months after surgery. The finding is encouraging, but the study was too small and had no control group, so it cannot show that the personalized vaccine prevented the cancer from returning.
What the kidney cancer vaccine study found
The trial, registered as NCT02950766, enrolled people with high-risk stage III or IV clear cell renal cell carcinoma after their tumours had been completely removed. Researchers reported no recurrence among the nine participants at a median of 40.2 months after surgery. The result, published in Nature, describes an outcome observed in this group; it does not establish what would have happened to the same patients without vaccination.
This was a study of clear cell renal cell carcinoma, not every kidney cancer type, and it took place after surgery rather than as a treatment for a tumour still present.
How the personalized vaccine was made
Researchers designed each vaccine around predicted neoantigens—features arising from mutations in that participant’s tumour. They synthesized a median of 15 neoantigen-containing peptides per participant. All nine participants received a successfully manufactured vaccine.

Five participants also received ipilimumab near the vaccination site; the remaining participants received the vaccine without it. Because the trial was small and not designed to compare these groups, it cannot determine whether adding ipilimumab changed outcomes.
Immune responses and side effects
All nine participants generated vaccine-specific T-cell responses, and seven had measured T-cell reactivity against their own tumour. The researchers also reported expansion of peripheral T-cell clones and responses to vaccine antigens that included selected kidney cancer driver mutations. These are laboratory measures of immune activity, not proof that the vaccine improved recurrence or survival.

Low-grade injection-site reactions occurred in all participants, and eight experienced transient flu-like symptoms. No dose-limiting toxicities were observed. A nine-person phase I trial cannot establish the treatment’s full safety profile or rule out less common or more serious harms.

The absence of reported recurrence in nine people is a promising signal that supports further study, not evidence that a vaccine can reliably prevent kidney cancer from returning. The trial had no randomized control group, and its authors said direct comparisons with randomized phase III adjuvant studies were not possible. Its findings do not establish efficacy, prove causation, or show that the outcome applies to other kidney cancer subtypes or patients.

The vaccine was individually manufactured for trial participants. The published account does not describe it as a treatment available to buy or receive routinely. David Braun, a study author, said the vaccine was feasible to manufacture for all participants and produced durable ex vivo immune responses that lasted months or years after vaccination ended, according to a Nature Reviews Nephrology research highlight.
